French medical students' opinion of psychiatry.

A questionnaire on attitudes toward psychiatry was submitted to French medical students in their first year of specialized training in psychiatry or in another medical specialty. The findings do not differ from those of previous United States investigations. In France, although psychiatry is a full medical specialty, psychiatrists are perceived as different from other physicians. Both groups of students believed that psychiatric treatment is efficacious but remained unsure of its scientific value.

Clinical and behavioural assessments: a study on the outcome of behaviour therapy in neurotic patients.

The present study compares two independent evaluations of the outcome of behavioural therapy in 33 phobic patients. The raters were a psychiatrist not involved in the therapeutic process and behaviour therapists. Assessments were done before and after four months of therapy. They concerned intensity of fears, anxiety, depressive affects, social impairment. The results indicated a significant agreement on the symptomatic level and on the global evaluation. No significant agreement was found on the level of "well-being".

[Deviations of dietary behavior].

Deviations of alimentary behavior, found in all psycho-pathological sectors, are sometimes isolated but more often accompany multiple psychic symptoms. These deviations are dominated by restrictive behavior. Epidemiological studies, case histories and transcultural data are scarce. Their obtention would undoubtedly yield more satisfactory therapeutics.

Anhedonia and blunted affect in major depressive disorder.

The relation between anhedonia and affective flattening was studied in 61 normal subjects and 61 major depressives. Affective flattening and anhedonia were defined by the following self-rating scales: Fawcett-Clark Pleasure Capacity Scale (FCPCS), Hardy Displeasure Capacity Scale (HDCS), FCPCS-PP (subscale of physical pleasure), and HDCS-PD (subscale of physical displeasure). The depressives are more sensitive to displeasure and more anhedonic than controls. Concerning physical stimulations (FCPCS-PP and HDCS-PD) in the depressed group, anhedonia and affective flattening are not linked, suggesting that physical anhedonia is not secondary to low emotional reactivity (affective flattening) and constitutes an independent dimension.

[Assessment of difficulties in mental representation of visual stimuli in anorexic patients].

The aim of this research is to assess the mental representation associated to visual stimuli for anorexic patients. Two studies are described. The first one is a systematic observation of food representation for five patients within a 3-month period. The disruptive factors, mainly related to food visual aspect decrease with time. Consequently, a second experiment was carried out. Two groups were compared as far as the food mental representation related to visual food stimuli is concerned: a group of anorexic patients and a group of control subjects selected according to sex, age, and educational standard. The results show the existence of habituation process as there is a decrease of disruptions concerning the mental representation of food color, texture and shape. This result must be confirmed on a larger sample.

[Bulimia and depression. Their clinical relations].

The multiplicity of actual studies about a possible correlation between eating disorders and affective disorders is found on clinical and family studies, biological, genetic and therapeutic studies. As depressive symptoms are associated to eating disorders in 30 to 88% of the cases according to the authors, family studies suggest the existence of a relationship between eating and affective disorders but no causal link between the two can be proved. Biological tests are difficult to interpret (the dexamethasone suppression test, the TRH stimulation test, a study of platelet MAO activity, of the metabolism of neurotransmitters). The imbrication of metabolic and neuroendocrine disorders being a characteristic of both affective and eating disorders. On the level of therapy, if the results of antidepressants--essentially tricyclics and IMAO--on bulimia are effective on a short-term basis, this is not enough to prove the dysthimic nature of eating disorders. Moreover, their efficiency on the long term (over 6 months) is very controversial. It is the research on genetic and biological index which will allow the precise characterization of the nature of the relationship between eating (bulimia) and affective disorders.
