[Injuries of the spine and spinal cord as isolated and multiple injuries].

A series of 511 patients after vertebral column and spinal trauma admitted to Department of Neurosurgery of University of Medical Sciences, Poznań from 1979 to 1992 has been analyzed. Isolated vertebral and spinal injuries in 49 per cent. In 51 per cent they were associated with multiple injuries. Traffic accidents constituted the main cause (50 per cent of multiple injuries). The severity of the injury was assessed on the base of AIS, ISS rating systems and spinal symptoms. The concomitant multiple injuries were no cause of delay to surgical intervention within vertebral column. The short-term results of the treatment were better in isolated spinal injuries, with 5 per cent mortality rate in both groups.

[Intraoral surgical approach to tumors destroying the axis. Report of three cases].

Primary and metastatic tumours of the axis vertebra are difficult surgical problems. The authors report 3 patients with axis tumours who underwent surgery in 1993 and 1994. Clinical symptoms and signs included pain in three, myelopathy in two, vertebrobasilar circulatory insufficiency and cranial nerve lesions in two patients. Tumours have been resected through transoral and posterior pharyngeal wall approach. The purpose of surgery was decompression of nervous and vascular structures, and histological verification of the tumour. Tumour pathology was diverse and included: multiple myeloma, chordoma and metastatic tumour from stomach. There were no postoperative complications and improvement of neurological conditions was observed.

[Vertebral column primary neoplasms].

Clinical analysis was carried out in 53 cases treated surgically because of primary vertebral column neoplasms in Department of Neurosurgery in Poznań between 1976-1995. Plasmocytoma solitaire (9 cases), angioma (5 cases), osteoblastoma (4 cases) and aneurysmal bone cysts (4 cases) were observed most frequently. Neoplasms localization was as follows: cranio-vertebral junction (4 cases), cervical segment (14 cases), thoracic (23 cases) and lumbo-sacral segments (12 cases). Severe neoplastic vertebral column destruction was disclosed in 34 (64%) patients, but complete transversal spinal syndrome was noted only in 7 (13%) cases. Surgical procedures: posterior and latero-posterior approaches were applied in most cases while anterior and lateral approaches--were rarely used. In 3 cases two stage operations were performed. Intervertebral fusion or posterior autogenous graft was necessary in 39 cases.

[Lumbar intervertebral disk herniation in children].

A consecutive series of 16 children from 9 to 18 years old, with lumbar intervertebral disc herniation who were treated surgically in Department of Neurosurgery Medical Academy in Poznań, between 1978-1992, was analysed. The clinical symptoms, X-ray assessment, type of operation and outcome are reviewed. In our analysis characteristic features of disc herniation in children are: the onset of pain due to violent injury in relation to lifting, bilateral neurological symptoms and radiculography imaging central intervertebral disc herniation.

[Tension measurement of pulse pressure of herniation in children].

Nine patients were operated on in Department of Neurosurgery in Poznań throughout 1990-1993 because of the insufficient carotid blood circulation. In patients measurements of the pulsation pressure of the carotid artery were carried out with a tensometric sensor of pressure during surgery. Neurological symptoms, EEG, evoked potentials and other examinations were conducted before an after operation and correlated with results of pulsation pressure.
