[Psychotherapy of patients with schizophrenia. Study of the significance of group supervision].

A Danish centre contributed 12 cases to a Nordic multicentre investigation concerning the psychotherapy of psychoses. Patients admitted consecutively to the psychiatric hospital with the diagnoses of schizophrenia, schizophreniform psychosis or schizoaffective psychosis were offered psychotherapy at least once weekly for two years in addition to the other treatment modalities administered. In the Danish design, the process of supervision in relation to the processes of psychotherapy was investigated. In the present article, examples are presented illustrating how core psychotic mechanisms in the patients are reflected not only in process-facilitating but also in process inhibiting psychotherapeutic interventions. The data of the investigation are these interventions which are written down prospectively in the summaries of the supervision.

Circadian variation in serum amikacin levels.

Variable serum amikacin levels have been reported in the same patient even after a steady state presumably has been reached. Therefore, the authors investigated the optimal schedule for monitoring serum levels of the drug in 50 neutropenic patients receiving continuous infusion amikacin therapy for infections. We found that levels obtained in the early morning hours were significantly higher than those obtained for the same patient in the evening. As these differences parallel those previously demonstrated for renal function, they may be explained by the pattern of drug clearance by the kidneys. We recommend that blood specimens for the detection of rising serum amikacin levels in a therapeutic setting be obtained in the early morning and at the same time each day so that meaningful comparisons of peak concentrations can be made. However, late evening samples should also be tested whenever dosage modifications are considered so that continuous therapeutic serum concentrations can be ensured.

The activity in vitro of trospectomycin sulphate (U-63366F) against aminoglycoside-resistant enterococci.

Trospectomycin (U-63366F), a 6'-propyl analogue of spectinomycin, was tested against aminoglycoside-resistant enterococci. The MIC90 for Enterococcus faecalis was 4 mg/l and that for E. faecium was 8 mg/l. Trospectomycin alone was not bactericidal for enterococci, with MBC90 4096 mg/l for both E. faecalis and E. faecium. The addition of commercially available polyvalent immunoglobulin decreased significantly both the MICs and the MBCs and the rendered trospectomycin bactericidal for enterococci. Chequerboard titration of a combination of trospectomycin with ampicillin revealed an FIC index of 1.0 for all the isolates tested. Time-kill curves also did not show any enhancement of bactericidal activity of ampicillin when combined with trospectomycin. A combination of ampicillin and gentamicin was synergistic for enterococci under similar experimental conditions. Trospectomycin can be used as a safe alternative to aminoglycosides or beta-lactam antibiotics in enterococcal infection where bactericidal activity is not required, or in the event of serious side effects from these two classes of antibiotics.

In vitro susceptibility of Xanthomonas (Pseudomonas) maltophilia to newer antimicrobial agents.

The susceptibilities of 45 clinical and 3 environmental isolates of Xanthomonas maltophilia to 14 antimicrobial agents was determined by broth microdilution. The newer quinolones PD117596, PD117558, PD127391, A-56620, amifloxacin, and fleroxacin were the most active agents tested, with 70 to 99% of isolates being susceptible to these agents. All isolates were resistant to trospectomycin. The new aminoglycosides SCH24120 and SCH22591 were active against 12 and 1% of isolates, respectively.

A model of sustained gastrointestinal colonization by Candida albicans in healthy adult mice.

Three-month-old male Crl:CD1(ICR)BR and C3H/HeJ mice were fed chow containing Candida albicans for 14 days, while similar control mice were fed regular food. Stool cultures were done for all mice before and after administration of the special diet. Stool cultures were repeated 48 h, 1 week, and 1 and 3 months after stopping the diet for Crl:CD1(ICR)BR mice and again 5 months afterward for C3H/HeJ mice. Some animals were sacrificed at the end of the special diet, and cultures and histopathologic examination of various organs were performed. Colonization with C. albicans occurred in the Candida-fed mice, and the fungus was maintained in the gastrointestinal tract at a concentration of 10(3) to 10(4) CFU/g of stool for up to 5 months. There was no histologic evidence of organ infection with Candida spp. The fungus was not found in stool cultures or organs of mice in the control group. The results suggest that persistent gastrointestinal colonization of adult mice by C. albicans can be achieved without immunosuppression. Thus, with additional manipulations, this model could be useful for studying the role of gastrointestinal colonization by C. albicans in the development of systemic infection.

[An evaluation of ambulatory psychotherapy].

Only few research studies on psychotherapy in Denmark have been published. Our investigation summarizes the results of the psychotherapeutic treatment given 1982-1987 at the Outpatient Clinic for the treatment of Neuroses, Psychiatric Hospital in Aarhus. 45% of the 803 patients who were referred to the clinic commenced psychotherapy. 30% of the patients were students aged 20-39 years. Diagnostically, 60% of the patients had neurotic disturbances, 20% were borderline personality organised and the remainder included patients with higher level personality disturbances and psychotic patients. 85% of the patients were on the waiting list for less than six months. 42% of the men and 52% of the women had more than 25 psychotherapy sessions. 80% had individual psychotherapy. According to the opinions of the psychotherapists, 48% of the male patients and 44% of the female patients gained considerably from the treatment. Those who were helped by the treatment received treatment significantly longer than those in whom the condition did not change or deteriorated. The results of the investigation indicate the need for a prospective qualitative study of the therapeutic alliance, and this investigation is in preparation.

Spectrum and outcome of microbiologically documented listeria monocytogenes infections in cancer patients.

At the M.D. Anderson Cancer Center (Houston), Listeria monocytogenes was cultured from 14 patients between 1980 and 1987. The case records of 11 of these patients were reviewed. Underlying malignancies included acute leukemia (three), lymphoma (two), myeloma (one), adenocarcinoma of colon (two), carcinoma of breast (one), carcinoma of lung (one), and Kaposi's sarcoma associated with the acquired immune deficiency syndrome (one). Listeria monocytogenes was cultured from blood (eight patients), cerebrospinal fluid (CSF) (two patients), and from both blood and CSF in one patient. All patients were receiving immunosuppressive therapy including corticosteroids in seven. An absolute neutrophil count of less than 1000/mm3 was noted in five. Bacteremia was the predominant type of infection and ten patients responded to antimicrobial therapy.

[Sectorized psychiatry. Ambulatory psychiatry in a geographically defined region].

The article describes the psychiatric outpatient treatment service in the catchment area of the county of Roskilde in Denmark. Eighty-four general practitioners, ten private practising psychiatric specialists and the psychiatric outpatient service at Roskilde Amts Sygehus Fjorden participated in a prospective cohort study based on questionnaires. This includes 404 patients from general practice, 326 from the psychiatric specialists and 166 from the psychiatric hospital. The patients' social conditions, demographic data, psychosocial problems, psychosocial strain, diagnoses, and the treatment are presented. Psychotics constitute 52% of the patients from the outpatient treatment service corresponding to 25% from specialist practice and 12% from general practice. The most common diagnoses at specialist level are neuroses and crises, in general practice, in addition, psychosomatic illness and depression without psychoses. The treatment for most of the patients is psychotherapy or in combination with psychopharmacology. Further acute psychiatric service and supervision are required. One year follow-up has been accomplished. Follow-up reviews after five and ten years are planned.

Comparative in-vitro activity of twenty antimicrobial agents against clinical isolates of Mycobacterium avium complex.

The in-vitro susceptibility of Mycobacterium avium complex isolates, obtained from immunosuppressed patients with and without the Acquired Immunodeficiency Syndrome (AIDS), to various antimicrobial agents was determined. Amikacin, the 4-quinolone compounds--ciprofloxacin, temafloxacin and PD 117558--and the penem SCH 34343 were active against most of the isolates. In-vitro synergism using selected antimicrobial combinations could not be demonstrated. No differences in the susceptibility, depending upon the source of the isolates (AIDS or non-AIDS), were noted.

Prospective randomized trial of antibiotic prophylaxis in acute leukemia.

Patients undergoing initial remission induction chemotherapy for acute leukemia in a protected environment unit were randomly assigned to parenteral antibiotic prophylaxis or oral and parenteral antibiotic prophylaxis. Complete remissions were obtained in 82 percent of the 45 patients receiving oral and parenteral antibiotic prophylaxis and 76 percent of the 41 patients receiving parenteral antibiotic prophylaxis. Approximately 20 percent of the patients in both groups have had a continuous complete remission for more than five years. The episodes of fever of unknown origin and major infection were significantly more common in patients receiving parenteral antibiotic prophylaxis, although the episodes of local infection were similar in both groups. The duration of remission and survival was similar in both groups. Hence, the oral and parenteral antibiotic regimen was more effective for infection prophylaxis, but had no effect on response to antileukemic chemotherapy.

A "dipstick" enzyme immunoassay for detection of antibody to Brucella abortus in cattle sera.

An enzyme immunoassay that utilizes antigen bound to a matrix which can be removed from the substrate to stop development is described. The assay which is performed in glass or plastic disposable tubes uses Gel-Bond film strips for attachment of antigen. The only equipment requirements are a rotary shaker and a spectrophotometer (optional). The antigen coated strips are passed through a series of tubes containing test serum, wash solution, antibody-enzyme conjugate, wash solution and substrate-chromogen taking about 45 minutes to perform. In testing sera with or without antibody to Brucella abortus a very high correlation existed between same day tests and tests performed over several days as well as with data on the same sera obtained by an enzyme immunoassay in a microtiter format.

A class capture enzyme immunoassay for immunoglobulin level determinations in bovine sera.

An enzyme immunoassay for determination of individual isotype concentrations in bovine serum was developed. Polystyrene tubes were coated with affinity purified goat antibovine IgA, IgG1, IgG2 or IgM, washed and then incubated with purified isotypes to ascertain crossreactivity and sensitivity limits. Bound isotype was detected using the homologous affinity purified antibody conjugated to horseradish peroxidase and hydrogen peroxide-2,2-azino-di-(3-ethylbenzthiazoline sulfonic acid), as the substrate/chromogen. A standardized serum was diluted and used as a control for comparison. Several dilutions were used initially, however, determinations may be made with a single dilution, 1:200, for all isotypes. Results for 100 sera were compared to data obtained with the same samples using a radial immunodiffusion technique. A low correlation coefficient was noted between results from the two assays. Day to day variation and within test repeatability were determined for both assays using ten samples. For the enzyme immunoassay method, day to day variation for IgA, IgM, IgG1 and IgG2 determinations was 17.5, 19.3, 7.6 and 7.3% while variation in repeatability (within a test) was 6.2, 5.9, 3.3 and 4.5%, respectively. Day to day variation for the single radial immunodiffusion test for IgA, IgM, IgG1 and IgG2 was 15.4, 26.0, 11.5 and 18.3% and variation repeatability (within a test) was 11.6, 13.9, 5.9 and 8.3%, respectively. The procedures consistently detected 0.1 micrograms of immunoglobulin whereas the radial diffusion sensitivity limit was approximately 500 micrograms.

Effects of various treatments of bovine complement on its lytic efficacy measured by two different tests.

Bovine complement was treated with various agents known to activate or inactivate one or more of the cascade components. The treated complement was then assessed for remaining hemolytic activity by a tube titration test and a radial hemolysis method. Divalent cation chelators (EDTA and EGTA); immune complexes prepared with serum and IgM, IgG1, IgG2, and IgA isotypes; smooth and rough lipopolysaccharides and lipid A; hydrazine; zymosan; cobra venom factor and brown recluse spider venom caused depletion of complement as determined in the tube titration test. Immune complexes (prepared with serum); hydrazine; cobra venom factor; EDTA and smooth lipopolysaccharide caused loss of hemolytic activity in the radial hemolysis test. This evidence suggests that the radial hemolysis test assessed complement consumed by the alternate pathway, while the tube titration method measured classical pathway consumption.

Effect of aztreonam on throat and stool flora of cancer patients.

Eighteen patients with hematological malignancies received aztreonam in one of two dosing regimens, 1 or 2 g every 8 h for a total of 7 to 9 days. Throat and stool cultures were obtained before and during treatment with aztreonam. Aztreonam had little effect on the predominant throat flora. In contrast, facultatively anaerobic gram-negative bacilli were markedly decreased in stools during the administration of aztreonam. Strict anaerobes in the stool were variably affected by aztreonam.

Effect of broad-spectrum cephalosporins on the microbial flora of recipients.

The flora in the throat and the stools of 10 patients receiving chemotherapy for malignant diseases in a laminar air-flow room was studied during the prophylactic administration of ceftazidime. Ten percent of aerobic gram-negative bacilli, 41% of aerobic gram-positive organisms, 59% of anaerobes, and 70% of fungi persisted in stool specimens during ceftazidime administration. This drug had a less pronounced effect on the throat flora; 66% of organisms persisted during antibiotic administration. The throat and fecal flora of another eight patients were studied during the prophylactic administration of ceftriaxone. This antibiotic had a profound effect on the fecal flora; none of the gram-negative bacilli, only 24% of aerobic gram-positive organisms, and only 10% of anaerobes persisted during ceftriaxone administration. Like ceftazidime, ceftriaxone had a less marked effect on the throat flora; 59% of organisms persisted during antibiotic administration. The results show that new, expanded-spectrum cephalosporins can have a major suppressive effect on patients' endogenous microbial flora.

Haemolysis in gel test for detecting bovine antibodies to Brucella abortus lipopolysaccharide.

Optimal reagent parameters for a haemolysis in gel test for detection of bovine anti-Brucella abortus antibody were established. Using an alkali-treated crude lipopolysaccharide antigen attached to J-negative bovine erythrocytes, 0.125 mg of antigen, 0.1 ml of erythrocytes and 0.4 ml of guinea pig complement were required to perform 15 tests with appropriate serum controls. The haemolysis in gel test and an IgM radioimmunoassay (RIA) procedure were compared for their ability to detect the onset of increased serum antibody levels and antibody levels in sera diluted to extinction. While the RIA was more sensitive in amount of antibody detected, the haemolysis in gel test was equally able to detect initiation of antibody synthesis.
