Aggressive fibromatosis in the urological system. Report of two adult patients and review of the literature.

Aggressive fibromatoses (AF) are locally aggressive neoplasms that do not metastasize but are frequently associated with one or more recurrences and subsequent associated morbidity. AF in the urological system is quite rare and has mainly been described in single case reports or as isolated cases in a large series of extra-abdominal desmoid tumors. We report 2 cases of AF in the bladder and scrotum, and provide a review of similar published cases.

Change of sexual function in patients before and after Ho:YAG laser enucleation of the prostate.

The aim of this study is to evaluate the effect of Ho:YAG laser enucleation of the prostate (HOLEP) to the sexual function of patients with benign prostatic hyperplasia (BPH). In the course of the study, 108 patients with BPH were recruited and accepted treatment with HOLEP. The effectiveness of treatment was evaluated by flow rate and the International Prostate Symptom Score (IPSS) before HOLEP and 6 months afterwards. Meanwhile, the sexual functions were evaluated with the Danish Prostate Symptom Score Sexual Function Questionnaire (DanPSS Sex). Before and 6 months after HOLEP treatment, the mean residual urine volume was reduced from 106.0 +/- 51.7 mL to 5.6 +/- 1.7 mL (P<.01), maximum flow rate was improved from 7.2 +/- 3.9 mL/s to 21.7 +/- 1.3 mL/s (P<.01), nocturia frequency was reduced from 5.5 to 1.5 (P<.01), and the mean IPSS score was decreased from 19.4 +/- 5.6 to 7.4 +/- 2.6 (P<.01). The proportion of patients satisfied with their libido was 55% before HOLEP and 57% 6 months afterwards, while 23.5% of the patients had no libido before and after HOLEP; 37% of the patients were satisfied with their erection before HOLEP and 40% after 6 months; 30% of the patients had completely satisfactory sex life before HOLEP, and 32% did 6 months later. The corresponding percentages of fully impotent patients increased from 33% before the procedure to 35% 6 months postoperation. Early morning erections were reported by 45% of the patients before the procedure and by 62% 6 months later (P<.01). In 70% of the patients with normal sex life, ejaculation was retrograde 6 months after HOLEP (P<.01). HOLEP does not affect the sexual function of patients with BPH but does did improve the ability of early morning erection, while causing retrograde ejaculation.

Urodynamic study in the neurogenic bladder dysfunction caused by intervertebral disk hernia.

AIMS: To discuss the clinical significance of urodynamic studies in neurogenic bladder dysfunction caused by intervertebral disk hernia. METHODS: Thirty patients with neurogenic bladder dysfunction caused by intervertebral disk hernia were divided into three groups according to the category of detrusor muscle activity: neurogenic detrusor overactivity without detrusor sphincter dyssynergia (DSD), neurogenic detrusor overactivity with DSD, or detrusor underactivity. The patients were divided into three groups according to the condition of urinary retention and incontinence: urinary retention, urinary incontinence, or neither urinary retention nor urinary incontinence. Also they were divided into three groups according to the location of the intervertebral disk hernia: cervical hernia, thoracic hernia, or lumbar hernia. All patients underwent computer tomography (CT), magnetic resonance imaging (MRI), and urodynamic study. The results were analyzed statistically. RESULTS: There was significant correlation between the category of detrusor muscle activity, bladder compliance (BC, P < 0.05), and the pressure at maximum flow (P(det,Qmax), P < 0.01). There were significant differences in post void residual (PVR), cystometric capacity (CC), and maximum flow rate (Qmax) between the three groups of urinary retention, urinary incontinence, and the group of neither retention nor incontinence (P < 0.05). There was statistical correlation between the location of the intervertebral disk hernia and the urodynamic results. There was significant correlation between the BC (the threshold was 15 ml/cmH(2)O), detrusor pressure (Pdet, the threshold was 40 cmH(2)O), DSD, the location of intervertebral disk hernia, and the upper urinary tract lesion (P < 0.01). CONCLUSIONS: Urodynamic studies are indispensable in the classification, diagnosis, treatment, and prognosis of the neurogenic bladder dysfunction caused by intervertebral disk hernia.

Development and validation of pheochromocytoma of the adrenal gland scaled score for predicting malignant pheochromocytomas.

OBJECTIVES: To evaluate the diagnostic performances of the pheochromocytoma of the adrenal gland scaled score (PASS) proposed in a previous report and that of a logistic model developed in this investigation. METHODS: In all 130 patients with malignant or assumed benign pheochromocytomas, 15 predictive variables were observed. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic performance of PASS. The logistic model was developed using the 15 predictive variables. Its performance was evaluated by calculating the area under the ROC curve and comparing it with that of the PASS. RESULTS: The PASS had the area under the ROC curve of 0.899 (95% confidence interval 0.844 to 0.954). Of the 15 variables entered in the logistic regression analysis, 9 were retained in the model. The area under the ROC curve for the logistic model was 0.983 (95% confidence interval 0.967 to 0.998). CONCLUSIONS: ROC analysis indicated that the PASS could be used for the diagnosis of malignant pheochromocytomas. The logistic model was able to improve the diagnostic performance of the PASS using a different variable weighting method. We emphasize, however, that a clinical prospective evaluation is needed to confirm their actual value.
