Therapeutic efficacy and safety of chaperonin 10 in patients with rheumatoid arthritis: a double-blind randomised trial.

BACKGROUND: Chaperonin 10 (heat shock protein 10, XToll) has anti-inflammatory properties related to the inhibition of Toll-like receptor signalling pathways. Our aim was to establish whether chaperonin 10 is safe and effective in the treatment of rheumatoid arthritis. METHODS: In this randomised, double-blind, multicentre study, 23 patients with moderate to severe active rheumatoid arthritis receiving disease-modifying antirheumatic drugs were randomly allocated to three treatment groups receiving intravenous chaperonin 10 twice weekly for 12 weeks at doses of 5 mg (n=8), 7.5 mg (8), or 10 mg (7). The primary outcomes were change in disease activity score (DAS28) and improvement of core disease measures (American College of Rheumatology response score) from baseline to week 12. All analyses were done by intention to treat. This study is registered with the Australian Clinical Trials Registry, number ACTRNO12606000041550. FINDINGS: Primary endpoint measures improved from day 14 in all groups and continued to improve to day 84. By end of study, a 20% improvement of core disease measures was seen in six (86%, 95% CI 43-100), a 50% improvement in four (57%, 14-86), and a 70% improvement in two (29%, 0-57) patients given the highest dose of chaperonin 10. Clinical remission (as defined by a DAS28 <2.6) was achieved in three (13%) of 23 patients. Three individuals dropped out during the study: one in the 5 mg group (rheumatoid arthritis not controlled), one in the 7.5 mg group (adverse event), and one in the 10 mg group (lost to follow-up). The most common adverse events were exacerbation of rheumatoid arthritis (both during and after the study) and upper respiratory tract infection. Only one adverse event was judged to be of severe intensity. INTERPRETATION: Chaperonin 10 seems to be well tolerated and efficacious in treatment of the symptoms of rheumatoid arthritis, at least in the short term.

Introducing clinical supervision for community-based nurses.

City and Hackney Teaching Primary Care Trust has introduced mandatory clinical supervision for all front-line nursing staff (district nurses, health visitors, school nurses, staff nurses, health care assistants). A group model is used, with sessions occurring monthly and lasting 90 minutes. Groups include a mixture of qualified and unqualified staff. The project was introduced in two phases. First, external facilitators were used to run sessions for 18 months. An evaluation toward the end of this period found very strong staff support for the innovation. Internal facilitators were then trained to implement the second phase. A second evaluation found continuing support for clinical supervision, and a preference for multidisciplinary groups. Clinical supervision is now firmly embedded in the organization. It is primarily 'restorative', offering staff relief from the stresses of working in a very deprived and ethnically diverse area where the recruitment and retention of staff is an enduring challenge.

Non-coordinate expression of peroxisome biogenesis, beta-oxidation and glyoxylate cycle genes in mature Arabidopsis plants.

The expression of three genes that encode proteins involved in peroxisome biogenesis, beta-oxidation and the glyoxylate cycle was studied in Arabidopsis plants by fusing their promoter regions to the reporter gene luciferase. Malate synthase showed an extremely restricted pattern of expression, being detected only in young seedlings and the root tips of older plants. PEX1 and 3-ketoacyl thiolase (PED1) were expressed in roots, mature leaves, stems and flowers. However, only thiolase was up-regulated by starvation. Immunoblotting confirmed that neither malate synthase nor the other unique glyoxylate cycle enzyme isocitrate lyase are expressed in senescent leaves. These results indicate that, in contrast to cucumber, pumpkin and barley, the glyoxylate cycle does not play a role in the recycling of carbon from the turnover of membrane lipids during senescence and starvation in Arabidopsis.

V-blender segregation patterns for free-flowing materials: effects of blender capacity and fill level.

Stable segregation patterns are shown to form in V-blenders over a wide range of vessel capacities, fill levels, and rotation rates. Slight changes in either rotation rate or fill level induce changes in pattern formation. Trajectory segregation in two regions of the flow, accumulating over many flow periods, drives segregation pattern formation. Scaling criteria derived to relate particle velocities to vessel size and rotation rate in rotating cylinders successfully predict the rotation rate for the transition between patterns across V-blenders of 0.8-26.5 quart total capacity. This agreement suggests that pattern formation is governed by the magnitude of particle velocities. Regardless of vessel size, when particle velocities at specific regions of the blender are below a certain value, one particular pattern appears, and when they increase beyond that speed (i.e. by changing the rotation rate or the vessel size), a different pattern emerges. A scaling relation between segregation pattern formation and blender fill level was not identified because the complex flow patterns in the V-blender (the length of the flowing layer and the mixture center of mass relative to the blender are constantly oscillating) preclude the determination of a relationship between blender fill level and particle velocities.

Collecting sensitive data: the impact on researchers.

The authors undertook an exploratory study to identify issues that arose for researchers while conducting fieldwork. Ten researchers engaged in "sensitive" inquiry were purposefully sampled and encouraged to talk about their experiences during the data collection process. Interviews were transcribed and common themes identified. The findings indicate that these researchers experienced a number of difficulties that centered around issues of lack of training, confidentiality, role conflict, costs to the participants, the desire for reciprocity, and feelings of isolation. The authors discuss the implications of the findings, including the need for support for researchers for further research.

Legal issues facing physicians: substance abuse in the workplace.

This chapter addresses key Department of Transportation (DOT) rules for drug testing employees working in industries regulated by the DOT. It also discusses the issue of substance abuse and the Americans with Disabilities Act. Finally, it reviews the iss e of confidentiality. It is important for the reader to recognize that this chapter is but a review of critical and crucial material. The reader is encouraged to pursue additional information if greater clarity and understanding are required.
