Formation of adherent monolayers of murine lymphocytes in vitro: the use of serum-free medium and concanavalin A-coated surfaces to promote adherence.

Murine thymus and spleen cells formed adherent monolayers in polystyrene tissue culture flasks when plated in serum-free medium. In the presence of 2% serum, thymus cells adhered poorly, but adherence was greatly enhanced if the flasks had been coated noncovalently with the lectin, concanavalin A. Adherence of leukemic lymphocytes (L1210) required both serum-free medium and concanavalin A-coated flasks; the extent of attachment was proportional to the concentration of the lectin used to coat the flasks at concentrations up to 0.1 mg/ml. Once L1210 cells had attached, they could not be removed by exposure to serum, ethylenediamine tetraacetic acid, trypsin, or alpha-methyl mannoside. Adherent L1210 cells remained capable of metabolism and proliferation during intervals of up to 7 days. The use of adherent monolayers for cytotoxicity assays was demonstrated by an assay for Pseudomonas aeruginosa toxin in EL4 murine leukemia cells.

[Treatment and prognosis of Wilms' tumor. Analysis of 30 patients (author's transl)].

The extraordinary results of treatment of Wilms' Tumor has made it a model for interdisciplinary treatment of malignancies in children. Grouping by pathological and clinical findings helps to estimate the prognosis and to establish an individual therapeutic protocol. Since 1972 the overall 5 years survival rate of our patients (n = 30) is 86%.

[Early ureteral complications after renal transplantation (author's transl)].

Ureteral complications occurred in 23 cases after 225 renal transplantations. The complications consisted of 17 urinary fistulas and 6 stenoses of the ureter. Fourteen of the urinary fistulas were successfully treated by conservative management. Two transplants were lost secondary to operation because of ureteral obstruction. Factors contributing to ureteral leakage were the technique of ureteral anastomosis, the handling of the donor ureter during donor nephrectomy and possibly ureteral necrosis by rejection crises. Ureteral stenosis and rejection crisis may have the same symptoms. Urinary fistulas can easily be observed by the urine extravasation. The treatment of urinary fistulas can be managed only by replacing the suprapubical drainage. The stenosis of ureter with subsequent deterioration of graft function needs an operation with the danger of loss of the transplanted kidney.

Ureteral replacement with a new prosthesis.

A new flexible silicone ureteral prosthesis has been developed and implanted by a new technique in 20 miniature pigs. The improved implantation technique and the results are described. All prostheses remained firmly attached in their position and drained the upper urine collecting system adequately during an observation period up to 12 mos. Leakage, incrustation, dislocation or rejection did not occur. The promising results encouraged us to continue our investigations to get long term experience for clinical trial in human patients for whom nephrostomy is the alternative.

[Which arguments favour radiation therapy in induratio penis plastica? (author's transl)].

Vitamin E and percutaneous irradiation are the most common treatment regimens of Peyronie's disease. This report presents the results of 49 patients irradiated with 137Cs gamma rays. 28 of them had also received Vitamin E. An objective improvement was achieved in 37 patients (75%). Vitamin E did not show any additional benefit. In spite of the fact that these results do not permit an objective evaluation of both treatment regimens, the following advantages of radiotherapy should be considered: a) short treatment time b) low costs c) no serious side effects.

[Ureteral drainage with the Gibbons indwelling ureteral stent (author's transl)].

The technique of endoscopically placing the Gibbons indwelling stent into the obstructed ureter is described. This technique offers several advantages, especially in managing poor-risk patients whose ureters are obstructed: 1. endoscopic placement of the ureteral stent is associated with less morbidity and mortality than supravesical diversion; 2. quality of life is much less reduced than in supravesical diversion; 3. it is readily reversible; 4 the ureteral stent does not interfere with subsequent operation and offers several advantages over PVC-splints and other materials used for long term ureteral drainage. The placement of an indwelling stent sometimes causes technical problems but complications are rare. 21 Gibbons indwelling ureteral stents have been placed in obstructed ureters of 16 patients at the age of 25--74 years. Our technique, indications, results and complications are described herein.

[Percutaneous radiation therapy of peyronie's disease with 137cesium (author's transl)].

From 1966 to 1977, 49 patients suffering from Peyronie's disease received percutaneous radiation treatment with 137 Cesium (137Cs). Therapy results were followed up for a period of from 2 months to 11 years. The method of 137Cs radiation is described and the results compared with other forms of therapy. Percutaneous radiation therapy with 137Cs seems to be the preferred treatment of Peyronie's disease because of its few side effects, rapid effectiveness, and low cost.

[Replacement of the trigonum vesicae by silicone rubber in mini pigs (author's transl)].

Prostheses of the trigonum vesicae made of silicone rubber and covered with a textile layer were implanted in 18 mini pigs. Investigations were done in two series: in the first the prostheses were covered on the outside with a narrow woven textile layer, in the second with Dacron-velour. While the narrow woven textile-lined prostheses were rejected into the urinary bladder after a short period, the Dacron-velour-lined prostheses remained well attached to the bladder wall.

[Embryonal rhabdomyosarcoma of the urinary bladder in childhood. Report of five own cases].

Malignant lesions of the bladder in infancy and early childhood arise from mesenchymal tissue. Among the mesenchymal tumors embryonal rhabdomyosarcoma is more common in young children than in adults. The gross appearance of rhabdomyosarcoma of the bladder is typically that of polypoid grapelike clusters of tissue, termed "sarcoma botryoides". Histologically, the lesion is consistent with embryonal rhabdomyosarcoma. The growths originate in the submucosal layer of the trigone and the internal urethral orifice. Rhabdomyosarcoma (RMS) of the bladder in children tends to remain within the disease. Deep extension and metastasis to regional lymph nodes and to distant sites is a relatively late occurrence. The symptoms are characteristically those of obstruction and strangury. Hematuris is uncommon. The intravenous urogram shows characteristic lobulated filling defects. Treatment of choice is almost always total cystectomy. Suprapubic cystotomy must be prohibited both as a method of drainage and as an approach for biopsy since tumors ulcerating through the abdominal wall after cystotomy and local excision have been observed. With concomitant administration of radiation therapy in selected cases and the combination of surgery with multiple drug chemotherapy, promising results have been obtained in the therapy of embryonal RMS of the bladder in children.
