Increased arabinitol levels in experimental candidiasis in rats: arabinitol appearance rates, arabinitol/creatinine ratios, and severity of infection.

Renal colony counts, total arabinitol appearance, and the arabinitol/creatinine ratios in the serum and urine of rats with lethal candidiasis were measured. Total arabinitol appearance and the arabinitol/creatinine ratios increased significantly after infection. The serum arabinitol/creatinine ratio was high (2 SD above the mean value in normal animals) in five of six infected rats at 24 hr, in eight of eight at 48 hr, and in none of the control animals. The urinary arabinitol/creatinine ratio was high in 14 of 19 infected rats by 24 hr, in 11 of 11 by 48 hr, and in none of the control animals. Total arabinitol appearance and the arabinitol/creatinine ratios were highly correlated with renal colony counts. Arabinitol production in vitro was sufficient to account for the increased arabinitol appearance observed in vivo in 12 of 14 rats. Thus it is concluded that the excess arabinitol that appeared in vivo was produced by the infecting fungus and that arabinitol is a quantitative marker substance for candidiasis.

The arabinitol appearance rate in laboratory animals and humans: estimation from the arabinitol/creatine ratio and relevance to the diagnosis of candidiasis.

The effects of renal function on serum concentrations of the candidal metabolite arabinitol were studied by examining the accumulation and elimination of arabinitol in animals and humans. Serum concentrations of arabinitol rose sharply and in direct proportion to creatine concentrations after nephrectomy in rats. The serum half-life of exogenous arabinitol was 20.8 hr in anephric rats but only 0.62 hr in sham-nephrectomized control animals. The mean +/- SD apparent space of distribution of arabinitol was 419 +/- 26 ml/kg. The fraction of exogenously administered arabinitol recovered in the urine was 0.73 +/- 0.13 in uremic rats, 0.85 +/- 0.28 in control rats, and 0.95 +/- 0.10 in normal dogs. The arabinitol/creatinine clearance ratio was 0.99 +/- 0.08 in normal dogs and 0.99 +/- 0.25 in 22 critically ill patients with cancer. Thus arabinitol is eliminated by nearly quantitative urinary excretion and its cleared at virtually the same rate as creatinine. Therefore, the rate of arabinitol appearance in the body from any source equals the urinary arabinitol excretion rate and is directly proportional to the concentration ratio of arabinitol to creatinine in serum or urine.

Examination of Pseudomonas aeruginosa-gentamicin discrepancies encountered in an Autobac I-disk diffusion comparison.

Seven Pseudomonas aeruginosa strains found to be susceptible to gentamicin by the Autobac I system and resistant by the Bauer-Kirby disk diffusion method were tested for the presence of mixed populations of cells. Double zones of inhibition randomly appeared on gentamicin disk diffusion plates, and susceptible and resistant subpopulations were isolated from these plates. Growth kinetic studies of separated strains and mixed subpopulations indicated that the susceptible organisms grew rapidly and were read as susceptible at 5 h with the Autobac I system. Resistant organisms entered a sustained lag phase and did not achieve sufficient turbidity to be read as resistant with the Autobac I system before 6 h. Thus, a false reading of susceptible could be obtained with the Autobac I system, depending on the ratio of susceptible organisms to resistant organisms selected for testing. A mixed susceptible and resistant population could be recognized by extending the incubation time of the Autobac I cuvette or by using other susceptibility methods to test isolates with light-scattering indexes of less than 1.0.

Bone infection caused by debaryomyces hansenii in a normal host: a case report.

Debaryomyces hansenii was isolated from a cystic lesion in the distal tibia of a healthy 23-year-old woman. Ascospores were demonstrated when the organism was incubated at 25 but not at 30 degrees C. Electron microscopy was necessary to demonstrate the warty surface of the ascospore wall. Growth was inhibited in vitro by low concentrations of amphotericin B, 5-fluorocytosine, miconazole, and ketoconazole. Four previous cases of infection caused by Debaryomyces spp. and one caused by the related fungus Torulopsis candida were reviewed.

Personality attributes of males and females marrying within vs. across racial/ethnic groups.

As part of a larger study, Adjective Check List scores were obtained from 543 couples in Hawaii who had married within their own ethnic group and 83 who had not. Females who married across ethnic groups generally scored higher in one domain of personality (dominance) than did females who married within their own ethnic group. Males who married cross-ethnically also differed in dominance from those who did not, but in different directions in different ethnic groups. In the group that had the highest status (Caucasian) in the decades in which these subjects married, they were less dominant; in the other groups, to varying degrees, they were more dominant. In general, males and females in cross-ethnic matings were more similar in personality test scores than males and females who married within their own ethnic groups. Spouse correlations were generally positive but trivial in magnitude, with very little difference in personality between homogamous and heterogamous couples. The average difference in personality between those who married within and outside their own ethnic group was associated with the rarity of outmarriage for the sex and ethnic group concerned.

Synthesis and evaluation of the male antifertility properties of a series of N-unsubstituted sulfamates.

A series of six aliphatic and one carbocyclic N-unsubstituted sulfamates have been synthesized and evaluated as potential male antifertility agents. Three of the aliphatic sulfamates, 1,2-ethanediyl sulfamate (1), 1,3-propanediyl sulfamate (2), and 1,4-butanediyl sulfamate (3), when administered orally to male rats caused a decrease in the number of pregnant females and/or implantation coupled with increased embryonic and fetal resorption. The compounds were prepared by treating the appropriate glycol salt with sulfamoyl chloride or by the cleavage of a tert-butylsulfamate with trifluoroacetic acid.

Long term therapeutic effects of a three month intensive growth group.

personality tests were administered to 33 participants in a 3-month intensive resident human growth program, at the beginning and end of the program and again 1-year later. A control group of 13 members of the families of some of the participants also took the tests on the same dates. The control group, showed no statistically significant changes on nearly all measured personality traits for the growth group participants were evident on post-testing and also 1 year later.

Urinary tritium excretion following intravenous 3H-norepinephrine administration in hypertensive rats.

Spontaneously hypertensive rats (SHR), salt-resistant (SRR), salt-sensitive (SSR), and Sprague-Dawley DOC-salt nephrectomized rats were tested by evaluation of tritiated norepinephrine uptake and blood pressure with diets containing various amounts of sodium. Only Sprague-Dawley rats who had been both nephrectomized and given DOC on a high salt diet (8%) illustrated the elevated tritium excretion similar to that observed in human subjects with essential hypertension.

Rickettsialpox. Case report and epidemiologic review.

In recent years, rickettsialpox was infrequently reported: one case per year was recorded by the Public Health Service during the past 15 years. We observed a case of rickettsialpox in New York City and reviewed the clinical, diagnostic, and epidemiologic features. Records of the New York City Health Department showed that serum samples from only six patients were submitted for serological testing for rickettsialpox during the past ten years, suggesting that although the disease is uncommon now, clinicians may be failing to recognize and diagnose rickettsialpox, as it may occur more frequently than is presently recognized.

The risk of pericardiocentesis.

The risk and potential risk factors of pericardiocentesis were assessed by a review of a series of 52 pericardiocenteses comprising all those performed in the cardiac catheterization laboratory of one institution from 1971 to 1978. On the basis of the operative results, the patients were separated into two groups for comparison; Group I comprised all patients with a successful uncomplicated (35) pericardiocentesis and Group II all those with a nonproductive (16), nontherapeutic (1) or complicated (8) pericardiocentesis. Complications consisted of one death, one cardiac arrest, one aspiration of a subdiaphragmatic abscess and five ventricular punctures without adverse sequelae. Among the patients who had a nonproductive pericardiocentesis, the condition of 11 had probably been misdiagnosed but at least 4 had a false negative pericardiocentesis. Comparison of the two groups showed no significant difference in the incidence of cardiac tamponade or in the clinical presentation based on historical, physical, electrocardiographic, roentgenographic or echocardiographic findings of pericardial disease. Pericardiocentesis was usually successful when performed for suspected malignant pericardial effusion but often unsuccessful when performed for suspected hemopericardium. Anatomically, all patients in Group II had either minimal or loculated posterior pericardial effusion. It is concluded that pericardiocentesis can be performed at a low risk that can be further minimized by consideration of the disease process and the anatomic location of the pericardial fluid.

Effect of intermittent left bundle branch block on left ventricular performance in the normal heart.

The effect of left bundle branch block on left ventricular performance was studied in a patient with rate-dependent left bundle branch block and an otherwise normal heart. The timing of left heart events, hemodynamic pressures and indexes of left ventricular contractility were determined during normal intraventricular conduction and during left bundle branch block. The results indicate that in the normal heart left bundle branch block has no significant effect on left ventricular performance.

Contragestational agents. 1. Steroidal O-aryloximes.

The preparation of a series of O-aryloximes of various steroids by two different routes is described. These compounds were prepared by reacting a keto steroid with a substituted O-arylhydroxylamine in the presence of an acid catalyst or, alternatively, by the reaction of a steroidal oxime with a substituted aryl halide in the presence of a suitable base. These compounds were examined for their ability to interrupt postimplantive gestation in female rats. The most significant contragestational activity was seen with compounds in which the basic steroid structure was a 5alpha-androstane and the 3-oxime was of the p-nitrophenyl series. One of the most active compounds in the series (16) was shown to have the ability to terminate pregnancy, when orally administered to rats at 2.5 mg/kg on days 9-12 of gestation. This compound was found to be devoid of androgenic activity at this dose level.

Retrograde catheterization of the left atrium in idiopathic hypertrophic subaortic stenosis.

Retrograde catheterization of the left atrium utilizing the Shirley technique was successfully performed in four patients with idiopathic hypertrophic subaortic stenosis. By traversing the inflow tract of the left ventricle on pullback from the left atrium to the aorta, this method served to separate definitively true hypertrophic subaortic stenosis from cavity obliteration. It was accomplished with relative ease and no complications. In patients with suspected idiopathic hypertrophic subaortic stenosis who are to have retrograde left heart catheterization, this approach to the left atrium is recommended as an alternative to the transseptal technique.

Idioventricular tachycardia with angina pectoris.

Idioventricular tachycardia (IVT) occurred in a patient with angina pectoris on three separate occasions associated with stress induced angina. It occurred immediately after a Master's test, a treadmill exercise test, and following an episode of paroxysmal atrial tachycardia which was precipitated by endocardial pacing. His bundle electrograms demonstrated that the rhythm was ventricular in origin. Cardiac catheterization documented the presence of severe coronary artery disease. Idioventricular tachycardia is often seen with acute myocardial infarction. It is rarely seen with angina or with exercise testing. To our knowledge this is the first reported case of IVT which could be repeatedly produced by stress induced angina.
