Local antiseptic therapy for non-gonococcal urethritis: phase II study with Instillagel.

Non-gonococcal urethritis (NGU) is conventionally treated with oral antibiotics. With this Phase II study, we investigated the action of a locally disinfecting substance, Instillagel, in symptomatic NGU. Instillation treatment was performed twice daily to 32 male patients with symptomatic NGU. To evaluate the therapeutic outcome, a smear was taken from the urethra and an urine examination was performed at baseline as well as at 5 and 8 days after the start of the treatment. Pain and micturition symptoms were determined by a questionnaire with analog scales taken before and after therapy. A pre/post comparison of the urethral smears of the patients with symptomatic NGU showed a significant difference (P < 0.0001). The microbial count in the urine did not show a significant difference. The symptoms micturition (P<0.0001) and pain in the urethra (P<0.0001) were significantly improved. This Phase II study confirmed that local antiseptic treatment of NGU can offer an alternative to systemic antibiotic treatment.

[Early results following radical prostatectomy in patients with capsule invasion, seminal vesicle infiltration and micrometastases].

One hundred thirty-three patients with prostatic carcinoma underwent bilateral pelvic lymphadenectomy and radical retropubic prostatectomy between 1975 and 1988. Patients who had a localized prostatic carcinoma (less than or equal to T2b N0 M0 or less than or equal to B 2) and a small number (n = 10) with limited T3 carcinoma were considered for surgical therapy on the basis of a digital prostatic examination. Histological examination revealed locally advanced prostatic carcinoma in 89 patients with capsular infiltration or perforation and seminal vesicle involvement. Microscopic lymph node metastases were noted in 14 cases. Some patients with capsular perforation and seminal vesicle involvement received adjuvant therapy (orchiectomy or radiation). All patients with lymph node metastases were treated by orchiectomy. One local failure occurred among 24 patients with capsular infiltration within 42 months of follow-up. No failure occurred in stage pT3 disease (capsular perforation) with adjuvant therapy (n = 12) and in stage pT3 disease (seminal vesicle involvement) with (n = 9) and without (n = 12) adjuvant therapy after mean follow-up periods of 35, 42 and 52 months, respectively. Distant metastases occurred in 2 patients with stage pT3 disease (capsular perforation) without adjuvant therapy (n = 18) within a mean follow-up of 51 months, and 1 of these patients died of prostatic cancer. Distant metastases occurred in 3 patients with pT2-pT3N1 disease within a mean follow-up of 54 months: 1 of these patients died of prostatic cancer. Local failure was noted in 1 patient in this group.(ABSTRACT TRUNCATED AT 250 WORDS)

[The work therapy workshop as part of a psychiatric service system--its contribution to the integration of medical, occupational and social rehabilitation].

The limitations faced by purely clinical facilities in providing comprehensive rehabilitation for long-term schizophrenic patients having been made increasingly clear by a changing labour market, the Department of Social Psychiatry's community mental health system had already in 1978 been extended to include a work therapy shop, i.e., a printing shop geared to general economic conditions. This effort of integrating various rehabilitative activities "under one roof" within the workshop while maintaining the continuity of medical treatment, is discussed by a synoptical presentation of findings from quantitative und qualitative analysis. It has been possible to show that a wide spectrum of graded rehabilitative activities can be implemented successfully in a well-defined, work life oriented setting. Given close linkages of the workshop with a structured, comprehensive care system, the advantages inherent in integrating medical, vocational, and psychosocial rehabilitation efforts obviously outweigh the disadvantages of insufficient specificity and diversification.

[Surgical therapy of erectile impotence of vascular venous origin].

We report about 32 men, who suffered from erectile impotence due to venous leakage. Three methods of penile surgery are described, the ligature of plexus santorini, the ligature of deep dorsal vein and the ligature of ectopic veins of the penis. By these operations 72% of the patients achieved potency again.

[Various staining procedures in exfoliative urine cytology. Use in routine urology].

Exfoliative urinary cytology should be included in the diagnostics and follow up of bladder carcinomas. Staining according to Papanicolaou is time consuming and expensive. Therefore the accuracy of cytology after methylene blue staining, use of prestained slides and Papanicolaou staining was investigated. Urine and bladder washouts were taken from 77 patients with histologically proven bladder tumors, 152 patients after resection of the tumor and 100 patients without tumor of the urinary tract. The results show comparable accuracy of the different staining techniques. Methylene-blue stained and prestained slides cannot be used for documentation; therefore all specimens, which show tumor or are suspicious should also be stained according to Papanicolaou.

[Endometriosis of the efferent urinary pathways].

Two cases of ureteral endometriosis and 1 case of vesical endometriosis are reported and a short review of the literature is presented. The diagnostic problems, despite modern image procedures, and therapeutic modalities are discussed.

[Familial pheochromocytoma].

A familial pheochromocytoma disease is described on the basis of two siblings and a cousin. The main symptoms consisted of troubles with the eyes, inclination to perspire and high blood pressure. Special in this family disease was the marriage between relatives. The method of surgery consisted in a transverse upper abdominal incision with excellent surgical exposure.

[Urological complications of the infusion urogram (author's transl)].

Among the patients of a urological clinic, within 5 years 15 cases of spontaneous rupture of the renal calices with urinary extravasation were seen after infusion urograms. At 1.5% of the patients, they are rare. In two cases septicemia with endotoxin shock occurred as unusual complications and in two cases provocation or exacerbation of a pyelonephritis. The clinical picture is described in the case records. Pathophysiological problems, therapeutic possibilities and indications for infusion urography are discussed.

[Treatment of advanced metastasizing carcinoma of the prostate with estracyt (author's transl)].

We treated 41 patients with advancing metastatic carcinoma of the prostate in our hospital for 21 days by giving 2 X 150 mg Estracyt intravenously per day. We saw good clinical results in 32 of the 41 patients (= 78%). We found a statistically significant (P less than 0.05) decrease of the acid and prostatic phosphatases. There was a significant (P less than 0.05) increase of the alkaline phosphatases. We did not see any renal or hematologic toxicity. Ten % of our intravenously treated patients experienced thrombophlebitis at the site of injection. Estracyt showed good clinical results.

[Arterial blood supply in double kidneys and kidney fusions].

Double kidneys and renal fusions are the most common anomalies of the upper urinary tract. The effecting factors are still widely unknown. Very often these kidneys are the location and reason of recurrent infections, stone-forming and hydronephrosis. Operations like nephrotomies and partial resections, in which the preoperative knowledge of the arterial blood supply is of great help, have to be performed on them. In a series of 600 kidneys which were examined by pyelography and angiography we found among the 549 kidneys with a normal, single collecting system multiple renal arteries in 13,3% of the cases. In 27,5% of 51 double kidneys and renal fusions in this series we saw more than one artery, counting up to 4 arteries. A connection between the extent of renal duplication and the number of arteries could not be seen. Prior to parenchymal operations on double kidneys and renal fusions a renal arteriography should be performed, as multiple arteries and uncommon arterial origins are to be expected.
