[Adnexal torsion during pregnancy. Diagnostic and therapeutic problems in Burkina Faso].

The authors report a case of adnexal torsion during early pregnancy. Because of a late diagnosis they had to practise an unilateral adnexectomy with a conservative medical treatment of the pregnancy. They recommend an early surgical treatment, that will save the adnexa and permit a normal evolution of the pregnancy. In a young patient, the loss of an ovary may be a tragic event.

[Maternal mortality in Guinea: magnitude and perspectives].

In recent years maternal mortality in developing countries has become a public health priority for international organizations. However, measuring the true magnitude of this problem is the subject of much methodological debate. In less than a century, the rate of maternal mortality in most industrialized countries has decreased one hundred-fold. In contrast, the rate remains very high in most developing countries, particularly in Africa. In this article based on the studies conducted in Guinea we review various operational proposals which may contribute to improving maternal health and reducing maternal morbidity and mortality, with particular reference to the work of the French speaking organization "Santé Maternelle Internationale".

Level and causes of maternal mortality in Guinea (West Africa).

In order to evaluate the level of maternal mortality at Conakry, capital of Guinea (West Africa), a descriptive epidemiological study was made of all maternal deaths occurring between July 1st, 1989 and June 30th, 1990. To ensure that cases of maternal death were recorded as exhaustively as possible, we conducted this study over 1 year in municipal and hospital maternity units, and 3 months in the urban community. One hundred thirty-nine maternal deaths were registered, representing an annual maternal mortality rate of 559/100,000 live births. The main causes of maternal death were abortion, complications linked with hypertension, and postpartum bleeding.

[Uterine rupture. Apropos of 155 cases observed in Conakry (Republic of Guinea)].

From 155 cases of uterus rupture observed during 5 years at the CHU of Donka (Conakry), the authors are reminding: a) the high frequency of this obstetrical emergency: 0.74% of child births and 12.76% of the extractions by caesarean section: b) the high rate of maternal mortality: 25.16%. They insist on: 1. The important role of some parameters: socio economic conditions, lack of an appropriate infrastructure and of a qualified staff leading to a lack of obstetrical observation, parity, age of pregnant women, way of recruiting.... 2. The necessity to apply a correct supervision of the pregnant women and of delivery by establishing a good system of prenatal examination. The therapeutic problems, including the future of the mother and of the child are discussed, insisting on the necessity of the chirurgical celerity in case of dystocia.
