Women who abuse their children: implications for pediatric practice.

Parents who abuse their children may not accept traditional therapy but may be influenced by the child's primary care physician. A comparative study of abusive and nonabusive mothers showed abusers to have lower self-concept and higher self-concept incongruence and inconsistency than nonabusers. They were also found to value authority over others more, and conformity and benevolence less, than nonabusers. Practically applied, the data lead the pediatrician to an educative and supportive role in which he or she may enhance self-esteem and lower unrealistic expectations in the course of treating the child. In addition, there seems to be a need to develop access to support groups, day care, and other avenues for the mother's personal growth. This may be done either within a pediatric practice or through liaison with community resources.

Video-feedback and personal attribution in anorexic, depressed and normal viewers.

Video-feedback has been shown to be particularly influential in changing bodily awareness and causal attribution by confronting participants with a concrete 'public' self-image. Body-image disturbance and denial are major aspects of patients suffering with anorexia nervosa. It was predicted that video-feedback should alter these attitudes in this group and result in a more realistic self-appraisal. Anorexics respond to video-feedback with a reduction in self-esteem whereas normal controls displayed an increase in self-esteem and depressed patients perceived themselves negatively both before and after video-feedback. It is concluded that video-feedback may prove to be a useful technique in the treatment of body-image disturbance in anorexic patients.

Comparison of the clinical effectiveness of "short" versus "long" stay psychiatric hospitalization. IV. Predictors of differential benefit.

One hundred seventy-three patients were randomly assigned to LT (long term, unlimited length of stay, mean 179 days) vs. ST (short term, 90-day limit) psychiatric hospitalization. Three-year follow-up results showed few between-group differences, although LT patients tended to be rehospitalized more and had superior relative ratings of psychopathology. Demographic and clinical data did not predict differential benefit from LT or ST hospitalization. Diagnosis did not generally predict differential benefit either, except that clinically diagnosed personality disorders, given LT as opposed to ST hospitalization, had poorer role functioning and less psychiatric treatments after discharge. Patients with a history of drug abuse did worse if given LT hospitalization in terms of role functioning and rehospitalization. Overall, this study agrees with other relevant studies in indicating that hospitalization should be kept as short as feasible.

A method of structured brief psychotherapy.

Structured brief psychotherapy is a method of organizing treatment along problem-solving lines. The features of this method are the selection of problem areas, the translation of insight into specific activity, the structuring of psychodynamics and a concentration on termination events. The method is illustrated by clinical examples.

Haloperidol and thiothixene in the long-term treatment of chronic schizophrenic outpatients in an urban community: social and vocational adjustment.

Social and vocational adjustment are highly important--but frequently overlooked--measures of the effectiveness of psychotropic agents in therapeutic regimens involving chronic schizophrenic outpatients. This 24-week, double-blind study compared the effectiveness of haloperidol and thiothixene in facilitating social and vocational adjustment, as well as in controlling more traditionally studied psychopathological parameters, among 36 urban schizophrenic outpatients. Significant improvement favoring thiothixene was shown on physicians' ratings, ratings by relatives living in the same household as the patient, and patients' self-ratings. Side effects occurred with similar frequency in both drug groups; no patients were required to discontinue therapy because of side effects. There were no clinically significant abnormalities in laboratory values in either group.

Community residents and mental health center staff: assessment of program and priorities.

Mental health professionals have emphasized the importance of establishing priorities and goals and gearing the delivery of mental health services to the needs of area residents. Sound View-Throgs Neck Community Mental Health Center of New York has made such an effort to involve the community residents and the mental health center staff in establishing priorities for development of the new program for services in the community. We have outlined the methods of gathering such information, and implementation of the conclusions derived from the survey which has resulted in developing comprehensive children's services and a geriatric program in our catchment area.

Death and injury of Staphytococcus aureus during thermal treatment of milk.

Staphylococcus aureus isolated from milk and grown in milk was heated in milk. The phenomena of death as well as injury was investigated in the range of 50 to 75 degrees C. The D60 value (decimal reduction time on salt-free medium) was 0.87 min, the D'60 value (decimal reduction time in salt-containing medium) was 0.62 min. Cultures were injured as soon as heating started. This initial thermal shock increased with increasing temperature. At 50-60 degrees C injury was more rapid than death. At greater than 60 degrees C death became faster than injury and the two processes coincided at 70 degrees C. The Z value was 9.46 degrees C and the Z' value was 9.93 degrees C.

Clinical effectiveness of "short" vs "long" psychiatric hospitalization. I. Inpatient results.

To evaluate the clinical effectiveness of short-term (three month maximum length of stay) and long-term (discharge based on clinical judgement) hospitalization, the inhospital course of 68 "short-" and 58 "long-term" psychiatric patients was studied. The results indicate that patients assigned without bias to short-term patterns, however, indicated that these results could be entirely accounted for by the significantly greater use of group therapy as an additional treatment modality in the short-term units. The need for a more systematic exploration of the effect of restricted hospital stay on the treatment patterns of the clinician and the effect of these differential treatment patterns on inhospital improvement is emphasized. The necessity for follow-up data to gain a complete picture of these treatment contrasts is clear.
