Herpesvirus type 1-8 in sinus aspirates from HIV-infected patients and immunocompetent individuals.

Sinusitis is frequently occurring in HIV-infected patients, but in a substantial number of cases the etiology is unknown. The purpose of this study was by PCR 1) to determine the prevalence of the eight human herpesviruses in sinus aspirates from 24 HIV-positive/AIDS patients with sinusitis 2) to relate the presence of herpesvirus DNA to clinical and immunological parameters and 3) to compare the prevalence of herpesvirus DNA in sinus aspirates from HIV-infected patients with the prevalence observed in 50 immunocompetent patients with sinusitis. DNA from HSV-1, EBV, CMV and HHV-8 was detected in 8 (33%) of the sinus aspirates from HIV-infected patients. In the immunocompetent patients, one of the herpesviruses, HHV-6, was found in one sinus aspirate. These data indicate that herpesviruses are frequently found in sinus aspirates from HIV-infected patients with sinusitis, whereas they do not seem to be related to clinical signs of sinusitis in immunocompetent individuals. The cause of these discrepancies may be due to uncontrolled reactivation of herpesviruses, which is known to occur in immunocompromised individuals. It remains to be established whether the herpesviruses play a pathogenic role in the development of sinusitis in HIV-infected patients.

[Local treatment of non-suppurative otitis externa. A double-blind randomized study].

A comparison between a plain steroid lotion and a steroid lotion with an additional keratolytic and antiseptic was conducted in order to assess the value, if any, of additives in the treatment of dry external otitis. Sixty patients with dry external otitis were treated with either a plain steroid lotion or a steroid lotion with additives. Assessments were made prior to and after one week of treatment regarding erythema, oedema, secretion, itching, pain and spontaneously reported side-effects. No significant differences in efficacy were observed between the treatments. Statistically significant differences in favour of the plain steroid lotion were found with regard to reduction of erythema and occurrence of spontaneously reported side-effects. These findings invalidate the demands of additives to steroid lotions in the treatment of dry external otitis.

Perennial rhinitis treated with a new steroid: fluocortin butylester (FCB).

The effect of fluocortin butylester (FCB) in the topical treatment of perennial rhinitis was investigated in a double-blind study using a cross-over technique. The daily dose was 4 mg. Of the 30 patients who completed the trial, 26 had either allergy or nasal eosinophilia. 20 of the 30 patients preferred FCB to placebo. Moreover, there was a positive, but not statistically significant, therapeutic effect according to the patient score cards (0.1 less than p less than 0.2). In other words, the results are positive, but not definite. It is concluded that possibly the dose ought to be increased to obtain a more reliable effect.

Otitis media in childhood. Sociomedical aspects with special reference to day-care conditions.

To assess the effect of day-care conditions on the occurrence of middle ear infection, 681 children, aged 3-4 yr, were investigated. The investigation comprised an interview with one of the parents, a case history was taken, and a physical examination including tympanometry. At that time 76% of the children had been in day-care away from their homes and half of these were younger than 6 months old. The two groups of children, those in day-care and those looked after at home, were comparable with respect to sex, housing conditions, parents' social status and smoking habits. An increased occurrence of otitis media was found among children in day-care Measurements of middle ear pressure revealed a significantly larger number of children with flat curves (secretory otitis) and thus poorer hearing among those in day-care than those at home. Among the children in day-care there was also a significantly larger number with a history of adenoidectomy than among those looked after at home. These differences could not be explained by sex, housing conditions, social status or parents' smoking habits. It is concluded that the apparently greater occurrence of middle ear infection among children in day-care, compared to those looked after at home is due to an increased risk of contamination from children of their own age.

Grass pollen-induced hay fever treated with a new steroid fluocortin butylester (FCB).

The clinical effect and the side effects of a new steroid, Fluocortin butylester (FCB) in the topical treatment of hay fever was studied in a double-blind trial. The drug was administered by inhalation of a fine powder through a special inhalator (Rhinolator). The material comprised 31 patients, and the treatment period was three weeks. There was a highly significant difference in favour of FCB compared with placebo as regards the nasal symptoms blockage and discharge. Side effects were few and mild.

A population study of otitis media in childhood.

A total of 494 children with an average age of 3 1/2 years were studied with a view to elucidating the frequency of both acute and secretory otitis media and their relationship, if any, to a number of social factors. Of the children, 41% had had at least one attack of otitis media, in 49% of these associated with aural discharge. 70% of the children were attending day-care centres, and the incidence of otitis media was highest in this group. According to Jerger's classification of the tympanometric curves, 51% of these were of type A, 17% of type B, and 32% of type C. Significantly higher frequencies of types B and C curves were found among children attending day-care centres.

Nasal mucociliary function during penicillin treatment.

Nasal mucociliary function was studied in 21 healthy subjects before, during and after penicillin administration. No change in the mucociliary function occurred during penicillin treatment. Nor was any difference observed between men and women or between smokers and non-smokers. On the other hand, the measurements revealed a significantly smaller intra-individual than inter-individual variation. As the epithelia of nasal cavity and Eustachian tube are morphologically of the same type it is concluded that the increased incidence of serous otitis media which has been observed after the introduction of penicillin in the treatment of acute otitis media can scarcely be due to an inhibitory effect of penicillin on the mucociliary function of the Eustachian tube.
