Tandem high dose chemotherapy with autologous bone marrow transplantation for initial relapse of testicular germ cell cancer.

BACKGROUND: The purpose of this study was to evaluate the use of two cycles of high dose chemotherapy with autologous bone marrow transplantation (ABMT) in the treatment of patients having a first relapse of testicular germ cell cancer. METHODS: Twenty-five patients whose disease had relapsed after 1 platinum-based regimen received 1-2 cycles of conventional dose salvage therapy, followed by a planned 2 consecutive cycles of carboplatin 2100 mg/m2 and etoposide 2250 mg/m2 with ABMT. Patients were required to have testicular germ cell cancer, adequate organ function, and performance status. The median patient age was 32 years; 3 cisplatin refractory. RESULTS: Conventional dose salvage therapy consisted of vinblastine, ifosfamide, and cisplatin (for 16 patients); etoposide, cisplatin, and ifosfamide (for 3 patients); cisplatin, vinblastine, and bleomycin (for 2 patients); or none (for 4 patients). Twenty-five patients received high dose therapy; of these, 19 (76%) received two cycles. The median time to an absolute neutrophil count of 500 was 12 days (range, 8-20 days) for the first cycle and and 11 days (range, 8-18 days) for the second. The median time to a platelet count of 20,000/microL, independent of transfusions, was 15 days (range, 8-60 days) for the first cycle and 14 days (range, 8-22 days) for the second. Extramyeloid toxicities were as follows: Grade 3-4 stomatitis in 17 of 25 patients, Grade 3-4 nausea/emesis in 12 of 25 patients, and Grade 3 ototoxicity in 3 of 25 patients. At the completion of therapy, nine patients were in complete remission, six had only teratoma found at resection, one had carcinoma resected, six were in partial remission, two had stable disease, and one had progressive disease. With a median follow-up period of 26 months (range, 14-36 months), 13 of 25 patients (52%) had been continuously progression free at last follow-up, 11 of 25 (44%) progressed, and 1 patient died in treatment. CONCLUSIONS: Salvage treatment incorporating brief conventional dose therapy followed by two cycles of high dose therapy resulted in prolonged disease free survival in a proportion of patients with relapsed testicular germ cell cancer.

[Arteriovenous fistula and kidney pelvis thrombus after percutaneous kidney puncture].

Percutaneous renal biopsy was performed in a 40-year old transplant recipient suffering from renal function impairment. The examination of the main transplant artery by duplex Doppler ultrasonography showed normal values for the resistive index and renal blood flow volume. Postbioptic realtime ultrasonography showed an echogenic mass in the renal collecting system, which was dilatated in prior examinations. In addition, we found an arteriovenous fistula by colour coded Doppler ultrasonography showing a characteristic Doppler wave-form shape in an interlobar artery. No changes in the resistive index and renal flow volume could be demonstrated by Doppler examination of the main renal artery. The complications of percutaneous biopsy disappeared spontaneously. Although the incidence of major complications after renal graft biopsy with the automated biopsy instrument is very low, our report demonstrates typical complications following percutaneous renal biopsy. We conclude that B-scan and colour coded Doppler ultrasonography are sensitive diagnostic methods to observe patients post biopsy for potential complications.

Secondary hyperparathyroidism and sonographic evaluation of parathyroid gland hyperplasia in dialysis patients.

Ninety-six hemodialysis patients were examined by ultrasonography of the parathyroid glands to study the prevalence of parathyroid gland hyperplasia and to assess the relevance of sonography in the evaluation of secondary hyperparathyroidism. The results were compared with clinical, biochemical and radiological parameters. Thirty-two (33.3%) patients had sonographically enlarged glands. Of them 19 had 1 and 13 had 2 and more enlarged glands. Patients with enlarged glands, compared to those with undetected glands, had a significantly higher frequency of bone and joint pains (65.5% vs 40.6%), radiological features of hyperparathyroid bone disease (in hands 28.1% vs 6.9%, in acromioclavicular joints 37.5% vs 13.6%) and higher levels of intact serum parathyroid hormone (1-84) concentration (52.8 +/- 47.9 pmol/l vs 18.1 +/- 18.0 pmol/l) and serum alkaline phosphatase concentration (260.2 +/- 201.1 U/l vs 129.8 +/- 127.3 U/l). Those with enlarged glands had been on dialysis for a longer period (87.7 +/- 51.0 months vs 62.5 +/- 47.4 months). The severity of secondary hyperparathyroidism increased with the number of enlarged glands. Our study shows that ultrasonography is a useful noninvasive screening method for the evaluation of secondary hyperparathyroidism in patients on hemodialysis and that sonographically enlarged glands may be a measure of the severity of secondary hyperparathyroidism.

[Grouped and combined blue nevi].

The simultaneous occurrence of blue and naevocytic naevus in the same lesion is rare and can be difficult to diagnose. Precise differentiation from nodular melanoma may only be possible by histopathological examination. Agminated blue naevi, first described by Upshaw et al. [25] in 1947, are also rare. It is important to differentiate these lesions from nodular melanoma with satellite metastases. We describe two patients with such unusual blue naevi.

[Computer-assisted analysis of the tocogram. Initial experience using a new evaluation procedure].

This paper describes first observations with a new computer aided analysis of tocogram. It seems that length of uterine contraction is an important parameter for quality of labour. In 9 from 12 cases promotion to constant length of labour with relatively low absolute duration of contractions was connected with a distinct increase of the velocity of cervical dilatation. The uniformity can be an important factor for the conversion into active phase of labour.
