The basket plate in the osteosynthesis of comminuted fractures of distal pole of the patella.

The purpose of this study was to evaluate the basket plate in the treatment of comminuted fractures of the distal pole of the patella (patellar apex fracture). The basket plate was designed by Smiljanić Branimir M.D., Clinical Professor of Surgery, at the Department of Surgery, The University Hospital Sestre milosrdnice, Zagreb, Croatia. In the period from 1988 to 2003, more than 100 patients with comminuted patellar apex fractures had been treated with basket plate osteosynthesis in the Department of Surgery. Only 51 patients were subsequently available for knee examination. For evaluation of the knee, we used a modified Cincinnati knee rating system, using a manual dynamometer in the evaluation of the knee extension. The results were excellent in 30 patients, good in 16 and satisfactory in 5 patients; no poor results were observed. The stability of the osteosynthesis by basket plate allows osseous consolidation if the fracture and permits loading the leg with full body weight early in the postoperative period.

[Surgical procedure in the treatment of pertrochanteric fractures].

The authors discuss the problem of treatment of pretrochanteric fractures and emphasize that this type of fracture generally heals well and that it is characterized by fast callus formation. Conservative therapy in coxofemoral plaster often leads to serious complications and has a high mortality rate. Lasting extension according to Russel demands a long hospitalization period and significantly diminishes the number of surgical beds available. Surgical therapy has several advantages: it has far fewer complications and a mortality rate which is only about half as great as in conservative therapy. The authors evaluate their results obtained in 120 cases of pertrochanteric fractures treated surgically as satisfactory. In 106 cases osteosynthesis according to the AO technique and in the remaining 16 cases osteosynthesis using 3 to 4 Kirschner wires percutaneously were performed.

[Our experiences with total hip prosthesis model "St. George"].

The advantages of the total prosthesis of the hip in the treatment of some degenerative diseases and single traumatic impairments of the hip are described, and the main characteristics of the St. Georg model total prosthesis of the hip are discussed. The results obtained in 92 cases of total prosthesis of the hip have been satisfactory. Special attention has been paid to measures for the prevention of infections. Among the patients treated only one developed infection. The author points to the fact that it is possible to replace an infected prosthesis and achieve complete cure by using bone cement mixed with an antibiotic. The functional results obtained are good: pain is either completely relived, or significantly diminished on walking and movement. During the author's five-year experience signs of wear of the prosthesis have not been recorded in any case.

[Preoperative irradiation of larynx and hypopharynx carcinoma].

30 patients with carcinoma of the larynx or hypopharynx were irradiated preoperatively with a dosis of 1,500 to 4,500 rads. The results are compared with those of an identical group of patients who were only operated. The operation was mostly partial (20 cases). In laryngeal cancer the cure rate after two years was 69% and 47%, in hypopharyngeal cancer 43% and 28% respectively.

[Use of a 7.5% NaCl/6% Dextran 70 solution in the prevention of hemodynamic disorders during surgery of abdominal aortic aneurysms].

The experience in the application of infusion solutions Ringer lactate (RL) and 6% of Dextran 70 is shown in order to replenish intravascular volume in 19 patients with the abdominal aorta aneurysm after the removal of the aortal clamp and hypertonic-hyper oncotic 7.5% NaCl/6% Dextran 70 solution in 4 to 5 ml/kg dose in 9 patients. The application 7.5% NaCl/6%Dextran 70 at the optimum time, 15-20 min before the removal of aortal clamp, in a dose of 4-5 ml/kg allowed the support of normal hemodynamic function and decreased the need for some solutions or blood, as well as the incidence of intra- and postoperative complications.
