[Clinical analysis of 11 cases of ovarian Setoli-Leydig cell tumor].

OBJECTIVE: To study the clinical characteristics, treatment and prognostic factors of ovarian Setoli-Leydig cell tumor. METHODS: During 1962 - 2002, a total of 11 patients with Setoli-Leydig cell tumor were retrospectively analyzed. RESULTS: Microscopically, seven of the neoplasms were well differentiated, 3 were moderately differentiated and 1 was poorly differentiated. Nine of the tumors were stage Ia, 1 was stage IIc and 1 was stage IIIc. The most frequent symptoms were abdominal-pelvic masses. Six patients presented with androgenization and virilization, 3 of which had their serum testosterone tested, and the levels were elevated. Five patients presented with metromenorrhagia and abnormal vaginal bleeding. One of them was both androgenized and estrogenized. In addition, five patients had diseases associated with excessive estrogenic stimulation, such as uterine myoma and endometrial hyperplasia. Two patients suffered from breast cancer. All patients were subjected to operation. And 5 patients with poorly differentiated or stage II-III tumors were subjected to postoperational chemotherapy. After 6 months to 34 years follow-up, no patient died of this disease. Three patients who received conservative surgery achieved normal menstruation 1 - 3 months after operation, and one of them gave birth to a child. CONCLUSIONS: Ovarian Setoli-Leydig cell tumor has good prognosis. Surgery alone is a currently acceptable treatment for patients with well-differentiated early stage tumors. For patients with poorly differentiated or advanced tumors, postoperational chemotherapy seems to be necessary. Conservative surgery should be the treatment of choice in young patients who need future fertility.

Sentinel lymph node identification in patients with early stage cervical cancer undergoing radical hysterectomy and pelvic lymphadenectomy.

BACKGROUND: In general, sentinel lymph node (SLN) can reflect the whole state of the entire drainage area. The present study evaluated the clinical significance of sentinel lymph node identification in the treatment of early stage cervical cancer. METHODS: Twenty-eight patients with early stage (Ia-IIa) cervical cancer undergoing radical hysterectomy and pelvic lymphadenectomy were included in this study. At two locations 8 hours before operation, 0.4 ml 37 Mbq technicium-99 labeled dextran was injected intracervically. After that, preoperative lymphoscintigraphy was performed to detect SLNs. During the operation, lymph nodes were detected ex vivo by a gamma-counter to identify SLNs. The samples of SLNs and non-SLNs were used for pathological examination separately and compared with the final results. RESULTS: SLNs were identified in 27 of 28 (96.4%) patients. A total of 123 SLNs were identified from 814 nodes. Six patients had altogether 11 positive lymph nodes, which were all SLNs. No patient had false-negative sentinel node. CONCLUSION: SLNs can successfully predict the lymphatic metastasis in patients with cervical cancer. The clinical validity of this technique should be evaluated prospectively.

[Clinical and pathological features of 273 cases of lacrimal epithelial tumors].

OBJECTIVE: A retrospective study of histopathological and clinical aspects of 261 cases of lacrimal tumors (273 paraffin specimens) from Ophthalmic Pathology Laboratory, Beijing Tongren Hospital (Dec. 1961 to Jun. 2002) was performed to investigate the pathological classification and clinical features of these tumors. METHODS: Clinical features were analyzed from 261 patients of lacrimal tumors, including patient's history, age, gender, laterality, visual acuity, ultrasound B scan, X-ray, CT, MRI, pre- and post-operation condition, follow-up data and others. These data were compared with the pathological features. RESULTS: Pleomorphic adenoma was the most common tumor in epithelial tumors of lacrimal gland, followed by adenoid cystic carcinoma, pleomorphic adenocarcinoma and adenocarcinoma. Recurrence was observed in 12 cases. The recurrence rate is 4.6%. Four cases was dead, the mortality was 1.5%. CONCLUSION: Pleomorphic adenoma shows a low mortality and a relatively high recurrence rate. The main treatment for pleomorphic adenoma is surgical treatment. It is important to have a correct clinical diagnosis, to select the proper surgical method, drug treatment and postoperative management based on the pathological diagnosis, which can result in a decrease of recurrence rate.

[The study on the correlation between telomerase and histopathologic features of retinoblastoma].

OBJECTIVE: To investigate the correlation between the expression of human telomerase RNA (hTR), human telomerase reverse transcriptase (hTRT) and the proliferation and differentiation of retinoblastoma (RB). METHODS: In 37 RB specimens, the expression of hTR and hTRT was investigated with the methods of in situ hybridization (ISH). Immunohistochemical method was performed to detect the expression of proliferating cell nuclear antigen (PCNA) in RB. All data were analyzed statistically with SPSS. RESULTS: Of 37 RB specimens, positive expression of hTR and hTRT was 83.8% and 89.2%, respectively, while negative results were observed in 2 normal retina tissues. The expressions of hTR and hTRT showed a close correlation to the proliferation and differentiation of RB. Furthermore, there was a close correlation among the expression of hTR and hTRT. CONCLUSIONS: These results suggest that higher expression of hTRT and hTR may be used as new parameters for evaluating the degree of malignancy of RB. The close correlation between the expression of hTR and hTRT and proliferation and differentiation degrees of RB indicats that the expression of telomerase can influence the proliferation and differentiation of RB. Because the telomerase is presented in the Rb, therefore, modulating the telomerase activity may be used as a novel treatment for the RB.

[Detection of fetal SRY gene in maternal plasma by real-time fluorescence quantitative PCR].

OBJECTIVE: To isolate fetal DNA from maternal plasma and examine its fetal origin. METHODS: Fetal DNA in maternal plasma was isolated from 150 samples in the first trimester and mid-trimester of pregnancy, respectively. Real-time fluorescence quantitative polymerase chain reaction PCR (FQ-PCR) was used to determine sex-determining region Y (SRY) gene on Y chromosome. RESULTS: Eighty-two women in the first trimester and 90 women in the mid-trimester carried male fetuses,70 and 90 samples of them were positive, respectively. The mean concentrations were (58.82+/-20.90) copies/ml and (152.08+/-62.61) copies/ml. The results of FQ-PCR were negative in the women who carried female fetuses. CONCLUSION: The results show that fetal SRY gene can be found at a time as early as 42 days of gestation in maternal plasma by the use of FQ-PCR. The number of fetal DNA increases with gestational age. The real-time FQ-PCR is of great value in the non-invasive prenatal diagnosis.

[The effect of minerals on transformation of sulfur during pyrolysis and partial gasification].

In this study, the effect of acid treatment of Yima coal on predesulfurization during pyrolysis and partial gasification were investigated. The coal was treated with HCl, HCl-HF and HCl-HNO3 solution, in which different minerals were eliminated. The whole process could be divided into pyrolysis and partial gasification. In pyrolysis process, the evolution of H2S and COS show two peaks, which were 450 degrees C and 600 degrees C respectively. At high temperature, the amount of H2S evolution from acid treated coal was markedly increased compared to the raw coal through the elimination of minerals. In the case of HCl-HNO3 treated sample, the H2S evolution in the low temperature region, below 500 degrees C, was reduced and the COS evolution in higher temperature region, above 500 degrees C, was increased by the chemical oxidation. In the partial gasification process, the sulfide sulfur removal of acid treated coal char was increased, reached about 100% at 800 degrees C. The maximum of total and sulfide sulfur removal of parent coal char were 46.39% and 52.97% at 700 degrees C. Compared to acid treated coal char, the effect of alkaline minerals for fixing sulfur was strengthened with increasing the temperature. The organic sulfur of acid treated coal char was released with carbon gasification, and the value could be constant.

[Multivariate analysis of prognostic factors in endometrial carcinoma].

BACKGROUND & OBJECTIVE: A number of prognostic factors have been evaluated in endometrial carcinoma. Among them, only a few factors have been recognized as major adverse factors affecting prognosis independently. This study was to explore the independent prognostic factors in endometrial carcinoma. METHODS: A total of 265 patients who accepted primary surgical treatment in our hospital from Jan. 1990 to Dec. 2000 were studied retrospectively. Prognostic factors were analyzed using univariate model and multivariate regression model. RESULTS: The 5-year recurrence-free survival rate of all patients was 83.3%, and 5-year overall survival rate was 84.3%. The univariate model revealed that clinical stage, pathological stage, pathologic grade, pathologic subtype, depth of muscularis invasion, cervical invasion, lymph node metastasis, peritoneal cytology, lymph-vascular invasion, and adnexa metastasis significantly associated with 5-year recurrence-free survival, and 5-year overall survival (P< 0.05), while age, and complications have no significant association with prognosis (P >0.05). However,on multivariate regression analysis, only pathohistological stage, pathologic grade, depth of muscularis invasion, and cervical invasion significantly correlated with 5-year recurrence-free survival, and 5-year overall survival (P< 0.05); clinical stage correlated with 5-year recurrence-free survival significantly (P< 0.001), but not with 5-year overall survival (P=0.074). Hematogenous dissemination rate of patients with muscularis invasion of >50% was significantly higher than that with muscularis invasion of <or=50% (12.9% vs 0.6%, P< 0.001). Lymphatic failure rate of patients with cervical invasion was significantly higher than that without cervical invasion (21.1% vs 3.6%, P< 0.001). CONCLUSIONS: FIGO stage, pathologic grade, depth of muscularis invasion, and cervical invasion are independent prognostic factors in endometrial carcinoma. Comparing with clinical stage, pathohistological stage could assess prognosis more objectively. Deep muscularis invasion is the strongest predictor of hematogenous dissemination; and cervical invasion increases the lymphatic failure rate.

[Construction of AFLP molecular markers linkage map and localization of green cocoon gene in silkworm (Bombyx mori)].

Based on an improved method of AFLP, AFLP markers were employed for construction of a linkage map and localization of Gc gene used a set of 44 backcross lines( BC1) of silkworm ( Bombyx mori) as a mapping population. In this work, all together 3 956 bands were obtained by 28 pairs of primers and 141.3 bands each primer pair on average. Among them 2 836 bands were in good agreement with the segregation pattern. A total of 1 018 (25.7%) polymorphic AFLP markers were detected. The 693 (68.1%) of polymorphic markers with 1:1 segregation ratio ( P < or = 0.05) were obtained. Furthermore,The analytical model was based on the backcross type and the parameters were set as following: LOD = 3.0, maximum recombination value of 0. 20 and use the command ' group', 'compare', 'try', 'map' and 'ripple' to construct the linkage maps. 407 of the 693 loci were chi2 tested in agreement with 1:1 segregation were divided into 33 linkages by Mapmaker/Exp(Version 3.0), with a total map distance of 3 676.7 cM and a mean distance of 9.1 cM between markers. The morphological gene Gc was located between L-P4T6-107 and L-PT6T4-84 on linkage group 22. In addition, 286 markers were not included in the linkage groups. The efficiency of loci mapping was 58.7%. Among the 33 linkage groups, the morphological marker Gc classically localized on linkage group 15 was relocated on linkage group 22 on the map, suggesting that this molecular linkage group corresponds to linkage group 15 on the linkage map based on morphological characters. All these have laid an important base for the marker assisted breeding of the silkworm.

[Construction of a three-dimensional human angiogenesis model in vitro for antiangiogenic drug selection].

OBJECTIVE: To develop a simple human angiogenesis model that simulates all the critical steps of the angiogenic process for screening antiangiogenic substances. METHODS: Human umbilical vein endothelial cells (HUVECs) were sandwiched into three-dimensional collagen gels for culturing to produce the angiogenesis model in vitro, on which the antiangiogenic effects of genistein, tyrphostin A23 and lavendustin C were assessed. RESULTS: HUVECs had the capacity to repidly form highly organized capillary-like structures in this model with uniformity of the entire culture, and was easily reproducible. Tyrosine kinase inhibitors, genistein, tyrphostin A23 and lavendustin C were found to inhibit the capillary tube formation. CONCLUSION: This new human angiogenesis model has proved to be simple, responsive, and reproducible for testing potential angiogenic inhibitors, and is suitable for screening anti-angiogenic drugs.

Effects of timely insulin treatment on protection of beta cells in a rat model of type 2 diabetes mellitus.

BACKGROUND: Insulin treatment plays a key role in management of diabetes mellitus. Clinical researches showed that extra improvements in restoration of insulin secretion of pancreatic beta cells were found in patients with newly diagnosed type 2 diabetes. The purpose of this study was to investigate the effects of early insulin treatment on insulin mRNA expression and morphological alterations of beta cells in a Sprague Dawley (SD) rat model of type 2 diabetes. METHODS: A rat model of type 2 diabetes mellitus (T2DM) was induced by a high fat diet (high energy, HE) and low doses of streptozotoxin (STZ, 40 mg/kg). A group of diabetic rats was then injected with protamine zinc insulin [PZI, 1 - 2 U x kg(-1) x d(-1)] for one week. Insulin mRNA expression, morphological features of pancreatic islets, and metabolic parameters were examined in rats using reverse transcriptase-polymerase chain reaction (RT-PCR), immunohistochemistry, and other techniques. RESULTS: In insulin-treated diabetic rats, insulin mRNA levels prominently increased by 81.3% (P < 0.05), as compared with untreated diabetic rats. Moreover, timely insulin treatment noticeably improved the insulin content of beta cells, with an increase of 10.2% (P < 0.05), despite a slight reduction in fasting blood glucose (FBG), triglyceride (TG), and free fatty acid (FFA) levels, as compared to an untreated diabetic group. CONCLUSION: Insulin treatment at the onset of T2DM effectively improves insulin synthesis, as confirmed by morphological changes to beta cells in a rat model of type 2 diabetes.

Comparison of diode and argon laser lesions in rabbit retina.

PURPOSE: To compare the histological alteration of retina with various spot intensities between diode and argon lasers in order to instruct the clinical use of 810 nm diode laser. METHODS: Transpupillary retinal photocoagulations were performed on 42 eyes of 27 pigmented rabbits. Histopathologic alteration of lesions in different intensities and different time intervals after irradiation produced by diode and argon laser was observed and compared using light microscopy. Areas of various lesions measured by image analysis system (CMIAS) were compared quantitatively. RESULTS: Histopathologically, two-week-old grade 2 lesions produced by diode laser induced the disappearance of outer nuclear cells. More than a half of all showed reduction in number of outer nuclear layer cells in argon. Fibroblasts appeared in the diode grade 3 lesions 5 days after irradiation. CMIAS data showed that all the areas of diode lesions immediately after photocoagulation were to be larger than those of argon laser lesions in the same spot intensity (P < 0.05). However, twenty-four hours after photocoagulation, the area of the diode lesions increased less than that of the argon laser lesions (8% vs. 23%). CONCLUSION: The acute histological effect caused by 810 nm diode laser and argon green laser is similar, while the expansion of lesion area 24 hours after photocoagulation was less with the diode laser compared to the argon. This may be the first report in the literature regarding quantitative analysis of the delayed reaction of argon green lasers.

Analysis of microsatellites derived from bee Ests.

To accelerate the molecular analysis of genetics, evolution and behavior, etc. in the honey bee (Apis), the frequency and density of simple sequence repeats (SSRs) have been analyzed from the bee EST (expressed sequence tag) database comprising 15 869 sequences amounting to 7.9 Mb. Results showed that the frequency of SSRs was 1/0.52 kb in bee ESTs, and hexanucleotide repeats (45.0%) motifs appeared to be the most abundant type in bee,the dinucleotide, mononucleotide, trinucleotide, tetranucleotide and pentanucleotide repeats are 17.9%, 14.1%, 11.6%, 9.2% and 2.2%, respectively. Meanwhile, the A-rich repeats are predominant in each type of SSRs, such as A, AT, AG, AC, AAT, AAG, AAC, AAAT, AAAG, AAAAG, AAAAT, AATAT, AAAAAG and AAAAAT repeats, whereas G-rich repeats are rare in the coding regions. The further analysis suggests that, apart from minor deviations, there is no significant difference in the distribution and density of microsatellites in the redundant and non-redundant set of bee ESTs. Furthermore, the availability of microsatellite markers can be expected to enhance the power and resolution of genome analysis in bee.

[Study on relationship between TCM Syndrome types (Xin-qi and Xin-yang deficiency) and contents of cytokines (tumor necrosis factor-alpha and interleukins) in patients with congestive heart failure].

OBJECTIVE: To explore the relationship between TCM Syndromes types, Xin-qi deficiency (XQD) and Xin-yang deficiency (XYD), and contents of cytokines, including tumor necrosis factor alpha (TNF-alpha), interleukin-6 (IL-6), IL-1beta and IL-10 in patients with congestive heart failure (CHF). METHODS: Sixty-seven CHF in-patients with Xin-qi deficiency or Xin-yang deficiency syndrome were enrolled, their NYHA (New York Heart Association) cardiac function was assessed, ejection fraction (EF) and E peak/A peak (E/A) ratio were determined by Doppler ultrasonic echocardiograph, and serum TNF-alpha, II-6, IL-1beta and IL-10 were measured by double antibody sandwich ELISA assay. RESULTS: The cardiac function grading in patients of XQD was mostly of I and II grade, while that in patients of XYD was mostly of III and IV grade (by Ridit analysis, P<0.01). The E/A and EF values were higher in patients of XQD than those in patients of XYD (P<0.01). Levels of TNF-alpha, IL-6 and IL-1beta were higher and IL-10 was lower in patients of XYD than those in patients of XQD (P<0.05 or P<0.01) respectively. CONCLUSION: TNF-alpha, IL-6, IL-1beta and IL-10 probably play an important role in the development process of XQD to XYD, and could be taken as the microcosmic indexes for differentiation of the two syndromes.

[Prognostic factors of stage IB and IIA carcinoma of the cervix treated by surgery].

OBJECTIVE: To evaluate prognostic factors in patients with stage IB-IIA of cervical carcinoma treated by surgery. METHODS: Between December 1992 and December 2001, 111 patients with stage IB-IIA cervical cancer surgically treated were analyzed. Median age 40 years. According to 1994 FIGO Staging System: IB 80 (IB1 40, IB2 40) and IIA 31. There were 93 cases of squamous cell carcinoma (83.5%), 17 cases of adenocarcinoma (15.3%) and one case of small cell carcinoma. All patients were treated by radical hysterectomy and pelvic lymphadenectomy, 74 patients had preoperative adjuvant radiotherapy, 24 patients had postoperative adjuvant treatment. Kaplan-Meier method was used to evaluate the survival, the related prognostic factors were assessed by Cox regression and chi(2) test. RESULTS: The overall 5-year survival rate was 85.9%, being 89.1%, 90.7% and 78.4% for stage IB1, IB2 and IIA, respectively. Univariate analysis showed that tumor size (hazards ratio [HR] = 1.479, P = 0.152), tumor type (HR = 1.440, P = 0.264), clinical stage (HR = 1.380, P = 0.354), adjuvant treatment (HR = 1.210, P = 0.450), lymph node metastasis (HR = 1.432, P = 0.540), endocervical involvement (HR = 2.244, P = 0.036), depth of myometrial invasion (HR = 3.295, P = 0.06) and multiple sexual partners during pregnancy (HR = 10.172, P = 0.000) were of prognostic significance. The latter two were the most important factors indicative of poor prognosis. CONCLUSION: The depth of myometrial invasion and multi-partners combined with pregnancy are closely related to the prognosis while the pre- and/or postoperative adjuvant therapy should be considered for stage IB-IIA cervical cancer with deep myometrial invasion and in pregnant patients with multiple sexual partners.

Pancreatoduodenectomy with vascular reconstruction in treating carcinoma of the pancreatic head.

BACKGROUND: The incidence of carcinoma of the pancreas is increasing in the world. Pancreatic carcinoma is characterized by early local extension to contiguous structures and metastases to regional lymph nodes and the liver. This study was conducted to increase the rate of pancreatoduodenectomy combined with vascular reconstruction. METHODS: Pancreatoduodenectomy with vascular reconstruction was performed for 79 patients at a number of hospitals in Fujian Province, Zhejiang Province, Shanghai and Xinjiang Uyghur Autonomous Region from April 1994 to December 2003. One of these patients also underwent right hemicolectomy; but all received through superior mesenteric vein (SMV)-portal vein (PV) reconstruction. The reconstructions of the superior mesenteric artery (SMA) and hepatic artery (HA) were performed in 4 patients, and reconstructions of the SMA or HA were carried out in 7 and 4 patients respectively. Partial reconstruction of the inferior vena cava (ICV) was done in 2 patients when the tumor was adhering to the wall of the inferior caval vein. RESULTS: Four patients died during the peri-operative period, with a mortality rate of 5%. No complications such as biliary or pancreatic fistulae or artificial blood vessel infection were noted. Histological examination showed one patient with neuroendocrine cancer and the other 78 patients with adenocarcinoma of the pancreatic head. Resected endothelia and vascular margins proved to be microscopically tumor-free. Follow-up for 3 months to 10 years for all except two patients showed 7 of the 9 patients who had undergone resection and reconstruction of the SMA and HA died 7 months or 4 years after operation and 37 survived for over 3 years and 12 for more than 5 years. The rest are still under follow-up. CONCLUSION: Pancreatoduodenectomy with vascular reconstruction for carefully selected patients with carcinoma of the pancreatic head has proved to be a safe and reliable treatment, capable of raising the rates of tumor resection and survival.

[Value of Doppler color echocardiography in diagnosis of Kawasaki disease with cardiac involvement].

OBJECTIVE: To assess the diagnostic value of Doppler color echocardiography for Kawasaki disease (KD) with cardiac involvement. METHODS: Doppler color echocardiography was performed in 58 KD cases for measurement of the diameter of the coronary artery and the dimensions of the ventricles. The ejection fraction (EF) of the heart was also calculated and the presence of pericardial effusion and valvular regurgitation examined. RESULTS: Cardiac involvement was identified in 31 (53.45%) of the total 58 patients, and 28 patients were found to have coronary artery lesions. Uncomplicated left and right main stem coronary artery lesions were detected in 17 and 3 patients, respectively, lesions involving both two coronary arteries in 8, coronary aneurysms in 2, left ventricle enlargement in 9, right ventricle enlargement in 6, and enlargement of the whole heart in 4 patients. Eighteen patients had mitral regurgitation, 16 had tricuspid regurgitation, 13 had pericardial effusion, and 2 developed temporary cardiac dysfunction. CONCLUSIONS: Doppler color echocardiography is capable of depicting dynamically the pathological changes and progression of the coronary artery lesions with prognostic indications, to serve as the most valuable non-invasive modality for diagnosis of KD with cardiac involvement.

[Clinical significance of mdm2 and p53 expression in orbital rhabdomyosarcoma].

OBJECTIVE: To evaluate the role of oncogene mdm2 and p53 in the development of orbital rhabdomyosarcoma (ORMS) and demonstrate the feasibility of mdm2 and p53 as biological characteristic markers and prognostic indicators. METHODS: Immunostainings of mdm2, p53 and Ki-67 antigen were performed on archival paraffin-embedded tissues taken from 31 ORMS patients. In situ hybridization was used to demonstrate the expression of mdm2 and p53 on the mRNA level. The correlations between the two genes and the clinical histopathologic parameters including age, gender, cells subtype, differentiation grade and proliferation index were analyzed. RESULTS: The positive expressions of mdm2 and p53 were 77.4% (24/31) and 71.0% (22/31) respectively; and the co-expression of mdm2 and p53 was 61.3%. The expressions of mdm2 and p53 on the protein level were in coincidence with that on the mRNA level. (1)The poorly and moderately differentiated group showed significantly higher expression of mdm2 and co-expression of mdm2-p53 than the well-differentiated group (P = 0.007; P = 0.009, respectively). (2) The samples were divided into actively-proliferating group and inactively-proliferating group in accordance with the expression of Ki-67 protein. The positive expression of p53 was significantly higher in actively-proliferating group than inactively-proliferating group. No statistic correlation of the positive expression of mdm2 and p53 with the other histopathologic parameters was observed. CONCLUSIONS: mdm2 and p53 were significantly correlated with the differentiation and proliferation of ORMS. The co-expression of mdm2 and p53 expression suggested that mdm2 might play a role in deactivation of p53. The overexpression of mdm2 and p53 leaded to the loss of cell cycle control and may contribute to the tumorigenesis and tumor progression of ORMS. Mdm2 and p53 may be prognostic indicators of the tumor.
