[Wrist joint deformities caused by multiple cartilaginous exostoses of the distal ulna].

On the basis of follow-up examinations it was found that exostoses at the distal ulna often result in shortening of the ulna and curvation of the radius. The resulting deformation of the wrist joint is particularly common in girls with exostoses which are over 7 mm wide and affect the ulnar epiphyseal cartilage. Particularly in the period of growth before puberty the progress of the deformation is considerable. Thus, female patients of group 3 in particular have to be examined carefully; in many cases surgery is necessary, in order to prevent further deformation and possibly even destruction of the radio-ulnar joint.

[Surgical treatment of the snapping hip].

Snapping hip (Coxa saltans) is the abrupt sliding of the iliotibial tract over the greater trochanter. In the case of pain, which cannot be alleviated by a conservative treatment, an operation is advised. The methods of operation applied up to this day showed--especially because of their high recidive-quotas--great disadvantages. In a detailed post-operational check-up over the past 20 years the new method of "diagonal notching" (27 cases) developed at the Orthopedic Hospital München-Harlaching was compared to the old method of fixing the tract according to M. Lange (24 cases). After "diagonal notching" the snapping occurred again seven times less than after fixing the tract. After "diagonal notching" only 4% of the patients had to be re-operated, however, after fixing the tract a re-operation was necessary on 38% of the patients. 91% of the patients were relieved from their pain almost completely after "diagonal notching", compared to only 50% after fixing the tract. Considering that the "diagonal notching" offers simple technics of operation and better results than other methods, the "diagonal notching" should be used as method of choice to eliminate a snapping hip.

[Results of the follow-up of varus osteotomy in Perthes disease with reference to the hip value].

A total of 89 patients with 105 hip joints affected by Perthes' disease at Catterall's stages II to IV, which had been treated by varus osteotomy, were re-examined clinically and radiologically between two and ten years after surgery. The clinical and subjective results of treatment were generally good. The best clinical and radiologic results were found in patients who contracted the disease before the age of seven and in whom necrosis was not yet at an advanced stage. The results in children who did not undergo treatment until after the age of eight and those with the disease at Catterall's stage IV were significantly poorer as far as arthrotic deformation of the hip joint was concerned. Radiologic evaluation of the hips examined, with regard to severity of arthrosis, epiphyseal index, and hip value show that the hip value is the best means of distinguishing prearthrotically deformed hip joints from normal findings. For example, at the time of follow-up severe prearthrotic changes were found in 40 of 105 hip joints when the hip values were analyzed. Therefore, in cases of Perthes' disease also, the hip value is of special importance in prognostic counseling of patients as regards the extent to which the hip joint can be subjected to stress either at work or in sports.

[Results of operative treatment of osteochondrosis dissecans of the knee joint in juveniles].

This paper reports about the check-up of 33 juvenile and 28 adult patients who were operated on because of osteochondrosis dissecans of the knee. The average period of observation was 4.7 years. According to the aspects of age and the stage of the disease the results were compared to each other. The age of the patient at the moment of operation has doubtless the greatest influence on the prognosis. Juveniles operated in stage 2 or 3 have a particularly good prognosis. From stage 4 on it does not seem reasonable to try to preserve the dissecate. In the case of juveniles metabolic or endocrinological diseases do not appear to be of any relevance whereas mechanical factors like overstress at sports, hereditary ligament laxity or deviations of axis of whatever origin seem to be of larger import.

[Late results of bilateral osteochondrosis dissecans of the femoral head].

This paper reports on long-term results following surgical treatment of 2 patients with bilateral osteochondrosis dissecans of the femoral head. The success of treatment of this rare condition depends less on the choice of surgical procedure than on continuous relief in the postoperative phase.
