[Comparison of early and late rehabilitation of stroke and cerebral trauma patients with visual field defects].

BACKGROUND: Most rehabilitation studies on visual field deficits after stroke or trauma are conducted after completion of the spontaneous recovery phase. However, the question arises whether more extensive visual field improvements can be reached when the training starts very soon after the lesion. METHODS: In this study, the results of 26 patients who began visual restoration therapy within the first 12 months after the lesion were compared with an age-related group whose lesions were more than 1 year old. RESULTS: The early-onset group showed an improvement of 8% in computer campimetry and 10-15% in conventional automated perimetry. The late-onset group had 13.5% improvement in campimetry and 20% in perimetry. CONCLUSION: In contrast to our assumptions, there was no significant difference between the groups. Furthermore, the late-onset group showed considerably greater improvement than the early-onset group. It is proposed that pronounced attention deficits soon after brain damage may complicate the training.

Influence of olfactory bulbectomy and subsequent imipramine treatment on 5-hydroxytryptaminergic presynapses in the rat frontal cortex: behavioural correlates.

1. Alterations of 5-hydroxytryptaminergic mechanisms are thought to play a special role in the pathogenesis of depression and antidepressant treatments are assumed to restore these changes. 2. We have used one of the most reliable models of depression, the olfactory bulbectomized rat to study the long term consequences of this manipulation and of subchronic imipramine treatment on two parameters of 5-hydroxytryptaminergic presynapses, 5-hydroxytryptamine (5-HT) transporter density and tryptophan hydroxylase apoenzyme concentration, in the frontal cortex as well as on active avoidance learning several weeks after bulbectomy. 3. The Bmax value of [3H]-paroxetine binding and the concentration of the 5-HT synthesizing enzyme were both significantly elevated in the frontal cortex of bulbectomized rats compared to sham-operated controls. 4. Imipramine treatment, either by daily injections or by subcutaneous implantation of slow release imipramine-containing polymers reduced the elevated tryptophan hydroxylase apoenzyme levels in the frontal cortex of bulbectomized, but not of sham-operated control rats and restored the deficient learning performance of bulbectomized rats. 5. Both effects were more pronounced after continuous drug administration by imipramine-releasing polymers compared to daily i.p. injections. 6. These findings indicate that bulbectomy leads to a compensatory 5-hydroxytryptaminergic hyperinnervation of the frontal cortex. Chronic antidepressant treatment seems to attenuate the increased output of the 5-hydroxytryptaminergic projections in the frontal cortex through the destabilization of the rate limiting enzyme of 5-HT synthesis of the 5-hydroxytryptaminergic nerve endings in this brain region.

Short photoperiod-dependent down-regulation of thyrotropin-alpha and -beta in hamster pars tuberalis-specific cells is prevented by pinealectomy.

Hamster hypophyseal pars tuberalis (PT)-specific cells are characterized by the expression of common alpha-chain and TSH beta. Immunoreactivity for these subunits and the morphology of these cells are known to exhibit remarkable seasonal changes. The high density of melatonin (Mel) receptors on PT-specific cells leads to the supposition that fluctuations in circulating Mel levels induced by photoperiodic signals are a crucial factor for the morphological alterations. To more closely investigate transcriptional and translational activities in PT-specific cells, we cloned and sequenced hamster alpha and TSH beta complementary DNA fragments and assessed messenger RNA/protein formation by in situ hybridization and immunocytochemistry under short and long photoperiod and in pinealectomized animals kept in short photoperiod. Hamster common alpha-chain and TSH beta exhibited high sequence homology with the corresponding rat hormones [94% (alpha-chain) and 90% (TSH beta) on the nucleotide level and 100% (alpha-chain) and 96% (TSH beta) on the amino acid level]. Immunocytochemical staining with antibodies directed against the common alpha-chain and TSH beta revealed a reduced immunoreactivity of PT-specific cells under short photoperiod, but this was not altered in pinealectomized animals exposed to short photoperiod. In situ hybridization against both hormonal subunits paralleled these changes, with a dramatic decrease in hormonal messenger RNA in short photoperiod. This regulatory influence was also blocked in pinealectomy. Taken together, these results demonstrate that transcription and translation of hormonal subunits are regulated by photoperiod in hamster PT-specific cells, whereas expression remained unchanged in short photoperiod if pinealectomy was performed. We, therefore, conclude that in hamsters, the Mel Signal not the light regimen per se, is a direct or indirect Zeitgeber for the transduction of photoperiodic information to the secretory activity in this pituitary cell type.

An ergonomic study of work methods and physical disorders among professional butchers.

Butchers considered that the back and the upper extremities were subjected to high loads from their work. Of the 92% with reported physical disorders, 80% had pains or complaints from more than one body area. Most frequent were disorders of the back and shoulders. 50% of the butchers had been on sick leave due to their disorders. By ergonomic analysis of the work, several work tasks which caused high loads on those body parts in which disorders had occurred were identified. The high loads were found to have the following main causes, which occurred together in many work situations: (1) Exertion of high forces when cutting the meat. (2) Frequent and heavy manual materials handling. (3) Inappropriate working postures. It appeared from the study that the workload on the butchers was unnecessarily high and that it could be the main reason for the high frequency of disorders amongst them. The strain on them was to a large extent caused by bad working postures, which could be improved by introducing improved equipment. The equipment should be such that the working-height may be adjusted both to the individual and for the task performed.

[Critical analysis of the saralasintest in the diagnosis of hypertension (author's transl)].

The reliability of the angiotensin II (AT II)-antagonist Saralasin in the diagnosis of AT II-dependent forms of hypertension was investigated in 61 cases of hypertension of different etiology. In 14 patients, lowering of blood pressure by Saralasin suggested an AT II-dependent hypertension which could be ascertained in 8 patients (5 had undergone successful surgery) by increased levels of plasma-renin-activity (PRA), AT II, PRA-ratio in renal vein blood and by angiography. Besides, depressor reactions by Saralasin yielded additional information in three patients with renovascular hypertension but normal levels of PRA and AT II, in two patients with high renin essential hypertension and one patient with pheochromocytoma. This test seems to be valuable in the diagnosis of renin-dependent hypertension.
