[Immunomorphologic and ultrastructural features of the thymus in myasthenia].

Morphologic, ultrastructural changes in the epithelial component of the thymus in 41 patients with myasthenia are analyzed. Studies by histochemical, ultrastructural, and immunofluorescent methods revealed that the severity of the clinical course of myasthenia was related to the degree of changes in the epithelial component of the thymus, in its specific structures, Hassal's bodies. Effect of thymic epithelial component on lymphoid immunogenesis-regulating component was detected.

[Surgical treatment of myasthenia gravis in children].

Thymectomy was included in the complex of therapeutic measures in the management of generalized myasthenia in 41 children. Much attention was given to preoperative management in which plasmapheresis was applied along with the traditional methods. As a result, maximum compensation of the myasthenic disorders was achieved and spontaneous respiration was restored in 9 patients who were given artificial ventilation of the lungs before the operation. The authors describe the techniques of thymectomy and the changes found in the thymus during histological study. No complications occurred. All of the children were discharged from the clinic in an improved condition. The late-term results were studied in 38 children in follow-up periods of 6 months to 4 years. Marked stable improvement was recorded in 31 (77.5%) patients.

[Efferent methods in the complex treatment of myasthenia gravis].

The results obtained with exchange plasmapheresis and hemosorption used as elements of myasthenic patient preparation for thymectomy and postoperative treatment are summarized. The efferent methods were used in 95 patients, and positive shifts were recorded by electromyography and immunologic studies. Isolated or combined use of exchange plasmapheresis and hemosorption in severe myasthenia makes it possible to prevent postoperative myasthenic crisis and prolong remission or essentially improve patient's condition at late dates after thymectomy.

[Current methods of pathogenetic therapy of infectious-allergic polyradiculoneuritis].

This is a survey of the experience in using corticosteroids, plasmapheresis, immunodepressants, hyperbaric oxygenation, laser and magnetotherapy in treating the infectious-allergic Guillain-Barre polyradiculoneuritis. The indications and counter-indications to individual techniques are presented as related to the character and course of the disease. The principles of interrelation of these techniques with other drug and physical therapies are discussed. The authors infer that combination of plasmapheresis with corticosteroids is the best for acute polyradiculoneuritis and prolonged use of maintenance doses of corticosteroids and immunodepressants, physical methods and gymnastics are recommended for chronic polyradiculoneuritis.

[Treatment of trigeminal neuralgia, ganglioneuritis of the pterygopalatine ganglion and other types of prosopalgias by helium-neon laser irradiation of the pterygopalatine ganglion].

The authors describe a method of contact helium-neon laser irradiation of the pterygopalatine node through lateral wall of the nasal cavity. The technique was employed in patients with trigeminal neuralgia, ganglioneuritis of pterygopalatine node, other forms of prosopalgia. Altogether 78 patients were treated. The initial course relieved pain in 56 patients. In 44 patients the remission has been persisting for 2 years.

[Hyperbaric oxygenation in the combined treatment of patients with polyradiculoneuritis].

The results of clinico-electromyographic examinations point to a sufficiently high effectiveness of hyperbaric oxygenation in polyradiculoneuritis and permit its inclusion into the multimodality treatment of the latter. Hyperbaric oxygenation (HBO) accelerates regression of neurological disorders, with the predominant effect on the severity of motor and autonomic-trophic disturbances. HBO makes it possible to reduce the doses of glucocorticoids and the period of treatment and hospitalization of patients.

[Dynamics of the bioelectric activity of the brain in patients with primary hyperparathyroidism treated surgically].

The clinico-electroencephalographic examination over time of 23 patients with primary hyperparathyrosis using the automated assessment of the EEG findings showed that the disease was characterized by unmarked inconclusive changes in the cerebral rhythm, the degree of these changes showing no dependence on the level of blood calcium. Six to seven days after surgery for removing the adenoma of the parathyroid gland, the EEG disorders either increased or remained unaltered; they exhibited no correlation with the post-surgical normo- or hypocalcemia. Stable normalization of the clinico-neurological and EEG parameters was observed at a later post-surgical period, varying from 6 to 12 months.
