Malignant catatonia--a continuing reality.

A comprehensive review of the world literature confirms that malignant catatonia continues to occur and is reported and discussed in psychiatric community. It is associated with affective disorders as well as schizophrenia. The pathophysiology is still unclear but most reports suggest a hypodopaminergic state. Its reported mortality has declined from 75-100% in the preneuroleptic era to 31% during the review period (1986-1992). Bomocriptine, dantrolene, and benzodiazepines have been used with success but ECT has been found to be life-saving. The present review supports earlier reports that neuroleptics are frequently lethal in some patients with malignant catatonia.

Diagnostic concordance between DSM-III, Feighner, and RDC.

The NIMH Diagnostic Interview Schedule was administered by psychiatrists to 216 individuals. The DSM-III, Feighner, and RDC diagnoses derived from the computerized interview results were then compared for eight psychiatric disorders. Rates of diagnostic concordance among the systems are given, and the causes of diagnostic discrepancies are discussed. Diagnostic concordance was highest for mania and alcoholism and lowest for schizophrenia and antisocial personality disorder. Implications of these findings and future research directions are discussed.

Emotional disturbance in hearing clinic patients.

A group of outpatients scheduled for hearing evaluation were screened for psychiatric morbidity using the General Health Questionnaire (GHQ-30). There was an association between objective hearing loss and elevated GHQ-30 score. An association was also found between the presence of tinnitus and vestibular symptoms and elevated GHQ-30 score.
