[Renal leiomyosarcoma].

Presentation of one case of a large, rapidly growing and highly aggressive renal leiomyosarcoma. The diagnostic and therapeutical possibilities of this infrequent tumour are analyzed.

[Gonococcal urethritis in men: clinical experience in 1978-1988].

Nine hundred thirty four samples of urethral secretion proceeding from male subjects without previous treatment were analyzed with Gram stain. These subjects consulted in Santiago Western area, and belong to medium and low socioeconomical status. In 82.5% of samples, intracellular gram-negative diplococci were found, that made the presumptive diagnosis of gonococcal urethritis. The importance of direct examination of urethral secretion for the diagnosis and immediate treatment of gonococcal urethritis is emphasized.

[Laboratory diagnosis of dermatophytosis: 10 years experience in the western area of Santiago].

From 1979 to 1988, 3890 positive samples for dermatophytes obtained at the Dermatology Service of Hospital San Juan de Dios (Western Santiago area) were analyzed for type of fungus involved and for relative yield of direct examination compared to culture. Direct examination was performed on samples treated with 20% KOH and stained with rhodamine. Cultures were obtained on Agar Sabouraud medium with addition of gentamycin and chloramphenicol. Yield of direct examination was 97.2% compared to 85% for culture. According to anatomic location, M canis was isolated from nearly all cases of T capitis. In cases of T pedis, nail mycosis, crural and palmar infections the most frequent species were T rubrum and T mentagrophytes M canis and T rubrum were responsible for infections of the body.

[Obtention, characterization of antigens and evaluation of the immunotransfer technique for the detection of systemic candidiasis].

A culture media containing peptone, glucose, yeast extract and serum was evaluated to promote the development of the mycelium formation in Candida albicans and it was compared with the commercial media Tc 199. No significant difference was found between them. Somatic antigens characterized by PAGE-SDS were obtained from yeast and mycelial forms. Proteins reported as having high diagnostic value were expressed in both antigens. It was determined by immunotransference that the antigenic proteins predominating in the mycelium formation had molecular weights of 19, 21, 27 and 57 Kda. The immunotransference was standardized for detecting antibodies in patients with systemic candidiasis. 24 positive sera were evaluated by this technique and it was compared with the ELISA technique, which proved to be more sensitive and less specific. The most recognized proteins were those of 136 and 21 Kda and the complex from 42 to 45 Kda.

[Tumor of the vitelline sac of the testicle (yolk sac carcinoma)].

The tumour of the yolk sac is a germinal cell neoplasia arising in the prepubertal testicles which was included in the WHO's histological classification of neoplasia of the testicles in 1977. We contribute a case of this histological variety in Stage I, according to the Royal Marsden criteria. The patient was treated with inguinal orchidectomy only and 3 years later remains disease-free.

Inhaled steroids treatment and withdrawal in asthmatic children.

Twenty young mild asthmatics (age range 7-22 yrs) were studied in order to determine the time course of bronchial hyperresponsiveness during inhaled steroid treatment and after steroid withdrawal. Study has been divided in three periods: Baseline (two weeks), Treatment (eight weeks) and placebo (eight weeks). Inhaled budesonide was administered during treatment period at a doses of 0.4 or 0.6 mg/ml twice a day, depending on age. After withdrawal, placebo was administered in the same way. Bronchial responsiveness (BR) to exercise was estimated trough the Bronchial Lability Index (BLI) defined as the amplitude of change of peak expiratory flow rate in a percentage related to the basal during a free running test. A BLI higher than 20% defined Bronchial Hyperresponsiveness (BHR). All patients presented BHR during the baseline period. Forced spirometry and exercise tests were performed every two weeks throughout the study. No changes in spirometric parameters (FVC, FEV1) were observed either during treatment or during placebo periods. A highly significant decrease in BR to exercise was observed during active drug treatment (BLI = 32.4 +/- 3.1 to 8.2 +/- 0.7 from basal to the end of the two months treatment period, mean +/- sd). During the placebo period 10/17 patients increased BLI to near baseline values, and 7/17 patients remained at normal levels. This behaviour was related to the severity of asthma during infancy. Our results confirm the close association between inflammation and BHR.(ABSTRACT TRUNCATED AT 250 WORDS)
