Appropriate use of drugs by elderly people.

The effectiveness and safety of prescribed medication theoretically depends on the patient correctly following the prescriber's instructions. However, patient compliance is often difficult to achieve. This paper discusses reasons for this in relation to elderly people, and attempts to remedy the situation are reviewed.

A behavioral method for efficient screening of visual acuity in young infants. I. Preliminary laboratory development.

A technique for rapid behavioral screening of grating acuity in infants 1 to 4 months of age is described. The approach, called forced-choice preferential looking (FPL), depends upon the fact that normal infants will stare fixedly at acuity gratings with stripe widths above a rather abrupt cutoff width. The present task is to find the minimum stripe width, termed the diagnostic stripe width, to which infants with normal visual acuity will readily respond. The expectation is that infants with below-normal acuity will not be able to respond to diagnostic stripes so defined. To assess the feasibility of such a test procedure and define preliminary diagnostic stripe widths for infants 4, 8, 12, and 16 weeks of age, 76 presumptively normal infants were tested in the laboratory with a 40-trial FPL procedure. Sixty-nine infants (91%) completed the test procedure. For each age group, a preliminary estimate of the diagnostic stripe width was found. The group performance was uniformly high for the stripe widths designated as diagnostic and fell off sharply for finer stripes, confirming the feasibility of the approach.

Results of school screening for scoliosis in the San Juan Unified School District, Sacramento, California.

Annual routine school examination for scoliosis has been established in the San Juan Unified School District. Additionally, several parochial schools and other schools in the county or nearby towns have expressed interest in such a program. A rapid, effective method, taking no more than 30 seconds per child, has been used to detect spinal curvature. The program is beneficial for those identified with scoliosis, because early detection, followed by proper treatment, can prevent major surgery. The need for careful school nurse follow-up must be emphasized. A standing x-ray and evaluation by a qualified physician are imperative. If scoliosis is diagnosed, the school nurse can be a very effective contact in assisting the students by discussing exercise or brace care and by providing encouragement and general supportive help.

Depression in older people: a study.

A longitudinal study is being carried out to monitor the levels of care received by older people following implementation of the GP contract, the granting of fundholding and trust status, and the implementation of the community care act. The study also aims to monitor the pattern of consumer satisfaction with different services and the input of practice nurses with regard to health assessment of older people. This article focuses on depression among older people and draws on data obtained in the first of three annual rounds of interviews with a cohort of 775 people over the age of 75 in areas covered by four first-wave community trusts. Thirty per cent of those interviewed were found to have symptoms indicating a high probability of depression. Those who appeared depressed were less likely to have had a health assessment by someone from general practice. They also had more physical symptoms and had consumed more drugs in the 24 hours before they were interviewed. Responses to open ended questions were also qualitatively different for those who appeared depressed on interview.
