[Selected dermatological diseases of the genital region].

Physicians who are not trained in dermatology often experience difficulty interpreting skin lesions. There seem to be a number of similar lesions, which are difficult to categorize. Thus, in this presentation the various dermatological diagnoses of cutaneous genital diseases are not listed systematically, e.g., cutaneous infections, cutaneous tumors. This presentation is based on the so-called "Effloreszenzen". They are lesions such as macules, papules, and ulcers, which are typical for various skin diseases. Knowledge of these skin lesions is helpful in categorizing skin diseases, not only those of the genital region.

Psoriasis vulgaris treated successfully with mycophenolate mofetil.

Mycophenolate mofetil (MMF) is a new immunosuppressive drug which non-competitively and reversibly blocks the de novo synthesis of guanine nucleotides required for DNA and RNA synthesis during T- and B-cell proliferation. This induces a selective inhibition of lymphocyte proliferation. Thus MMF is currently used to prolong graft survival in renal transplant patients. In this communication we describe the first case of a man with severe psoriasis treated successfully with oral MMF without short-term side-effects. The psoriasis area and severity index score decreased during therapy (5 weeks) from 22.0 to 11.4. Thus MMF appears to be an effective therapeutic alternative in the treatment of severe psoriasis.

[Scintigraphy studies of lymph flow in paralyzed upper extremities].

BACKGROUND: In paralysed limbs quite often edemas are seen, which might pose problems during rehabilitation. The origin of these edemas is not fully understood. METHODS: 48 patients with hemiplegia and edema of the paralysed arm were studied; in 40 patients lymphoscintigraphic studies (static and dynamic lymphography) were performed. RESULTS: In 80% of these cases lymphatic flow in the paralysed arm was increased compared to the healthy arm. If there was marked edema in the paralysed arm, the increase of lymph flow was considerable (p < 0.01). CONCLUSIONS: Edemas in hemiplegic extremities are not due to lymphedema, as in lymphedema the lymph flow usually is very slow. They most likely are caused by disorders of filtration and reabsorption, due to a dysfunction of the autonomic nervous system.

[PUVA-induced acro-bullous dermatosis].

PUVA-induced acrobullous dermatosis is characterized by the occurrence of blisters on the acral extremities during PUVA therapy. The tense blisters apparently arise as a result of PUVA damage to the epidermodermal cohesion coupled with friction or trauma. They must be distinguished from phototoxic reactions induced either by UVA over-dosage or excessive psoralen uptake. We report on, two cases of acrobullous PUVA-induced dermatosis, discussing clinical features, pathophysiological aspects and differential diagnosis.

Eosinophilic pustular folliculitis: successful treatment with interferon-alpha.

Eosinophilic pustular folliculitis (Ofuji's disease) is a rare skin disease of unknown etiology characterized by infiltrated circinate plaques with sterile follicular pustules in primarily seborrheic areas. Several therapeutic regimens have been reported to control the disease with inconsistent results. We here report on a patient with Ofuji's disease, who was successfully treated with interferon-alpha 2b.

[Isotretinoin treatment of acne conglobata. Andrologic follow-up].

Twenty patients (21.4 +/- 4.3 years) with extensive acne conglobata affecting the face, chest and back, were treated for a period of six months with isotretinoin (13-cis retinoic acid, 13-cis RA) at a dosage of 1 mg/kg body weight/day (mean daily dose 72.7 +/- 9.8 mg 13-cis RA). In all cases, the acne conglobata cleared up completely. With the exception of symptoms produced by mucocutaneous exsiccation, no side effects were observed; the laboratory parameters were all within normal limits during the anti-acne treatment phase. There was no recurrence of the disease within a period of one year after cessation of treatment (n = 13). Screening spermatograms performed at two-month intervals during the treatment phase revealed, from the fourth month of treatment onward, a statistically significant increase in the mean sperm density with sperm morphology and motility unchanged as compared with the situation prior to treatment. One year after conclusion of treatment, all measured spermatogram parameters had returned to the initial values. The investigations revealed no evidence of any negative influence of the 6-month treatment with isotretinoin on spermatogenesis.

Concretions in the human testis are derived from the basal lamina of seminiferous cords.

We provide evidence that testicular concretions form exclusively in association with seminiferous cords. Testicular material obtained from 6 patients with various disorders of spermatogenesis was studied by means of light- and electron microscopy. Seminiferous cords in the testes examined form so-called hypoplastic zones. They contain immature Sertoli cells and are surrounded by an unusually thick basal lamina and one or two layers of myofibroblasts. They display extensive coiling and form ring-like structures. Concretions are located at the periphery or in the center of the cords and consist of a hyaline material with an occasional granular core. They are enveloped by Sertoli cells or myofibroblasts. Data presented indicate that concretions may develop within the above-mentioned ring-like structures or in deep indentations of the surrounding connective tissue. Computer-assisted 3-D reconstructions of a seminiferous cord based on 24 serial semithin sections revealed that the majority of concretions communicate with the interstitial tissue through a broad opening comparable to a "meso". On the basis of our observations we could demonstrate that concretions are accumulations of basal lamina that occasionally contain remnants of degenerated myofibroblasts. They form outside the cords and belong to the connective tissue compartment. As the components of the basal lamina are thought to be produced by the interaction of Sertoli cells and myofibroblasts we tentatively assume that the development of concretions reflects a disturbance in this complex interaction. We suggest that the presence of concretions refers to a developmental disorder and may be pathognomonic for the occurrence of defective seminiferous cords.

[Andrologic problems in adolescence].

No precise distinction is usually made now between puberty and adolescence. Therefore, we describe the physiology and pathology of both phases with their characteristic clinical features, hormonal changes, and therapeutic consequences. In addition, we refer to the typical problems of this age group, the psychic and somatic variants of the norm; special regard is paid to puberal gynecomastia and obesity.

[New diagnostic procedures in assessing male fertility].

In 160 patients with normal and pathological semen samples, we studied the penetration of human spermatozoa in standardized bovine cervical mucus (assay Penetrak). Our results indicate that Penetrak can detect those dysfunctions of sperm motility which cannot be diagnosed by conventional semen analysis - The toluidine blue-pyronine staining is an easy way to differentiate between spermatozoa within an hour. The reliability of this technique is comparable to that of conventional methods and therefore appropriate for routine diagnostics. - As case studies on 140 patients proved, the level of carnitine in the seminal plasm can be regarded as a parameter of epididymal function. In combination with the examination of FSH serum levels, this method may be helpful in the differential diagnosis of azoospermia due to obstruction, Sertoli-cell-only syndrome, or spermatogenic arrest. - In severe oligozoospermia, testicular biopsy using semithin sections may be of high diagnostic and prognostic value. In contrast to paraffin sections, this technique is particularly appropriate for the detection of cytological characteristics of germ cells. The type and number of pathological germ cells are decisive regarding the prognosis. Furthermore, a carcinoma in situ (CIS) can be definitely identified by means of semithin sections. - In a study on 2047 patients, we found seminoma cells in 15 cases (0.73%); 5 of these patients already had a solid seminoma in the testicular rete. Risk-patients are those showing oligozoospermia of less than 10 million spermatozoa per ml or azoospermia associated with unilaterally or bilaterally subnormal testicular volume. In these cases, the testicles are inconspicuous, both palpatorically and sonographifically.

Morphology of normal and malignant germ cells.

By means of semithin section histology and electron microscopy the following male germ cells are described: gonocytes and prospermatogonia in the embryonic and fetal testes; spermatogonia and primary spermatocytes in the testis of the adult; abnormal spermatogonia and spermatocytes, which are not tumour cells. In contrast to these cells, four main tumour cell types which on the basis of morphological criteria can be said to be derived from germ cells, are classified and designated as Tc 1 to Tc 4. The various tissues accompanying the tumour cells are briefly described.

Penetration ability of human spermatozoa into standardized bovine cervical mucus (Penetrak) in patients with normal and pathological semen samples.

Semen samples from 89 men were analyzed and evaluated with a standardized bovine cervical mucus penetration test (Penetrak). 29 patients had a normozoospermia, 22 patients a teratozoospermia and 38 patients an oligozoospermia of various degree. The most important parameters of semen analysis were compared with sperm mucus penetration. The sperm mucus penetration test correlated best with progressive motility (r = 0.6758), followed by total motility (r = 0.6302), sperm count (r = 0.6190), and % normal spermatozoa (r = 0.5493) (p less than 0.01 for each correlation coefficient). In the group of patients with normozoospermia 6 cases had been detected with insufficient sperm mucus penetration and in both groups with subfertility 15 cases with adequate sperm mucus penetration. 8 semen samples with normal sperm count and normal sperm motility were used for IVF. Only 6 patients with normal sperm penetration (greater than 30 mm/90') the spermatozoa fertilized the ova, but not from two patients, who had a sperm mucus penetration of less than 30 mm/90'. The study demonstrated that standardized bovine mucus penetration test detects disturbances of sperm motility which may not be discovered by conventional semen analysis.

Cellular mechanisms of action of atrial natriuretic factor.

Atrial natriuretic factor (ANF) interacts with its target cells through specific receptors. This interaction induces, in most cell types, the activation of particulate guanylate cyclase and decreased Calcium mobilisation. In addition, ANF also decreases adenylate cyclase activity in some tissues. Activation of particulate guanylate cyclase, and additionally inhibition of adenylate cyclase, appear to initiate the cellular responses to circulating ANF. The activation of particulate guanylate cyclase is tissue-specific, immediate and can be demonstrated also on the solubilized enzyme. The ANF receptor appears to be tightly coupled to particulate guanylate cyclase. The increased formation of cyclic GMP induces cGMP-dependent protein phosphorylation in target cells. In addition, cyclic GMP inhibits Calcium mobilisation in several tissues. This may explain observations of inhibition of Calcium mobilisation after ANF. Cyclic GMP is not only degraded by phosphodiesterase, but is also extruded from target cells. As a consequence of cGMP extrusion, ANF increases the levels of cyclic GMP in plasma and urine in animals and man. Cyclic GMP is also increased in various disease states, in which ANF is increased. In contrast, cyclic AMP plasma levels are unaltered after ANF elevations. At present, the exact mechanisms, by which cellular functions are altered by ANF are still incompletely understood. It is anticipated, that the close correlation between the cyclic GMP system and effects of ANF in various target tissues is a key finding that will help elucidate the exact mechanisms of ANF action.

[Exocrine and endocrine testicular function following unilateral torsion--a retrospective clinical study of 36 patients].

Of 86 patients who had undergone unilateral testicular torsion the fertility in 36 patients could be examined 1-9 years later. The testis in 27 patients (= 75%) was surgically untwisted. Of 9 patients (= 25%) the testis was removed because of marked hemorrhagic infarction. Fixation of the contralateral testis was prophylactically performed in all cases. The testis of all patients who had undergone the detorsion within the first 22 h were saved and there was no significant difference of the testicular volume compared with the contralateral side. An atrophy of the contralateral testis following prophylactical fixation was never observed. In most of the 36 patients the fertility was diminished: Seminal analysis revealed an oligozoospermia in 44%, a teratozoospermia in 20%, an asthenozoospermia in 20% and a normozoospermia in 16% only. This result was independent of the duration of the torsion. There was no difference in the group of patients with detorsion and the group with orchidectomy. The endocrinological examination revealed a statistically significant correlation between FSH and sperm density and between FSH and the duration of the torsion. But both correlation coefficients were less than 0.5 and are therefore without any clinical relevance. Testosterone, LH and prolactin were normal in all cases.

[Light microscopic studies of spermatogenesis in rats following the administration of a high dose of 13-cis-retinoic acid].

In an experimental study, we investigated the influence of a toxic dose of 13-cis-retinoic acid (40 mg per kg body weight daily) on the spermatogenesis of 30 Wistar rats. 15 rats were killed immediately after 8 weeks application; the remaining 15 animals were put to death after another 8 weeks without application. A control group of another 20 rats only received physiological salt solution daily over a period of 8 weeks. All 30 rats which had been treated with 13-cis-retinoic acid showed as normal a spermatogenesis in all parts of the testes as the 20 control animals.

[Relations between spermatozoa density of germ cells of the testis and hormone analyses (especially FSH) in patients with oligozoospermia)].

The study concerns 84 patients with oligozoospermia between 0.3-20.0 mill. spermatozoa/ml. 20 patients with an obstructive azoospermia served as a collective control. From all patients a testicular biopsy from each testis was conducted. Semithin sections of the testicular tissue were made and the different germ cells in the seminiferous tubules were counted. Furthermore the values of FSH, LH and testosterone in the plasma of every patient were determined. There was a significant correlation between sperm count and FSH (r = -0.30, P less than 0.01), n = 84) as well as LH (r = -0.24, P less than 0.05, n = 84). Also the correlations between the counted germ cells and FSH were significant (P less than 0.01). The closest correlation existed between the spermatogonia A pale + A dark and FSH (r = -0.59, n = 104). But there was no biological significance, since the coefficient of these correlations are not close enough to -1.0. Therefore it is concluded, that the parameters of FSH and LH are not absolutely reliable for judging the spermatogenesis in patients with oligozoospermia.
