Viscocanalostomy for primary open-angle glaucoma: the Gross Pankow experience.

PURPOSE: To assess the pressure-lowering effect and postoperative complications of the viscocanalostomy nonpenetrating filtering procedure. SETTING: A private practice ophthalmic surgery referral center. METHODS: Fifty-six eyes of 41 patients with medically uncontrolled primary open-angle glaucoma had a viscocanalostomy. After a superficial scleral flap was raised, a deep sclerectomy was performed in the scleral bed with deroofing of Schlemm's canal and preparation of a window of Descemet's membrane. The ostia of Schlemm's canal were probed and stretched with sodium hyaluronate 1.4% (Healon GV(R)), and the scleral flap and conjunctiva were sutured. Examinations were performed before surgery and 1, 3, and 14 days and 1, 3, 6, and 12 months postoperatively. RESULTS: Mean preoperative intraocular pressure (IOP) was 28.1 mm Hg +/- 7.4 (SD) with a mean of 2.4 +/- 0.7 medications. Mean postoperative IOP was 18.6 +/- 7.5 mm Hg with 0 medications at 1 day, 17.4 +/- 5.2 mm Hg with 0.1 medications at 3 days, 19.1 +/- 4. 3 mm Hg with 0.1 medications at 14 days, 19.4 +/- 4.3 mm Hg with 0.4 medications at 1 month, 18.3 +/- 3.6 mm Hg with 0.6 medications at 3 months, 18.0 +/- 2.6 mm Hg with 0.6 medications at 6 months, and 17. 8 +/- 3.8 mm Hg with 0.7 medications at 1 year. After 1 year, IOP was lower than 21 mm Hg without medication in 36% of patients and lower than 21 mm Hg with medication in 79%. Five patients (9%) required a second operation for pressure control. The following postoperative complications occurred: hyphema (2%); postoperative hypotony less than 10 mm Hg (2%); positive Seidel test (17%); further surgery to lower IOP (12%). Cataract surgery was performed in 1 patient (2%) at 4 months. Despite an attempt to close the scleral flap watertight, 26 patients had evidence of subconjunctival drainage (conjunctival microcysts or filtration bleb) at 1 year. CONCLUSION: Viscocanalostomy lowered IOP and reduced the need for pressure-controlling medications with a low postoperative complication rate. The high success rates of earlier publications were not reproduced.

[Chamber angle findings following implantation of "chamber angle" supported lenses of the Dubroff and Kelman-Multiflex type].

The authors examined 92 eyes after planned ICCE combined with implantation of anterior chamber lenses on average 16.5 months after surgery (range: 6 to 37 months). Two types of lenses were used, namely Kelman-Multiflex and Dubroff. The tissue reactions at the points of contact of the lens haptics in the chamber angle were classified gonioscopically. Even though it was found that reactive tissue reached the filtering trabecular meshwork in some cases (2% of haptic points of Kelman-Multiflex lenses and 15% of those of Dubroff lenses), no significant increase in intraocular pressure was observed. Comparing advantages and disadvantages as regards facility of implantation as well as the probability of hazardous tissue reactions, neither type can be said to be clearly preferable.

[Iris suture in cataract surgery (author's transl)].

Cataract extraction can cause a diffuse traumatic dilatation of a narrow pupil in patients with long-standing application of miotics or after iridocyclitis. In order to avoid a total iridectomy a technique for performing an iridotomy with a single iris suture is described. We present the long-term observation of twenty cases: a pear-shaped pupil in a fairly centric position was obtained in all cases and an iris diaphragm was present. The nylon suture was tight in all cases and a bridge of cicatricial iris tissue present.

[Retinal detachment with spontaneous reattachment (author's transl)].

A report on eight patients with retinal detachment in whom spontaneous reattachment occurred. The reattachment process was slow and was accompanied by massive subretinal pigmentations. Those patients who were examined had complete loss of function in the area of the visual field affected by the reattachment.

[Retinal detachment limited by spontaneously developing pigment bands (author's transl)].

A report on twelve patients seen during a period of ten years, all of whom presented with retinal detachment limited by spontaneously developing "barriers" consisting of heavily pigmented or depigmented bands. The strength of these bands is discussed. On the basis of clinical experience they can only be considered stable in rare cases; the detachment usually progresses beyond them in time.

[The unilateral emigration of leukocytes from conjunctival vessels (author's transl)].

The phenomenon of unilateral leukodiapedesis is demonstrated in the irritated conjunctiva of rabbits and rhesus-monkeys. The leukocytes will penetrate the vessel walls first in the neighbourhood of the irritation. This can be demonstrated during the first minutes after the onset of an inflammatory reaction only, i.e. prior to a general leukocytosis of the tissue. By fixation with glutaraldehyde or osmium-tetroxyde the dynamic movement can be visualized and the directions of the irritation and of the migration are obvious in the conjunctiva. So this tissue provides an excellent model to demonstrate the unilateral sticking and the directed migration of leukocytes through vessel walls, a fact to which references seem to be missing in the ophthalmopathologic and pathologic literature.

[Macular pucker after retinal detachment surgery (author's transl)].

A report is given on 6 cases of macular pucker after retinal detachment surgery. Two of these 6 patients were found only by a check up of 145 detachment patients one year after surgery. In accordance with the literature presumbly causative factors were present in 5 of the cases: vitreous hemorrhage (4), reoperations (3) vitreous loss (1). A 6Th case with very unusual centrally located retinal breaks was operated on with a silicone explant and was seen with a macular pucker 6 months later.
