Safety of simultaneous resections of colorectal cancer and liver metastases.

Liver resection is the only potentially curative method for patients with colorectal cancer metastases and 5-year survival rates are 20%-40%. Simultaneous resection of colorectal cancer and synchronous liver metastases has been recommended if minor hepatectomy is indicated. The purpose of this paper is to analyze the treatment of hepatic colorectal secondaries and to assess the safety of simultaneous and delayed liver resections and relations of morbidity to the extensiveness of hepatectomy and perioperative factors. Analyzed were 21 patients with liver metastases from colorectal cancer operated between 1997 and 1999 in the Clinical Hospital "Sestre milosrdnice". Operating time for simultaneous colorectal and liver resections was not significantly longer compared to liver resections alone. No significant difference in complication rate was found after simultaneous procedures and liver resection alone (38% vs. 31%). Complication rate after major liver resections was not significantly greater than after minor resections (38% vs. 31%). No statistically significant differences were found in operation time and blood replacement between patients who developed postoperative complications and those who did not. In conclusion, simultaneous resections of primary colorectal cancer and liver metastases may be considered safe. Morbidity rates are not significantly different from those after liver resections alone, nor depend significantly upon the extensiveness of liver resection, providing that the operation time and blood loss are within the range observed in this study.

Distribution of immune cells expressing CD3a, CD21 and S-100 protein markers in the porcine gut-associated lymphoid tissues.

The distribution of immune cells within the gut-associated lymphoid tissues (GALT) of swine is highly organized. The appearance of such cells could not be separated from the effects of age, weaning and exposure to environment. Here, we have examined the distribution patterns of a subset of CD3a+ T and CD21+ B cells as well as S-100 protein+ cells and secretory (s) IgA+ cells within GALT compartments (such as jejunal lamina propria = JLP, ileal Peyerís patches = IPP, and mesenteric lymph node = MLN) of juvenile 8-week-old conventionally reared pigs using either two monoclonal antibodies (mAbs) or polyclonal antibodies (pAbs) in the immunohistochemical staining techniques with avidin-biotin complex (ABC) or peroxidase-antiperoxidase complex (PAP), respectively. The most potent porcine T-cell marker--CD3 surface antigen--is expressed as CD3a epitope on ileal intraepithelial lymphocytes, and numerous lymphocytes in the extrafollicular areas of MLN and dome region of IPP. Conversely, the cells expressing CD21 surface molecules were only demonstrable in the interfollicular areas of MLN and in the germinal centers of IPP. A strong reaction to sIgA was displayed by the plasma cells in the lumen of crypts and those residing the lamina propria of jejunum and ileum. The S-100 protein+ cells were numerous in JLP around the crypts and in IPP of weaned pigs. Both applied mAbs proved to be useful reagents for phenotypic and functional analyses of porcine lymphoid cell subsets by the ABC technique. However, further investigation of the S-100 protein marker is needed to determine which (if any) subset of porcine CD3+ CD4- CD8+ T cells could be designated as orthologue of human CD8+ CD11b+ suppressor T cells.

Immunohistochemical study of normal and mange (S. scabiei var. rupicaprae) infested chamois (Rupicapra rupicapra L.) skin.

The immunohistochemical study of chamois (Rupicapra rupicapra L.) skin showed that a limited number of available monoclonal and polyclonal antibodies expressed reactivity with skin cell components. These included cytokeratins, vimentin, desmin, neuron-specific enolase and S-100 protein with almost the same distribution pattern as already described in the skin of humans and animals. Antibodies used for labelling skin-associated lymphoid tissues and other cells with the immunologic function in human skin failed to demonstrate these cells in the chamois skin with the exception of LCA and OKT6 antibodies. Epidermal Langerhans cells were reliably demonstrated only by the enzyme histochemical method for adenosine triphosphatase, while the majority of mononuclear cells in dermal infiltrates showed a strong immunoreaction with OKT6 antibody. The histologic and histochemical analysis showed that the dermal infiltrations in infested skin consisted of macrophages, lymphocytes, granulocytes, mastocytes and fibroblasts. The chamois skin affected with sarcoptes mange showed a significant loss of cytokeratins in the epidermis and its derivatives. Particular keratinocytes showing nonspecific staining with several antibodies were also described and discussed in this paper.

Cyclosporine A increases serum cortisol levels in rabbits.

P-glycoprotein (P-gp), a membrane protein that was originally found to be involved in the efflux of cytotoxic drugs out of the tumor cells, is also present in a variety of normal human and animal tissues, such as the adrenal cortex. The function of P-gp in the adrenal cortex has not been defined yet. The aim of our study was to determine whether the blockade of P-gp by cyclosporine A (CsA) dissolved in Cremophor EL (Crem) inhibits cortisol secretion in rabbits. In 14 rabbits, the baseline and ACTH stimulated serum cortisol levels were measured before and after CsA treatment. Seven rabbits were treated with 2 x 30 mg/kg CsA and seven with 2 x 90 mg/kg CsA injected s.c. Serum cortisol levels were determined by radioimmunoassay adjusted for expected values. The whole blood CsA levels were determined by a commercially available fluorescence polarization immunoassay. Serum cortisol levels, both baseline and ACTH stimulated, significantly increased after both low and high dose CsA treatment. The increase was dose dependent. The mean baseline cortisol levels increased from 5.7 (SD = 6.3) to 15.0 nmol/l (SD = 7.2) in the low dose group and from 7.7 (SD = 4.9) to 44.9 nmol/l (SD = 13.8) in the high dose group. The mean cortisol levels 8 h after ACTH stimulation increased from 53.3 (SD = 34.5) to 106.0 nmol/l (SD = 33.0) in the low dose group and from 47.7 (SD = 12.2) to 153.0 nmol/l (SD = 55.1) in the high dose group.(ABSTRACT TRUNCATED AT 250 WORDS)

Langerhans cells in the immunopathology of contact allergic dermatitis.

The functional morphology, topography and frequency of Langerhans cells (LCs), which are significant factors in the pathogenesis of contact allergic dermatitis (CAD), were studied by histoenzymatic methods (adenosine triphosphatase (ATP-ase), acid phosphatase (AF) and alpha naphtylacetate esterase (ANAE), immunohistochemical methods (indirect immunoperoxidase (IPO) with the monoclonal antibody OKT 6), and the peroxidase-antiperoxidase (PAP) method with the polyclonal S-100 antibody in skin biopsies of 24 patients with CAD, as well in skin biopsies in experimental models in guinea pigs. The results confirmed the significant role of LCs in the pathogenesis of contact allergic dermatitis.

Basal cell carcinoma treated with interferon.

Human natural leukocytic interferon (HNLI) and recombinant interferon (rIFN) alpha 2c can be used in the therapy of basal cell carcinoma. Seventy-two patients were treated for 3 to 6 weeks with a single dose of 400,000 to 1,200,000 units (HNLI) and 2,000,000 to 5,000,000 units (rIFN alpha 2c). According to histopathologic and clinical findings, 52 patients were cured, and cancer cells were not found in the biopsy material taken after HNLI treatment for the second biopsy. In nine of 72 patients the cancer lesion was reduced 25% to 90%. In 11 patients it was not reduced or was reduced less than 25%. With rIFN alpha 2c therapy given five times per week for 4 weeks, histopathologic and clinical findings showed that five patients out of 12 were cured, and cancer cells were not found in the biopsy material taken after treatment. In seven patients out of 12 the cancer lesion was reduced 25% to 90%. Both types of interferons are effective in the treatment of basal cell carcinoma. Side effects were mild.

Treatment of squamous cell carcinoma with interferon.

Human natural leukocytic interferon and recombinant HulFN alpha 2c can be used in the therapy of squamous cell carcinoma. The duration of treatment was 3-6 weeks. A single dose was 400,000-5,000,000 units given weekly for 3-6 weeks. Clinically and histologically 19 of 32 patients were cured and tumor cells were not found in the material taken after interferon treatment for the second biopsy. In ten patients tumor size was reduced 25-90%, and in three patients tumor size was not reduced according to clinical findings. With recombinant HulFN alpha 2c therapy given 5 times per week for 4 weeks. Four of ten patients with similar tumors were cured clinically and histologically clinical findings. In five patients tumor size was reduced 25-90%, while in one patient there was no reduction in tumor size. Both types of interferons are effective in the treatment of squamous cell carcinoma. Side reactions were mild.

Immunohistochemical and stereological study of neuroendocrine cells in human antrum during the perinatal period.

A correlative immunohistochemical and stereological study of neuroendocrine cells (NEC) was carried out in the antrum of twenty human fetuses with gestational ages from 18 to 42 weeks and of two specimens postnatally. Neuron-specific enolase (NSE) as a common marker of neurons and NEC, as well as gastrin (G-) and somatostatin (D-) immunoreactive cells served for evaluation of volume density, which proved to be the most convenient method for quantitative analysis of NEC. It was observed that a considerable frequency of NEC appeared at 23-24 weeks of gestation (8% of NSE- and 6% of G- cells) and coincided with the adult pattern of intramural innervation. After a repeated increase of NEC in the 26-week-old fetus, the frequency of NEC remained persistant during the perinatal period (10-12% of NSE- and 7-8% of G- cells). An exception was a specimen with a prolonged pregnancy (42 weeks) in which the percentage of NSE- (17%) and G- (10%) cells was almost the same as at 6 weeks postnatally. The maximal quantitative difference of NEC was noted between 6- and 8-week specimens postnatally, e.g. 9% to 22% of G- cells, respectively. Observations obtained by NSE and S-100 protein were also demonstrated in lymphoid cells of gut associated and mesenteric lymphoid tissue.

Interferon therapy for basal cell carcinoma and squamous cell carcinoma.

Totally 161 basal cell and squamous cell carcinoma (BCC, SCC) patients were treated with human natural leucocytic interferon (HNLI) and recombinant IFN alpha 2c. After HNLI treatment, 61 out of 86 BCC patients and 29 out of 45 SCC patients were cured according to histopathologic and clinical findings. In 13 BCC and 13 SCC patients, the cancer lesion was reduced 25%-90%. After recombinant IFN alpha 2c treatment, 14 of 20 BCC patients and 4 of 10 SCC patients were cured according to histopathologic and clinical findings. In 6 BCC patients and 5 SCC patients the cancer lesion was reduced 25% to 90%. Both types of interferons are effective in the treatment of BCC and SCC patients. Local application of interferon stimulates immune reaction at the site of the tumor. There is a marked difference between the spontaneous macrophage activity and that induced by interferon. The interferon activated macrophages are significantly larger, the number of lysosomes and the density of macrophages is increased. In difficult locations intralesional therapy can be considered to avoid disfigurement of the patients with or without surgery.

Baermannization of Dictyocaulus spp. from faeces of cattle, sheep and donkeys.

Faeces were collected per rectum from calves infected with Dictyocaulus viviparus (D.v.), from lambs infected with Dictyocaulus filaria (D.f.) and donkeys infected with Dictyocaulus arnfieldi (D.a.). In one experiment, the influence of storage temperature before Baermannization was investigated. Recovery rate for D.v. was approximately 80% after 24 h at 4 degrees C or 16 degrees C but only 40% at 20 degrees C. After two days at 20 degrees C the rate had fallen to 20%. Recovery rates for D.f. decreased so markedly during the first 12 h at 4, 16 and 20 degrees C that storage can not be recommended. Losses in the recovery rates of D.a. appeared insignificant after 48 h at 4 degrees C but not at 16 degrees C and 20 degrees C. In experiment II the time taken for larvae to emerge from a 10-g sample as well as the sedimentation time in Baermann tubes was investigated. The bulk of the D.v. larvae remained in the faeces for about 10 h whereas D.f. larvae emerged during the first few hours. D.a. larvae were intermediate in this respect. Sedimentation of the bulk of larvae from all three species took place within a few hours with the modified Baermann technique used.

[Coronarography findings following systemic short-term lysis of acute myocardial infarct].

In 50 patients with acute infarction who were admitted within 6 hours after the beginning of the complaints randomizedly an intravenous streptokinase short-term lysis or a conventional heparin phenprocoumon treatment was performed and 4 weeks later the results were objectified by coronarography and ventriculography. The patients were subdivided into 3 groups: 1. control group (without fibrinolysis), 2. early lysis (within 4 hours) and 3. late lysis (4-6 hours). Four weeks after the infarction the proportions of the open infarction vessels are approximately of the same size in all three groups, the functional results are best (end-diastolic pressure and kinetics of the cardiac wall) in the group of patients who underwent an early lysis. Successfully lysed patients with remaining residual stenosis must be regarded as particularly threatened by infarction and remain under special control (residual angina, ergometrically objectified exercise tolerance, perhaps coronarography) in the phase of rehabilitation.

[Clinical experiences with systemic high-dosage short-term fibrinolysis in acute myocardial infarct].

In 50 patients with acute myocardial infarction the systemic highly dosed short-term lysis with streptokinase took place within the 6-hour limit. 27 patients showed a normalisation of the ST-distance still during the treatment, 16 of whom did not develop any signs of necrosis in the ECG also in the further course. Pre-existing disturbances of rhythm receded under the infusion of streptokinase in 8 of 9 patients. In 11 patients arrhythmias appeared under the lysis, which could be brought under control in intensive-medical conditions. 4 patients died, 1 patient suffered a re-infarction. The results of the treatment were better, when the fibrinolysis began within 3 hours after the acute onset. Severe side effects did not appear.

Histopathologic and histoenzymatic observations in carcinomas treated with human leukocyte interferon.

In this study histologic and histochemical examinations were performed on the bioptic material of 30 cases of head and neck tumors and 34 cases of uterocervical carcinomas after treatment with human leukocyte interferon (HLI). Particular attention was paid to the interaction between stromal and neoplastic cells as a possible host response to the treatment with interferon. In addition to tumor biopsies, regional lymph nodes were also examined microscopically. Stromal cells (macrophages, T and B lymphocytes, and granulocytes) were detected by enzyme-histochemical methods. Histologic and histochemical criteria for classification of tumor biopsies into five groups included the frequency, the cellular composition of peritumoral stroma, the grade of macrophage infiltration into the tumor (expressed quantitatively), and cytologic changes, followed by the regression of tumor. The overall appraisal of patients with cervical carcinoma based on histopathologic and chemical criteria showed the positive effect of HLI therapy in 70% of the cases; observations of head and neck carcinoma treated with HLI showed positive effects in 82% of the cases.

Application of human leucocyte interferon in patients with urinary bladder papillomatosis, breast cancer, and melanoma.

Eight patients with malignant papillomaosis of the bladder, four with breast cancer, and two with malignant melanoma, all of whom had a poor prognosis, were treated with injection of crude human leucocyte interferon (HLI) into the tumour or the adjacent tissue and in some cases with intramuscular HLI injection also. All tumours showed complete or partial regression after treatment for up to 6 months. One patient with breast cancer died from distant metastases; all other patients in the trial were recurrence-free after 3--30 months of follow-up.

Application of human leucocyte interferon in patients with carcinoma of the uterine cervix.

15 patients with invasive squamous-cell carcinoma of the uterine cervix were treated with crude human leucocyte interferon (HLI) for 3 weeks before surgical removal of the tumour. 9 patients were given HLI topically and intramuscularly and 6 topically only. In 3 patients the surgical material was free from tumour cells, in 3 it showed a lower grade of carcinoma, and in 9 the findings remained unchanged. In only 1 patient the tumour metastasised to the lymph nodes. Typically a tumour regressed to about a third of its original size. There was a sharp distinction between the tumour mass and the healthy tissue manifested in the formation of a fibrous wall. On the basis of the scores--i.e., stromal response, relation between tumour-cell and macrophage activity and reactivity, and reactivity of the original lymph nodes--the overall appraisal of the response caused by HLI therapy was as follows: in 6 patients excellent, in 5 very good, in 1 moderate, in 2 poor, and in 1 no response. It is suggested that HLI is suitable for administration both before and after surgery in patients with cervical cancer of grade I or II. If stroma cannot be induced to respond within 21 days of treatment, HLI application should be discontinued.

Influence of human leukocyte interferon on squamous cell carcinoma of uterine cervix: clinical, histological, and histochemical observations. III. Communication.

Fifteen patients with invasive squamous cell carcinoma of the uterine cervix were treated with human leukocyte interferon (HLI). HLI was applied topically and i.m. to nine patients, and only topically to six. After therapy, surgical material was free from tumor cells in three patients, tumor cells were identified as invasive carcinoma in one patient and as CA in situ in two patients. Findings remained unchanged in nine patients. Tumor metastases in the lymph nodes were found in only one patient. The patients' overall response to therapy was assessed on the basis of stromal response, relation of tumor cells and macrophage activity, and the reactivity of lymph nodes.

Human leukocyte interferon in therapy of patients with urinary bladder papillomatosis, breast cancer, and melanoma.

The authors report the result of local and parenteral applications of human leukocyte crude interferon in the therapy of eight patients with urinary bladder papillomatosis, four patients with breast cancer, and two patients with melanoma. The doses and effects of treatment are presented in tables. Clinical application of human leukocyte interferon (HLI) to these three types of malignant tumors has yielded encouraging results. The authors believe that investigations on HLI applications to some malignant tumors should continue because the results obtained indicate that interferon possesses not only an antiviral but also an antitumor activity.
