[Anaphylactic reaction to protamine in cardiovascular surgery].

The authors describe a serious anaphylactic reaction after protamine administration and its successful control. In cardiovascular surgery protamine a basic peptide, is used as a matter of routine to eliminate the anticoagulation effect of heparin. The use of this drug is associated with a number of negative haemodynamic reactions, most of them not serious, the incidence of which varies from 0 to 100%. The incidence of serious reactions associated with a life threatening drop of the blood pressure is reported to be between 0.13% to 4%, depending whether a retrospective or prospective study is involved. The pathogenesis of this anaphylactic reaction is not quite clear. Prevention in patients with known risk factors is problematic. Treatment of the allergic reaction to protamine is still symptomatic. In the authors' patient the condition was coped with by administration of colloid solutions, adrenaline and solumedrol.

[Anesthesia using high doses of fentanyl in cardiac surgery].

The authors describe their own experience with anaesthesia using large doses of fentanyl in patients operated on account of ischaemic heart disease and acquired valvular defects. Considerable stability of the circulation and careful monitoring of cardiorespiratory parameters along with aimed influencing of serious deviations is the basis of success of this procedure, which makes it possible to perform cardiosurgery in high risk patients. The authors try to improve the described method further by using some modern procedures.

[Catabolism after direct myocardial revascularization].

The objective of the investigation was to assemble more knowledge of catabolic processes after cardiosurgery. In 14 patients after 6-hour intervals following an aortocoronary bypass the total production of urea nitrogen, Bistrian's catabolic index and the sodium:potassium ratio in urine was assessed. The urea nitrogen production varied within a range of 5.4-9.5 g per 12 hours. These values when calculated per 24 hours are 94-108 g of protein. The catabolic index was in the range of 5.8-11.2, i.e. within the zone of intense stress. Also the values of the Na/K ratio (1.0-1.9) suggested intense stress. The authors discuss the causes and possibilities to relieve the stress response.

Serum enzyme activity in bone tumors and osteomyelitis (LDH, GOT, GPT, CPK, CHE, ALP, AP, PP, ALD).

Enzyme activity of lactate dehydrogenase, glutamate-oxalacetate and glutamate-pyruvate transaminase, creatine phosphokinase, cholinesterase, alkaline, acid and prostatic phosphatase and aldolase has been studied in a total of 213 subjects, of whom 97 were of good health, 63 had bone tumors and 53 suffered from osteomyelitis. The activities of the majority of the enzymes were found to become significantly changed in comparison with the norm. In both patient groups, the more striking differences being noted in that of osteomyelitis. However, enzymatic activity alone does not allow to differentiate the group of bone tumors from that of osteomyelitis, the differences between these two groups not being of significance in any one of the enzymes followed.
