[Iopromide, a new contrast medium for angiography. Clinical study and comparison with other low-osmolar contrast media].

In a multi-centre randomized double-blind study the ionic dimer ioxaglate was compared with the nonionic monomer iopromide. In a second randomized double-blind study iopamidol was included in the trial. It was shown that with low-osmolar substances sensitivity to pain and warmth occurred to only a slight extent, whether the contrast medium was ionic or non-ionic. An advantage of the non-ionic preparation was the definitely lower incidence of skin-rash, dizziness, nausea and vomiting.

Angiography↗

[Ureteral duplication with extravesical ureteral orifice].

The renal segment supplying an ectopic ureter is nearly always dysplastic. Usually the ureters cross and the ectopic ureter terminates caudally in relation to the ureter from the normal portion of the kidney. Extravesical termination in women leads to urinary incontinence, since it frequently ends in the vagina or the labia. In men, the effects are much less noticeable. The ureters usually terminate in the urethra, the seminal vesicles, the ductus deferens or the epididymis. Duplication of a ureter or mega-ureter with dysplasia of the appropriate part of the kidney can be readily explained embryologically.

Adult↗

[Co-existence of fibrous bone dysplasia and familial intestinal polyposis].

The aetiology of fibrous bone dysplasia is unknown. Combinations with endocrine and non-endocrine diseases are recognised. The co-existence of polyostotic fibrous dysplasia with Peutz-Jeghers syndrome has not been previously described. A patient is now reported who had both these diseases. They have in common a benign proliferative tendency in various organs, but the common denominator is unknown. The basic relationship between the conditions requires further observation and study.

Adult↗

[Primary duodenal carcinoma. Clinico-radiologic study].

Primary carcinoma of the duodenum represents about 0.35% of all gastrointestinal carcinomas and is thus a rare disease. The radiological diagnosis of a duodenal origin is made more difficult by the close proximity of the pancreas, biliary system, right kidney and transverse colon. Histologically, duodenal malignant tumours are usually adenocarcinomas. Between 1973 and 1983, eight patients with primary duodenal carcinomas were diagnosed at the University of Tübingen. Average age of the patients was 67 years. There was no sex difference. Average survival was 3.3 years.

Adenocarcinoma↗

Iopromide. Clinical experience with a new non-ionic contrast medium.

The first clinical experience with a new non-ionic, renally excreted contrast medium is described. It is stable in aqueous solution and thus directly applicable. Urography was performed in 7 patients after intravenous injection of 30 to 100 ml, 300 mg I/ml. The usual laboratory examinations of blood and urine parameters and the determinations necessary for complete pharmacokinetics were performed for 6 days after the examination. In 6 healthy volunteers the laboratory parameters obtained in the patients were tested for reproducibility. The kidneys and the pelvicalyceal details were well demonstrated but the urinary bladder only partly so, due to the reduced diuresis. No change of laboratory parameters which could be considered to be caused by the contrast medium itself occurred. Fluctuations in the enzyme activities within the normal range during the first 12 to 24 hours were considered to be circadian fluctuations. Microscopic haematuria found in some patients was not confirmed in the volunteers. The pharmacokinetics were determined by the high polarity of the substance. The heterotopic elimination was negligible. The renal elimination took place by glomerular filtration. No metabolization of the contrast medium was found. According to the results obtained in a sample of 13 subjects the substance appears suitable as a contrast medium for urography and angiography.

[Changes in thyroid function following iodine-containing contrast administration in patients in an endemic goitre area (author's transl)].

A prospective study of thyroid function after the administration of iodine-containing radiographic contrast media was carried out on 119 patients from an area where goitres are endemic. Eighteen patients had a negative TRH test after 28 days. 27.9% of patients after oral cholecystography, 15.1% of patients after intravenous cholangiography and 5.3% of patients after infusion urography, showed hyperthyroid function. In these patients, thyroxin and triiodothyronine levels were higher than in euthyroid patients. Thyroid stimulating antibodies could not be demonstrated. Induction of hyperthyroidism by iodine is considered to be due to autonomy of the thyroid gland parenchyma. For this reason the risk of iodine-induced hyperthyroidism is greater in patients from a goitre endemic region (by a factor of 1.3 to 17.1).

Contrast Media↗

[Unusual abscess formation in the posterior mediastinum (author's transl)].

Pathological processes in the interpleural space appear on x-ray film as space-occupying growths. The assessment of such processes will always require x-ray films of the thoracic organs in two planes, rotating fluoroscopy and spot films as well as tomography. Additional information is obtained via oesophagus contrast radiography, angiography and mediastinoscopy. Computer tomography, being a non-invasive method, has proved very effective in such cases. Formation of an abscess in the mediastinum is rare, since the usually preceding mediastinitis is fatal even before an abscess is formed. In patients from countries where tuberculosis is still frequent, the possibility of formation of a cold abscess via liquafaction of specifically infected lymph nodes must be included in the differential diagnosis of mediastinal enlargement.

Abscess↗

[Iodine-induced hyperthyroidism due to contrast media (author's transl)].

A prospective study in an area with endemic goitre showed 18 patients (15.1%) out of 119 investigated with iodine containing contrast media to be hyperthyroid after 28 days. Oral cholecystography led to the highest (27.9%) and infusion urography to the lowest (5.3%) rate of hyperthyroidism. Determination of the serum concentration of thyroxine, triiodothyronine, basal and stimulated thyrotropine is not a suitable method to detect the risk for hormonally euthyroid patients before a contrast media investigation.
