[Personality disorders: old and new problems].

Correlations were found between the structure of personality disorders (PD) (neurotic--borderline personality organization) and the regularities of PD dynamics. Taking into consideration the regularities of interaction of psychopathologic manifestations with positive symptomatology, three types of PD dynamics were psychopathologically identified, which show information on polar (compensation--decompensation) processes: 1) that in form of personality development; 2) that in form of PD-associated quasipsychoses; associated with DP; 3) that in form of transformation of characterological disorders into psychopathologic ones. The first two types of dynamics, which appear as as actualization of pathocharacterological formations in the pattern of mental disorders, reflect decompensation processes. In neurotic personality (emotionally labile, anxious, dependent, sensitive schizoids, etc) there is a deep (in terms of personality development) reorganization of characterological abnormalies. In borderline personality (paranoid, expansive schizoids, anancasts, borderline, etc.), aggravation and chronic pattern of mental disorders occurs with dynamics of PD-associated quasipsychoses. The third type of dynamics is not accompanied by aggravation of pathocharacterological manifestations and reflects compensation process. Separation of the isolated symptom complexes, PD derivatives (steady-state catathymically charged pathocharacterological formations are symptom-forming) is possible in any type of personality organization, but more frequently is observed at complex forms with a high proportion of facultative complexes and accentuations.

[A comparative evaluation of the biochemical and psychopathological characteristics in subjects with signs of pedophilia].

RIA was employed to examine 54 test subjects who committed sexual crimes with regard to children of tender age. A syndrome of sexual perversions in the form of pedophilia was discovered in 42 persons. Different patients' groups manifested significant differences in the content of hypophyseal and sexual hormones in the blood serum. A significant lowering of testosterone was revealed in persons with heterosexual pedophilia and with pedophilia without any aggressive and sadistic disorders. They also demonstrated a high prolactin content.

[The clinico-biochemical characteristics of persons with paraphilias and a chromosome anomaly (47, XYY)].

The authors describe the results of a comprehensive examination of 5 persons with chromosomal aberration (47, XYY) manifesting the signs of deviation from normal sexual behavior. The examined were shown to have common dysontogenesis that manifested itself by retardation of the biological basis of sexuality, psychosexual formation together with retardation of the mental processes on the whole. Whatever variants of deviations from normal sexual behavior the common feature was predominance in the libido structure of sadistic aims, that contributed to the revealing of aggressive sexual behavior. Material changes were discovered by radioimmunoassay in the function of the hypothalamohypophyseal and gonadal system in persons with the "YY syndrome". The levels of prolactin and luteinizing hormone along with the LH/FSH ratio appeared significantly elevated. Disorders were also identified in the hypothalamohypophyseal and thyroid system. It is assumed that the discovered changes influence the serotoninergic mechanisms by which aggressive and pathological sexual behavior is formed in such persons.

[The level of sex hormones in men with a sexual behavior disorder in the form of exhibitionism].

The paper is concerned with the results of radioimmunological studies reflecting the importance of the function of the hypothalamohypophyseal-gonadal system in 29 men with the signs of organic brain lesions, against whom criminal proceedings were instituted because of exhibitionism. Among the examinees the subgroups with the hypothalamic and temporal syndromes were distinguished. The data obtained were compared to those in 25 mentally normal men and 17 examinees with residual organic brain lesions without sexual behavior abnormalities. The data derived have demonstrated the impairment of the function of the indicated system in the test subjects, associated with sexual behavior impairment in the form of exhibitionism which may form the biological basis for distortion of sexual self-consciousness.

On social dangerousness of mental patients.

The relationship of the definition of social dangerousness and its application to mental patients is explored from a criminology perspective wherein social dangerousness is described as a universal concept used to assess damage done to society; and a socially dangerous individual is defined as having the potential to commit an offense. The risk of social dangerousness of mental patients is high and typically occurs during the first 4 years of the illness. It is dependent upon complex interactions between psychopathologic phenomena, environmental stimuli, and personality characteristics of the patient; and detection requires a general definition of the dangerous psychotic state and an elaboration of the methods for diagnosis.

[Classification of psychopathies].

The author proposes a new "ring-like" classification of psychopathies which includes the most definitely described prevalent clinical variants with due consideration of mutual transitions of some forms of psychopathy into others (mixed forms, "complex" psychopathic syndrome) and also an energy potential of individuals with some persons being referred to the sthenic (excitable) pole and others to the asthenic (inhibitory) pole. The main disorder obligate to all psychopathic personalities is mental infantilism whereas the inadequacy of emotional manifestations and alterations in the sphere of thinking are considered as secondary manifestations determining one or another clinical variant of psychopathy and are related to congenital mental activity of a given individual.

[Clinico-psychological features of mild forms of reactive states].

Using clinical and psychological methods, 56 patients with psychogenically induced non-psychotic affective states were examined. In relation to the clinical picture and the dominating affect, the following variants of the mild forms of reactive states were identified: anxiety-depressive, depressive-hypochondriac, astheno-depressive and pseudomanic. The specific features of the structure of the above syndromes are described. The utilization of the data obtained will contribute to the early diagnosis of psychogenic affective disturbances and also to preventing the development of serious psychogenic conditions.

[Reactive states among psychopathic personalities of different clinical groups].

On the basis of a clinical follow-up of 71 psychopaths with manifestations of reactive psychosis, the authors established the clinico-typological characteristics of both groups of pathology and the system of correlations between them. They also described the structure of the psychopathic and psychogenic syndromes, the type of psychopathic personality reaction and the form of psychogenic responses and proved statistically their close interrelationship. Data were obtained on some general regularities of interrelations of reactive psychoses with a psychopathic background which are important for the prognosis, prophylaxis and therapy of reactive states.

[Critical capabilities in psychopathics].

The authors studied the capabilities of psychopathic personalities (58 cases) to control their behaviour by involving such factors as the past experience, the existing knowledge selfestimation, the level of pretension. The results of such studies established insufficiency of controlling the activities in using the past experience, inadequate selfestimation and level of pretension, incapability to a prognosis of the aftereffects of personal actions. These disturbances may determine the disturbances of critical capabilities in psychopathy.
