Acute effect of an antihypertensive drug, labetalol, on uteroplacental blood flow.

The effect of a new antihypertensive drug, labetalol, on uteroplacental blood flow was determined in eight pre-eclamptic women. After injection of 0.5 mCi of 113mIn the radioactivity in the placenta was recorded by a gamma camera linked to a computer and the placental blood-flow index was calculated from the ratio between the maximum radioactivity of the isotope-accumulation curve and the rise time of the curve. Labetalol, a combined alpha- and beta-adrenoceptor antagonist was given intravenously and after 30 min a second uteroplacental blood-flow index was calculated. There was a significant mean decrease of blood pressure from 147/98 to 128/83 mmHg, but no change in uteroplacental blood-flow index, so that uteroplacental vascular resistance tended to decrease.

Utero-placental blood flow and the effect of beta 2-adrenoceptor stimulating drugs.

The immediate effect of a beta 2-adrenoceptor stimulating drug, salbutamol, on utero-placental blood flow in the last trimester of pregnancy was evaluated in 16 women without uterine contractions, using a method applying 113-m Indium and a computer-linked gamma camera. Serial scintigrams over the placental site showed a 15 per cent increase in activity during a short salbutamol infusion, denoting an augmented blood pool in the intervillous space. At the end of a 25--30 min salbutamol infusion, a mean prolongation of the rise time of the isotope accumulation curve by 100 per cent could be calculated. A utero-placental blood flow index derived from the ratio of the maximum activity and the rise time of the accumulation curve above the placenta showed a decrease ranging from 18 to 50 per cent. It is conceivable that this initial decrease would disappear during continued infusion, as has been shown to occur in the pregnant sheep model.

Uteroplacental blood flow in pre-eclampsia measurements with indium-113m and a computer-linked gamma camera.

Uteroplacental blood flow was measured with a computer-linked gamma camera after intravenous injection of 1 mCi indium-113m. Results of the measurements from 32 pre-eclamptic pregnancies and 37 normal controls are compared. The uteroplacental blood flow was measured as an index calculated from the rise time and maximum activity of the isotope accumulation curve. The uteroplacental blood flow was reduced with 50% in pre-eclampsia. In severe pre-eclampsia it was more compromised than in mild pre-eclampsia. A diminished uteroplacental blood flow was found in pre-eclampsia even in the absence of intrauterine growth retardation. The maternal placental circulation in the supine position was reduced with one third compared to that in the left lateral recumbent position.

Double-blind dose-response study of indomethacin in rheumatoid arthritis.

Indomethacin 0, 15, 25 and 35 mg t.i.d. was given in randomized order for two weeks to eight outpatients with rheumatoid arthritis in a double-blind study. At the end of each treatment period the clinical response was evaluated by subjective and objective methods and the plasma indomethacin concentration was measured by GLC-mass fragmentography. Compared with non-treatment periods, indomethacin had a statistically significant therapeutic effect as judged by global assessment, duration of morning stiffness, use of escape analgesia, articular index and pain score, but there was no relation between the clinical effect and the size of the dose or the plasma concentration of the drug. Technetium uptake over the affected joints did not change during indomethacin therapy, which might reflect a lack of effect on the local activity of the disease. Lower doses than those currently prescribed are probably sufficient to alleviate symptoms in this disease.

Comparison of uteroplacental blood flow in normal and in intrauterine growth-retarded pregnancy. Measurements with Indium-113m and a computer-linked gammacamera.

Uteroplacental blood flow was studied with a noninvasive method using indium-113m and a computer-linked gammacamera. The blood flow was determined from the ratio of the maximum and the rise time of the isotope accumulation curve of the placenta. Eight pregnancies with intrauterine growth retardation (IUGR) were compared with 11 normal pregnancies. In the IUGR group the mean placental blood flow was only 1/4 of corresponding mean values of the normal group. The difference was highly significant (p less than 0.01).

Scintigraphy of induced myocardial infarcts with 99Tcm-gluconate.

A new isotope compound, 99Tcm-gluconate, for detection of myocardial infarction has been tested in dogs. A close correlation was found between the isotope uptake measured in vivo with a gamma camera and the infarct weight of early irreversible myocardial infarcts.

Short-term effects of 1-alpha-hydroxy-vitamin D3 in patients on corticosteroid treatment and in patients with senile osteoporosis.

Six patients with bronchial asthma undergoing long-term corticosteroid treatment and six patients with senile osteoporosis were given the same oral dose of 1-alpha-hydroxy-vitamin D3 (1alpha-OH-D3) and calcium. The immediate effect on blood and urine chemistry and on the intestinal calcium absorption rate were studied. Hypercalcaemia occurred frequently among the patients treated with corticosteroids but not among those with senile osteoporosis. We conclude that corticosteroids do not counteract the effects of 1alpha-OH-D3. No correlation was found between the calcium absorption rate and the degree of osteoporosis, nor did the serum PTH levels show any differences that could be attributed to the treatment.

The effect of beta-receptor-stimulating agents on the utero-placental blood flow.

The influence of salbutamol, a beta-2-receptor-stimulating agent, on the blood flow through the utero-placental unit was evaluated in the human. Serial placenta scintigrams were analysed quantitatively, after injection of 0.5 mCi Indium-113m, by means of a gamma-camera connected on line to a computer. The examinations were performed in the third trimester of pregnancy. No sedation was used. Uterine contractions were not present. Salbutamol caused an increase in activity over the placental region, corresponding to a 15% increase in blood volume. The rise time of the initial phase of isotope accumulation (calculated from 5 to 95% of final activity) was prolonged by 100% during salbutamol infusion. As the rise time is proportional to the volume/flow ratio of blood in the uteroplacental region, our data indicate that salbutamol infusion causes a decrease in blood flow in the absence of uterine contractions.

Influence of absorbed dose and field size on the geometry of the radiation-surgical brain lesion.

Specimens from a number of patients locally irradiated in the thalamus for intractable pain with a multiple gamma beam technique have been examined. The geometry of well circumscribed necrotic lesions was ascertained. No correlation was seen between the dimensions of the lesions and the size of the field, 3 mm X 5 mm or 3 mm X 7 mm, within the clinically useful dose range, 16 to 25 krad.

Destruction of small intracranial tumours with 60Co gamma radiation. Physical and technical considerations.

The clinical and physical conditions for the destruction of small intracranial tumours by means of multiple gamma beam irradiation are evaluated. The effects of the collimation of the single beam and the spatial distribution of 179 beams on the superimposed three-dimensional dose distribution are investigated experimentally and theoretically. Techniques for selective irradiation of tumours of various types and sizes are discussed.

Radiation therapy for carcinoma of the urinary bladder. Radiologic planning and treatment techniques.

In radiation therapy for carcinoma of the bladder, various anatomic and physiologic factors affect the position of the target volume and introduce errors in the treatment technique. The radiologic procedures for planning and follow-up of the patients must consider these factors. The beam geometry is also of decisive importance for minimizing the effect of these factors and for avoiding injury to surrounding structures. The irradiation procedure of choice should be a four beam technique with supervoltage roentgen radiation.

Irradiation of small structures through the intact skull.

The physical conditions for producing minute disc-shaped lesions for functional radiation surgery by means of narrow beam irradiation are discussed. The intention was a treatment procedure for routine clinical use and with high physical and mechanical reproducibility. The possibilities of using high energy electrons, supervoltage roentgen radiation, gamma emitting nuclides or fast neutrons in a technique for routine clinical use were investigated. The radiation of choice taking physical properties, radiation biologic factors and practical circumstances into account was considered to be 60-Co gamma radiation. A treatment apparatus containing 179 60-Co sources within a spherical sector of 70 degrees latitude and 160 degrees longitude was constructed.
