[Diverticulosis of the digestive tract in our patients over a 10-year period].

Incidence, localization and clinical characteristics of diverticulosis of the digestive tract detected within the period 1971-1980 are analysed. Of 12.352 examined patients, diverticulosis was found in 425 (3,44%). The largest number was detected radiologically and some endoscopically. Diverticulosis was most commonly found in: colon 355, duodenum 42, eosophagus 20 and small intestine 16 (of them 11 with Meckel's) and most rarely in the stomach, 12 patients. Symtomatology was non-characteristic. The treatment was conservative, except in the case of complications. Complications occurred in 39 (9,2%) patients. Due to complications, Meckel's diverticuli were actively searched for and all cases were operated on.

[Hepatocellular carcinoma--retrospective analysis].

The etiologic relationship of hepatocellular carcinoma (HCC) and chronic hepatitis B viral infection was proved clinically, epidemiologically, virologically, serologically and by other methods. Together with viruses, carcinogenesis is promoted by faster recovery of hepatocytes (liver cirrhosis, chronic hepatitis, etc.). We have discovered 103 HCC in the period of 21 years. There were 81 men and 22 women; they were in the sixth and seventh decade of life in 90% of cases. Liver cirrhosis was also confirmed in 55% of cases and chronic hepatitis in 19%, namely, in 74% of patients HCC has developed on the changed liver. Fifty three patients were HBsAg positive (51.45%). The ultrasound and determination of alpha-feto proteins proved to be most valuable. It is a slow growing tumour and by its early detection surgical treatment is possible.

[Praziquantel in the treatment of human echinococcosis].

In the period from 1989 to 1993, 73 patients with Echinococcus granulosus cysts were treated by prazyquantel. To differentiate cysts tests of indirect immunofluoroscence and hemagglutination were used. The tests were performed during and after the completed treatment. During the treatment three patients were excluded so that 70 patients completed the treatment. Fifty five (78.9%) patients were treated only with drug with a success of 67% (37 patients). The preventive treatment, due to cyst perforation, was performed in 3 (4.3%) patients. Dissemination of illness occurred in no patients. Twelve (16.8%) patients were treated surgically. The parasite was not alive in 7 (58.5%) patients. The vitality of the parasite was controlled by microscopic examination of protoscolex and by their inoculation into rats of AO strain. The total success of treatment was 67.4% (47) of patients. In two patients the treatment was discontinued due to allergic manifestations. In our opinion, the drug treatment was the therapy of choice in patients with Echinococcus granulosus. It should be given prophylactically, preoperatively, to sterilize the cyst and also as a curative treatment.

[Determination of albendazole and its main metabolite albendazole sulfoxide in the hydatid fluid of human echinococcal cysts using HPLC].

Concentrations of albendazole and its active metabolite in echinococcal cyst of the human liver were determined in order to evaluate drug effects of the decrease of protoscolex vitality. Albendazole concentration of 0-64.9 micrograms/ml and albendazole sulfoxide of 0-40.8 micrograms/ml were found in cysts. The protoscolexes showed markedly manifested morphologic changes up to the disintegration. The postoperative follow up of patients within 24 months discovered no recidives of the disease and the patients were regarded as cured. On the basis of the results obtained it has been concluded that the use of albendazole in the dose of 800 mg/day within 28 day is sufficient for achievement of therapeutic drug level in vivo conditions.
