[Rupture of the renal artery aneurysm in a patient with symptoms of a systemic connective tissue disorder--A case review].

The renal arteries aneurysms are relatively rare, however, their rupture has a high mortality rate. The authors present a case-review of a 38-year old female, hospitalized with a pain in the left lumbar region, lasting for 24 hours, which spread into the mesogastrium and the left groin. Upon angiographic examination, the left renal artery aneurysm was detected. Embolization was conducted, aiming to save the kidney, however, it proved unsuccessful and was complicated by a rupture of the aneurysm. Consequently, urgent nephrectomy was completed. Prior to the procedure, massive haemorrhaging from the right groin following angiography, was managed. Relapsing haemorrhaging from the right groin and development of a major retroperitoneal haematoma, complicated the postoperative course and required repetitive postoperative revisions. The accidental finding of a minor aneurysm of the lienal artery, the patient's constitution, her anamnestic data and the complicated course of hospitalization, suggested the systemic connective tissue disorder. Furthermore, the authors discuss contemporary diagnostic and therapeutic approach to the renal arteries aneurysms.

[Pedal bypass versus PTA (percutaneous transluminal angioplasty) of the crural arteries].

Diabetic foot syndrome in patients with atherosclerotic angiopathy of the crural arteries frequently develops in critical ischaemia and gangrene. Revascularization then may be the only approach to limb salvage. In the ongoing decade, pedal bypass grafting has become the method of choice for treatment of the most severe forms of chronic critical leg ischaemia caused by obliteration of the crural arteries. However, in patients in whom angiographic findings are favourable (short stenosis of the crural artery and the presence of suitable distal outflow arteries), percutaneous transluminal angioplasty (PTA), with stent placement, if indicated, is preferred. In multisegmental or diffuse disease or when PTA is associated with complications (occlusion or dissection) with subsequent limb ischaemia, bypass surgery is the method of treatment. In this study, the group of 30 patients undergoing bypass grafting showed 90% patency of the pedal bypasses at 12-month follow-up; in 18 patients treated by PTA, the patency of crural arteries at 6 months after angioplasty was 78%. Since either method has its advantage and disadvantages, an individual approach to each patient is recommended. In decision-making on the basis of angiographic findings, PTA is not confronted with pedal bypass grafting but the two methods are regarded as complementary in treatment of patients with crural artery disease.

[Diabetic peripheral arterial disease].

Macrovascular impairment of peripheral arteries in diabetes mellitus corresponds with such an impairment in atherosclerosis. Furthermore, peripheral angiopathy, microangiopathy, and polyneuropathy resulting from microangiopathy develop in diabetes. Impairment of microcirculation worsens peripheral blood flow. Infection and insufficient trophism worsen state. A wide range of factors participate in development of vascular disease in diabetes with adverse influence both on veins and reactions coming under rheology, haemocoagulation, and immune reactions. In type II diabetes an average interval of arterial impairment is 9-10 years, in type I diabetes approximately 17-22 years. A relative risk of non-traumatic amputation of a lower limb in diabetics is several times higher than in non-diabetics. A prevention of lower limbs impairment in diabetics is the most efficient method of reducing risk of amputation. An intervention therapy, endovascular intervention or angiosurgery are indicated in patients with clinical problems. A state of distal bed is an important factor for success of interventions. As a pivotal drug treatment in intervention and traditional methods is considered an effective antiaggregation treatment. A diabetic peripheral arterial disease is a complex disease requiring a wide view and considering various and changing pathophysiological mechanisms.

[The effect of clover silage treated with acrylic acid on selected indicators of metabolism in cows around the time of parturition and the status of their calves].

A trial was conducted with 16 cows divided into control and test group (eight cows in either); since day 57.6 +/- 21.1 before parturition, test cows were given feed rations for 107 days which contained 16.0 kg clover-grass silage a day, treated with 3 l acrylic acid per t ensiled matter. Control group of cows was administered feed rations containing silage treated with 4 l formic acid per 1 t ensiled matter from day 55.5 +/- 21.9. In the blood (plasma) of test cows the alkaline phosphatase activity was increasing gradually, in the time after parturition a tendency of lower glucose (P 0.05 to 0.01), thyroxine and triiodothyronine concentrations was observed, and the phagocytal activity of leucocytes and its index decreased. The levels of ketone bodies and aspartate aminotransferase activity were found to be higher. In this period in urine the pH value and net acid-base output were lower, while the inorganic phosphorus concentration was higher. No difference were observed in colostrum density, liveweight of born calves and in the target metabolic parameters in the blood (plasma) of calves. In the 30-day period of investigation the cows of test group gave an average daily milk yield lower by 1.5 l (in comparison with control cows). The residues of acrylic acid (method sensitivity of 1.0 mg acrylic acid in 1 l sample) were not detected in their colostrum and milk.

[Milk thistle (Silybum marianum, L., Gaertn.) in the feed of ketotic cows].

Two comparative trials were performed, each with 16 cows which in the period of 2-6 weeks after parturition had 7.9 mg and more acetone in 1 litre of milk. The cows, crossbreds of the Czech Red-Pied cattle with the Holstein cattle, were divided into control and test groups, eight in each using the system of pairs. The cows of test groups were given for a fortnight feed rations containing a meal of milk thistle (Silybum marianum, L., Gaert.) seeds, at a rate of 0.3 kg per head/day with the contents of 2.34% silybin and silydianin (substances of the so called silymarin complex of the flavonolignane group). In comparison with the control cows, in the blood and milk of the former ones a decrease was demonstrated in the sum of acetone + acetoacetic acid (up to P less than 0.01) and beta-hydroxybutyric acid in the blood (up to P less than 0.05). The ketonuria degree dropped remarkably. Although there were not observed any differences in the parameters of acid-base metabolism in the blood (pH, PCO2, BE, SB, BB), the pH values and net acid-base output in urine were higher in these cows. Milk production in the cows of control groups was decreasing during the trial (up to P 0.01), but in the test cows it was higher by 7.7% (trial 1) and by 3.4% (trial 2), in comparison with the milk yield at the beginning of the trials. Differences in metabolism parameters and milk production in favour of the cows which were given milk thistle in their feed rations were observed even in a fortnight after the diet stopped to contain this ingredient.

[The effect of dry standing time in cows with the obesity syndrome on selected indicators of metabolism and the state of their calves].

Differences in the composition of the blood (plasma) and urine of cows were evaluated in groups (n = 8) with a normal time of dry standing (50-60 days) and in cows standing dry for 80 to 124 days. Two approaches were used: repeated examination of the same animals two to three weeks before parturition and six to eight weeks after parturition, and single examination of the cows being in the same stage of reproduction cycle. A number of changes were recorded, testifying to a reduction of the function of the adrenal glands and to a decrease in the activity of the liver in the cows left to stand dry for a longer time. The calves of these cows had changes in their blood (plasma), indicating a hypofunction of the adrenal glands and a reduced phagocytic activity of the leucocytes, one of the parameters of the immunity system.

[Use of sodium acetate in feed rations in ketotic cows].

On a farm where the ration of cows contained 88.0 g of butyric acid, an experimental group of cows (n = 8), producing milk containing 7.9 mg or more acetone per litre, was studied for 14 days for the effect of a 250 g supplement of sodium acetate to the ration (combined with single i.m. administration of vitamins A, D2 and E) on selected metabolism parameters and on milk production. As distinct from the control group of cows (n = 8) from the same farm which produced milk containing 3.9 mg or less acetone per litre and which were fed without sodium acetone supplements, a tendency of increased alkaemia of the organism was suggested in the experimental cows. This tendency manifested itself during the trial in increased pH values, increased base excess (BE) and standard bicarbonate (SB) in the blood, and in an increase in the pH value and net acido basic secretion in urine. A decrease was recorded in the concentration of the acetone + acetacetic acid sum, the same as beta-hydroxybutyric acid in blood and the sum of acetone and acetacetic acid in milk (P less than 0.01). An insignificant increase of the activity of gammaglutamyl transpeptidase (GMT) was recorded in the blood serum of the experimental cows and a significant increase occurred in the content of potassium (up to P less than 0.01) and urea (up to P less than 0.01) in urine. The supplement of sodium acetate to the feed ration did not influence the degree of ketonuria and the finding of urobilinogen in urine.(ABSTRACT TRUNCATED AT 250 WORDS)

[The effect of feeding corn silage juices on selected indicators of metabolism in dairy cows given feed containing various proportions of silage and hay].

In farm production conditions two comparative trials lasting 58 and 128 days were carried out. Acidogenic exposure to acids of silage juices, silage and haylage (lactic acid, acetic and butyric acids) in feed ration ranged from 0.33 to 2.85 mol per 100 kg live weight. It was proved that it was possible to include 9 to 10 litres of silage juices in feed ration under the condition that the total daily intake would not exceed 1.94 mol of acids per 100 kg live weight. This supplement of silage juices increased milk production. Higher amount of acid (2.65 and 2.85 mol) resulted in acidogenic and ketogenic type of silage juice enriched feed ration, which caused a more rapid decrease of lactation curve in dairy cows fed this feed ration than in those in the control group.

[The effect of feeding silage juices on selected hematologic and biochemical parameters in the blood (serum) and urine of feedlot bulls].

The influence of silage juice feeding on the changes in 24 haematological and biochemical parameters in blood (blood serum) and urine of feeder bulls was evaluated in two comparative experiments in which no silage was included in feed rations. The supplement of 2 or 4 litres of juice per 100 kg of live weight represented a daily acidogenous stress of 0.79 to 1.93 moles of organic acids. In the first experiment this supplement was not proved to evoke any significant changes in the evaluated parameters. In the second experiment, in which the silage juices were enriched with sodium chloride, aqueous ammonia solution (26%) and molasses, a statistically significant increase in the level of beta-hydroxybutyric acid in blood and in the corresponding sum of ketone bodies (P less than 0.05 to 0.01) was observed in the experimental group of animals. The levels of sodium in serum were also increased (P less than 0.05). The average weight gains of bulls belonging to the experimental group were higher than those in the control groups.

[Development of aspartate aminotransferase, alanine and gamma-glutamyltransferase in the blood serum of dairy cows during the last month of pregnancy and the puerperium in relation to liver function].

Over the period of two years, the blood serum of dairy cows from three localities was examined repeatedly for the activities of aspartate aminotransferase (EC 2.6.1.1) (AST), alanine aminotransferase (EC 2.6.1.2) (ALT) and gammaglutamyltransferase (EC 2.3.2.1) (GMT). First examinations were performed two to four weeks before the expected term of delivery, the repeated examinations between the sixth and eighth week after delivery. The results obtained were used to calculate AST/ALT and GMT/ALT indexes in the studied group of animals (n = 12). As found out, the decrease in these indexes or the equality of values from the sixth to the eighth week after delivery, as compared with the period of two to four weeks prior to delivery, were connected with the clinically detectable disturbances of puerperium, i. e. inflammable discharge from the reproductive organs of dairy cows; this was observed even in the cases when the enzymatic activity was within the references standards.

[The effect of a light regimen on the dynamics of biochemical indicators in the blood, serum and urine of heifers during 1 day].

Circadian dynamics of 38 biochemical parameters of the blood, serum and urine of heifers was studied under the natural light conditions of a light (160-180 lux) and dark (10-40 lux) small cow house. In each house, eight parameters were subject to a significant influence during the day, six of them being the same in both houses: calcium, chlorides, urea and thyroxine of blood serum, glucose and haematocrit in the blood of heifers. The comparison of parameters in different groups showed that different light regimes exerted a significant influence on calcium, chlorides, total protein, cholesterol, vitamin A and thyroxine in serum, haematocrit and leucocyte count in blood, and sodium and urea in the urine of heifers. The effect of feeding regime on the dynamics of parameters was tested by the calculation of the percentual fluctuation of their average values in heifer blood and serum before feeding (samples taken at 6.30 hrs) and after feeding (sampled at 10 hrs). In the light house, six parameters were judged to be less stable and one unstable (urea in serum). In the dark house six parameters were less stable and four unstable (calcium, urea, copper and thyroxine in serum).

[The effect of the light schedule on biochemical indicators in the blood, serum and urine of heifers during their estrus cycle].

The dynamics of 38 biochemical parameters of blood, serum and urine was studied in ten heifers during their oestrous cycle in a light house (80-340 lux) and a dark small house (10-40 lux). In the light stable a significant influence (P less than 0.05) was found to be exerted on five parameters and in the dark stable nine parameters, four being influenced in the same way in both houses: haematocrit value and haemoglobin in blood and cholesterol and progesterone in serum; the relationship of progesterone content in serum to the days of oestrous cycle was particularly high in the light house. The comparison of the parameters between the groups demonstrated a significant (P less than 0.05) influence of light regime, exerted on some days of the oestrous cycle upon the levels of inorganic phosphorus, total protein and aspartate aminotransferase activity (AST) in serum and upon haematocrit, haemoglobin and ketone bodies in the blood of heifers. However, the light regime did not influence the levels of progesterone which were somewhat higher in the light house but with no statistically significant difference from the dark house. A significant difference between the groups was obtained in the levels of sodium and phosphorus in urine (P less than 0.05) between the first and tenth days of the oestrous cycle. No differences in the oestrous cycle between the heifers in the light and dark stables were recorded. Neither was the duration of heat influenced significantly; it was only less manifest in the dark stable.

[Lecithin and sphingomyelin in amniotic fluid in late pregnancy in dairy cows].

Phospholipides were isolated from 26 amniotic fluids of highly pregnant dairy cows and the lecithin : sphingomyelin ratio (the L/S index) was determined; this ratio ranged from 1.9 to 12.0 (mean = 6.5; n = 20), and in other cases only lecithin was clearly detected (n = 6). During the studied period of 249 to 277 days of pregnancy, no significant correlation was found, by the method used, between the age of the foetus and the L/S index in the amniotic fluid. The modifications of the methods in which the concentrations of different phospholipids are used for the determination of the maturity of foetal lungs, are discussed, together with the possibility of using and interpreting the L/S index in cattle.

[Changes in biochemical indicators in the blood and organs of pigs fed dried grape press cake].

In two experiments the effect of feeding dried crushed white and red grape press cake replacing 10--20% of the complex feed mixture A1 and SOL, was studied on the 21 biochemical indicators of blood serum, plasma, suprarenal glands, liver and tissue of fattened pigs. Changes indicating unsuitability of this non-traditional feed were not observed. During feeding red grape press cake, the young pigs of 35kg body weight had a lower concentration of glucose in blood serum, in comparison with the control. The temporary increase of calcium level and decrease of inorganic phosphorus in these animals was accompanied by a lower activity of alkaline phosphatase. White and red grape press cake affected positively the vitamin E level in blood serum. In the muscles of the experimental slaughter pigs protein proportion was increased and fat proportion was decreased.

[Carotene and fluorometrically determined vitamin A levels in the blood plasma of cows in relation to gravidity and type of feeding].

From January to June we determined the concentration of vitamin A and carotene in the blood plasma of 66 high-pregnant cows and cows 23 to 54 days after calving. Seasonal changes and a decrease in the content of both substances before delivery were proved. In field conditions of three farms it was determined that the haylage type of winter feeding was better than the silage type of winter feeding in view of provitamin A supply. The concentration of carotene in blood was always higher with haylage feeding as well as that of vitamin A during the period of high lactation. Regardless of that the level of vitamin A in dairy cows in April and May was lower by almost a quarter compared with feeding fresh green forage in June. The results reveal that the suitable time for assessing the sufficiency of vitamin A supply in cows is the time starting one month after the beginning of lactation. The usability and accuracy of the fluorometric method for determining vitamin A, while respecting the interference caused by phytofluen and beta-carotene, have been proved.

Follow-up and its importance for long-term pedal bypass functionality.

Pedal bypasses, for salvage of a critically ischaemic limb, demonstrate excellent long-term outcomes. However, regular follow-up of patients and timely detection and treatment of a contingent haemodynamically significant bypass stenosis, which could cause its thrombosis, are prerequisites. The authors report a case of femoropedal bypass graft in which regular duplex follow-up detected critical stenoses at 18 months and at 36 months. Patch-angioplasty (at 18 months) and percutaneous balloon angioplasty (at 36 months) preserved the long-term patency of this pedal bypass graft.
