[Skateboard injuries (author's transl)].

Following the introduction of skateboards into Germany in 1976 an increasing number of accidents and injuries have been noted, affecting mainly children 10 to 14 years of age. The causes were lack of experience and the careless use of the boards on public streets. More than 30% of injuries were fractures mainly affecting the upper limb. Because of the frequent involvement of the epiphyseal plates the post-accidental growth may be seriously disturbed. Severe and sometimes fatal head or abdominal injuries have been reported, mainly after collision with cars.

Measuring of pressure forces in sitting.

We were able to re-examine already known facts in tests of pressure areas in seats by using a new analytic system. It was shown that only in backrest seat angles of 90 and 110 degrees pressure areas below the ischial tuberosities were clearly defined. The transition area of zones of high to medium pressure was sharply defined with these angles. In the anterior position and with large backrest seat angles high pressures were not found and the transition areas between zones of medium and low pressure became fluent. Only the pressure distribution pattern in the 90 and 110 degrees position is physiologic and corresponds to the pressure tolerance of different parts of the human body in seats. This pattern has to be maintained principally in a contoured and upholstered seat.

[The isolated loss of extension of the great toe following osteotomy of the fibula (author's transl)].

A peroneal nerve palsy can be observed following operative procedures or traumatic lesions of the lower leg. Primary damage of the nerve must be differentiated from the tibialis-anterior syndrome and the pseudo-paralysis. Following corrective osteotomies of the tibia with dissection of the fibula in the upper or medial third isolated lesions of the extensor hallucis longus muscle can be seen. Electromyographic and anatomical studies reveal that they may be caused by an isolated damage of the motor nerve fibres connecting the deep branch of the peroneal nerve with the extensor hallucis longus muscle lying very close to the fibula. Suggestions how to avoid this damage are made in the paper.

Electromyography↗

[Muscles of the leg during immobilisation. An electromyographic study (author's transl)].

By means of electromyographic evaluation we investigated the function of the rectus femoris, the vastus medialis, the tibialis anterior and the triceps muscle. The effects of immobilisation in above or below knee PoP were investigated with view to the muscular activity in walking and the effect of the immobilization in isometric exercises. The results are discussed. The vastus medialis can only be activated very poorly in a PoP and is not inervated at all during gait. The functional activity of the triceps is reduced markedly in either above or below knee PoP. In knee movement this muscle starts to be activated only over 90 degrees of flexion. The results suggest that an above knee PoP is sufficient to immobilise defects of the quadriceps and its tendon. Defects of the triceps and its tendon seem not necessarily to request an above knee PoP for immobilization. This case has however the advantage to decrease the danger of unexpected knee movements followed by a reflectoric muscle contraction. Its force could be greater than the stability of the repaired tendon.

[Fractures of the neck of femur in juveniles (author's transl)].

The problems of the treatment of femoral neck fractures in young patients are discussed on 22 cases as well as the operative technique, post-operative treatment, late complications and their treatment. We feel that femoral neck fractures in children and young ones should be treated operatively. The three flanged nail should be used for internal fixation, giving a better fixation than K-wires. We have not seen any damage caused by the crossing of the epiphyseal plate with a nail or K-wires. Postoperatively the patient should walk non weight bearing on the fractured hip for 9 months, especially in transcervical fractures. The danger of aseptic necrosis of the femoral head is clearly diminished by this precaution. The late results of femoral neck fractures in juveniles are clearly worse than in children. The femoral neck fracture of the young is to be treated as an emergency. If the therapy is too late or not adequate the consequences for the hip joints of the juveniles can be drastic.

[Fractures of the neck of femur in young children (author's transl)].

The problems of femoral neck fractures in young children are discussed. Results of 12 own cases are reviewed with an average age of the patients of 7.5 years at the time of accident. The average time between accident and last examination was 12 years. The operative technique, pre- and postoperative care, indications for conservative treatment and late complications are discussed. All types of fractures should be treated operatively. K-wire fixation is sufficient in most cases and does not damage the epiphyseal plate. Immobilization in POP spika is advisable for 6 weeks. With delayed operative treatment of intracapsulare fractures the intraarticular haematoma should be aspirated. Traumatic epiphyseolysis should be reduced by manipulation or operatively and stabilized by K-wires. The treatment of pseudarthrosis is by internal fixation and bone grafting. Late valgus and especially varus deformations should be corrected by intertrochanteric osteotomy.

[Quadriceps femoris muscle during immobilisation and remobilisation (author's transl)].

40 patients of which one leg had been immobilized in a pop cast for 3 and 12 weeks have been investigated with view to the quadriceps muscle. Mechanical and electromyographic investigations revealed the muscular changes under immobilisation and their recovery after removal of the cast. After 12 weeks of immobilisation and 24 weeks of remobilisation the muscle mass is still diminished.

[Sitting posture after arthrodesis of the hip joint or lumbosacral fusion (author's transl)].

The sitting posture of persons with arthrodesis of the hip joint and with lumbosacral fusion are analyzed. It is shown that any disorder in the hip joint - pelvis - lumbar spine system results in a change of posture. In particular, a defective hip joint produces an unfavorable posture by folding over the lumbar spine. The effect of fusion of the lumbosacral area on sitting posture is less marked.

[Kienböck's disease. Follow-up studies and therapeutic considerations (author's transl)].

This paper presents a follow-up of patients with Kienböck's disease up to 30 years after their first symptoms. Only the shortening of the radius and the grafting of the lunate with cancellous bone offers some kind of basic therapy. The intercarpal arthrodesis, the arthrodesis of the wrist and the operation named after Steinhäuser should only be considered in cases with severe deformation and disabling pain.

[Late results in hemialloarthroplasty of the hip (author's transl)].

This paper presents a study of long-term results of femoral head replacement. 104 patients with Moore prosthesis could be followed up for an average of 9 1/2 years. In cases of fractures and complications of trauma the results were excellent and good in 65%. Practically 100% poor results could be shown in patients with osteoarthritis
