Gastric electrical stimulation using endoscopically placed mucosal electrodes reduces food intake in humans.

BACKGROUND: Implantable gastric stimulation (IGS) has been proposed for treating obesity. The aim of this study was to investigate the effects of temporary mucosal electrical stimulation on water and food intake as well as gastric emptying in healthy humans. METHODS: The study was designed to study the effects of temporary gastric electrical stimulation (GES) on symptoms, gastric accommodation, food intake, and gastric emptying. It was performed in 12 healthy volunteers on 3 consecutive days. GES was performed using mucosal electrodes endoscopically placed in the fundus. RESULTS: The amount of maximum water intake was reduced with GES (894 +/- 326 mL) compared with sham-GES (1,093 +/- 417 mL, p = 0.01). The food intake was also reduced with GES (p = 0.012). In comparison with sham stimulation, GES delayed gastric emptying during the first 45 min after the meal but not during the remaining time. GES with parameters effective in reducing water and food intake and delaying gastric emptying did not induce significant dyspeptic symptoms, compared with sham stimulation. CONCLUSIONS: GES using temporary mucosal electrodes decreases food intake as well as maximum intake of water, and has a tendency of delaying gastric emptying. It may have a potential application for the treatment of obesity.

Theoretical and experimental research on a disk-type non-contact ultrasonic motor.

We developed a disk-type non-contact ultrasonic motor based on B22 vibration mode. The rotors of SU-8 photoresist are fabricated by the UV-LIGA process to control their shapes and thicknesses. So the structures of them are optimized by the experiments. It is found that the revolution speed of disk-type non-contact ultrasonic motor not only depends on the vibration amplitude of the stator, but also the weight and construction of the rotors. The maximum revolution speed of the optimal rotor is 3569 rpm at the input voltage of 20 V and the driving frequency of 45.6 kHz. The exciting principle of traveling wave is presented with theoretical equations. The electric signals applied to the piezoelectric ceramic are designed by the principle. The natural frequency and corresponding vibration mode are calculated and analyzed using finite element method. It is shown that experimental results are in good agreement with simulation, which verifies the effectiveness of the finite element model. Moreover, the levitation distance between the stator and rotor is measured by a CCD laser displacement transducer.

Premature mortality in people with epilepsy in rural China: a prospective study.

BACKGROUND: In China, few studies have described annual mortality associated with epilepsy in a general population and these have provided a range of 3.0-7.9 deaths per 100,000 people. We calculated the case fatality rate (CFR), proportional mortality rate (PMR), and standardised mortality ratio (SMR) to assess mortality in people with epilepsy in rural China. METHODS: The target population was people with epilepsy who participated in an assessment of epilepsy management at primary health level in rural China. Neurologists confirmed the diagnosis using strict criteria in all participants who were then treated with phenobarbital. Demographic data and putative cause of death were recorded for each person whose death was reported. PMRs for each cause of death and SMRs were estimated on the basis of the 2004 Chinese population. FINDINGS: Case fatality rate was 1.4% (35 deaths) among 2455 people with epilepsy. The age-adjusted PMRs for injury, stroke, neoplasm, myocardial infarction, and pneumonia were 30%, 30%, 15%, 6%, and 5%, respectively. The SMR was 3.9 (95% CI 3.8-3.9). Patients aged 15-29 years had higher mortality ratios than did those in other age-groups, with SMRs exceeding 23. INTERPRETATION: Risk for premature death is three to four times higher in people with epilepsy than in the general Chinese population. Furthermore, the risk in young people with epilepsy in China is much higher than previously reported. Injury, stroke, myocardial infarction, and pneumonia are among the leading putative causes of death in patients with epilepsy in rural China.

Biomarkers of oxidative damage to predict ischaemia-reperfusion injury in an isolated organ perfusion model of the transplanted kidney.

Ischaemia-reperfusion (IR) injury is known to be a risk factor influencing both short and long-term graft function following transplantation. The pathophysiology of IR injury is suggested to involve elevated reactive oxygen species production resulting in oxidative damaged cellular macromolecules. The objective of this study was to evaluate oxidative damage following IR using an isolated organ perfusion model of the transplanted kidney, in order to determine a simple, preferably non-invasive biomarker for IR injury. Porcine kidneys were retrieved with 10 or 40 min warm ischaemic (WI) time and haemoperfused for 6 h on an isolated organ perfusion machine. ELISA was used to detect carbonyls, 8-isporostane and 8-hydroxy-2'-deoxyguanosine, representing protein, lipid and DNA damage respectively in pre and post reperfusion samples of plasma, urine and biopsy material. Plasma carbonyl and 8-isporostane and were significantly increased in the 40 min group compared to pre-perfusion (0.96 +/- 0.10 vs. 0.62 +/- 0.06, P < 0.001 and 1.57(1.28-4.9) vs. 0.36(0.09-0.59), P < 0.05). The levels also correlated with creatinine clearance used to determine renal function (r = - 0.6150, P < 0.01 and r = - 0.7727, P < 0.01). The results of this study suggest both plasma carbonyl and 8-isporostane to be reliable biomarkers to predict the level IR injury.

Production of an anti-severe acute respiratory syndrome (SARS) coronavirus human monoclonal antibody Fab fragment by using a combinatorial immunoglobulin gene library derived from patients who recovered from SARS.

A combinatorial human immunoglobulin gene library was constructed from the peripheral lymphocytes of two patients who recovered from severe acute respiratory syndrome (SARS). The library was screened for the production of Fab antibody fragments to a recombinant spike protein of SARS-associated coronavirus (SARS-CoV). One Fab clone, AS3-3, reacted with the spike protein in an enzyme-linked immunosorbent assay. The dissociation constant of AS3-3 was 1.98 x 10(-8) M. Immunofluorescent microscopy revealed that it reacted with SARS-CoV-infected cells. The library seems to be a potent tool for the production of human antibodies to SARS-CoV.

[Methylation status of the 5' CpG islands in FHIT gene in the plasma and tissues of cervical cancer patients].

To evaluate the methylation status of the 5' CpG islands in FHIT gene using plasma and tissue samples from cervical cancer patients and find a novel marker for non-invasive diagnosis of cervical cancer, methylation-specific PCR (MSP) was employed to examine CpG island methylation in FHIT gene in 151 pretreatment plasma samples and 30 tumor tissue samples obtained from cervical cancer patients. MSP product was cloned and sequenced directly. CpG island methylation of FHIT was detected in 31.13% of the plasma samples, and in 53.33% of the tissue samples. The total concordant rate of methylation status between plasma and tissue samples in FHIT gene was 80.00%. We found a strong positive correlation between FHIT methylation in the plasma and the clinical stage and histological grade of the tumor. The data showed that CpG island methylation of the FHIT gene is prevalent in the plasma and tissue samples from cervical cancer patients. FHIT detection may be used as a non-invasive marker for diagnosis of cervical cancer and prognostic treatment evaluation.

[Determination of contents of the effective ingredients in root of Scutellaria baicalensis by HPLC-ECD].

OBJECTIVE: To develop an HPLC-electrochemical detection (HPLC-ECD) method for determination of contents of three effective ingredients baicalin, baicalein and wogonin in Scutellaria baicalensis simultaneously. METHOD: 70% ethanol extract of S. baicalensis was separated on a Kromasil C18 column (4.6 mm x 250 mm, 5 microm) using a mixture solution of CH3OH and H2O-THF-H3PO4(80:10:0.2) as mobile phase and detected by Waters2465 electrochemical detector at + 800 mV ( vs. ISAAC). Temperature of column was 30 degrees C. RESULT AND CONCLUSION: The linear ranges of baicalin, baicalein and wogonin were 0.045 6-1.14 microg (r = 0.999 6), 0.013-0.325 microg (r = 1) and 0.009 5-0.047 5 microg ( r = 0.999 9), respectively. The average recoveries of baicalin, baicalein and wogonin were 97.9% (n = 5, RSD 2.3%), 97.2% (n = 5, RSD 3.3%) and 103.5% (n = 5, RSD 3.5%), respectively. The contents of three effective ingredients from 14 different germplasm resourses and 9 different growing areas of S. baicalensis were determined, the quality of S. baicalensis was evaluated.

[Efficacy of intersphincteric resection in the sphincter-preserving operation for ultra-lower rectal cancer].

OBJECTIVE: To evaluate the clinical efficacy of intersphincteric resection in the sphincter- preserving operation for ultra-lower rectum cancer. METHODS: Thirty-one rectal cancer patients with the distal edge of the tumour less than 2 cm from the dentate line were evaluated. Eighteen advanced rectal cancer patients received preoperative chemo-radiation. Total mesorectal excision (TME) was performed with the rectum immobilized down, and the puborectal ligament and partial levator cut to the level of the dentate line. In some well-exposed patients, it was possible to further immobilize the rectum between the external sphincter ring and the rectum inner sphincter wall. In anal approach,good exposure was attained and the cut-line was made vertically to the anal canal 2 cm below the lower edge of the tumor, and further intersphincteric immobilization was made upright. Colon or colon pouch were anastomosed to the distal anal wall. RESULTS: There was no peri-operative death. Thirty patients had good fecal control. Twenty-nine patients showed no evidence of recurrence or metastasis after follow-up for 12 months. Recurrence occurred in one case 1 year after operation. Another one had higher CEA 19.9 level, but without evidence of metastasis. CONCLUSION: Radical resection can be attained and anal sphincter preserved by intersphincter resection which is an alternative sphincter-preserving operation.

[Determination of epicatechin in Rhizoma Fagopyri dibotoryis by HPLC-ECD].

OBJECTIVE: To establish a quantitative method of determination of epicatechin in Rhizoma fagopyri dibotoryis by HPLC-electrochemical detection. METHOD: The sample was separated on a column of Diamonsil C18 (4.6 mm x 250 mm, 5 microm) which was eluted with methanol-0.1 mol x L(-1) phosphate buffer (1:3, pH 2.5), the flow rate was 1.0 mL x min(-1), the column temperature was 35 degrees C, the reference electrode was ISAAC (in-situ silver/silver chloride), the work electrode was glassy carbon, the applied potential was + 600 mV. RESULT: The linear range of epicatechin was 0.004 875-1.56 microg, r = 0.999 9. The average recovery was 100.7% with RSD 1.9% (n = 5). The content of epicatechin was different in nine samples purchased. CONCLUSION: The method is good reproducible and can be used to determine epicatechin in Rhizoma fagopyri dibotoryis.

[Fast harvest of seed cells for composite tissue engineered skin at one time].

OBJECTIVE: To find a feasible method that can fast isolate seed cells, keratinocyte stem cell and fibroblasts, for composite tissue engineered skin. METHODS: The foreskin could be attained from posthectomy, the subcutaneous tissue was removed completely, and the full-thick skin was cut into pieces, 2 mm x 2 mm in size, then the pieces were submerged into a centrifuge tube containing collagenase I in a oscillator. After 3-hour digestion at 37 degrees C, the dermis was dissolved completely with all the fibroblasts in the digestion solution and the epidermis could be separated easily. With more than 10-minute digestion in trypsin at 37 degrees C, the epidermal cells could be harvested. Then flowcytometry and FITC-immunofluorescence for cytokeratin 19 of epidermal cells and FITC-immunofluorescence of vimentin of fibroblast were conducted to identify keratinocyte stem cells in the epidermal cells and fibroblasts in the digestion solution. Moreover, epidermal cells and fibroblasts were cultured in vitro for 7 days to investigate their biological behavior. RESULTS: Using collagenase I combined with trypsin, epidermal cells and fibroblasts could be isolated at one time within 3 hours. Up to 17% cells demonstrated cytokeratin 19 positive in the epidermal cells, with fibroblast vimentin positive. The amount of fibroblast could be enlarged to more than 100 times within 6 days, but the putative keratinocyte stem cells were difficult for subculture. CONCLUSION: Seed cells for composite tissue engineered skin could be harvested fast at one time, that made it possible to reconstruct composite tissue-engineered skin in vitro.

[Methylation status of FHIT gene in plasma and expression of FHIT gene in cancer tissue of cervical cancer patients].

OBJECTIVE: To explore the methylation status of 5' CpG island of fragile histidine triad (FHIT) gene in plasma and the expression of FHIT protein in cancer tissue of cervical cancer patients. METHODS: Methylation-specific PCR (MS-PCR) was employed to examine methylation of FHIT gene in 151 plasma samples before treatment. The immunohistochemistry was used to the expression of FHIT protein in cancer tissues. RESULTS: CpG island methylation of FHIT was detected in 31.13% of plasma samples. The expression of FHIT protein was decreased or discarded in 59.60% of cervical cancer tissues. Among them 47.78% was included in methylation positive samples. CONCLUSION: CpG island methylation of FHIT gene in plasma plays an important role on cervical cancer, which results in decreased expression of FHIT protein. It can be used to diagnose and evaluate the effect of treatment to cervical cancers.

Photoisomerisable cholesterol derivatives as photo-trigger of liposomes: effect of lipid polarity, temperature, incorporation ratio, and cholesterol.

Three cholesterol derivatives containing an azobenzene moiety with different polarities were designed and synthesized (AB lipids 1 to 3). The effects of structure, temperature and incorporation ratio on liposomes were studied, with the results showing that the polarity in 4-substituent and in some cases, 4'-substituent may be important for their incorporation feasibility and photoisomerizability in liposomes. Liposomes incorporated with AB lipid 3 could release multi-pulsatilely upon UV and visible light irradiation both in gel state and liquid crystal state of liposomes. An increase in the incorporation ratio of AB lipid 3 enhanced the amount of drug released greatly. Unlike other azobenzene photo-triggers reported, AB lipid 3 did not increase the spontaneous release of liposomes. Furthermore, cholesterol suppressed the spontaneous release of liposomes.

Methylation-based molecular margin analysis in hepatocellular carcinoma.

The positive surgical margins are associated with postsurgical recurrence in hepatocellular carcinoma patients, and molecular margin analysis is considered more sensitive in detecting preneoplastic lesions than conventional histological margin examination. To evaluate the feasibility of methylation-based molecular margin analysis in HCC and explore its clinical application, we investigated CDKN2A methylation status in the surgical margins of 20 HCC patients using a nested BS-MSP protocol and compared the methylation patterns in resection margins with those in the corresponding tumor and adjacent nonmalignant tissues. The results showed that a considerable frequency (35%, 7 of 20) of CDKN2A methylation was present in histologically negative margins, and methylation pattern analysis might be valuable for studying the cellular origin of recurrent carcinoma. Therefore, methylation-based molecular surgical margin analysis offers a promising tool in prognosis for HCC patients who underwent hepatectomy.

A chemical strategy to promote helical peptide-protein interactions involved in apoptosis.

Protein-protein interactions often involve secondary structural elements, such as helices. Protein-protein interactions within the Bcl-2 family are mediated by the helical BH3 domain of proapoptotic family members, such as Bad, Bak, Bax, or Bid. Here, we report that two 5-residue fragments located at the N- and C-termini of the 16-residue BH3 domain of Bad, respectively, serve as affinity-enhancing motifs (AEMs) for the BH3 domain. When added to the BH3 domain derived from other proapoptotic proteins such as Bak, Bax, or Bid, these AEMs significantly increased the Bcl-2-binding affinity of these BH3 peptides by promoting the helical structure. This finding may point to a new strategy for studying and mimicking helical peptide-protein interactions involved in apoptosis.

Comparison of cytocompatibility of zein film with other biomaterials and its degradability in vitro.

UNLABELLED: Cytocompatibility of particle zein (Pzein) and film zein (Fzein) was evaluated and compared with polyhydroxybutyrate (PHB), its copolymer poly(hydroxybutyrate-co-hydroxyvalerate) (PHBV), polylactic acid (PLA), and collagen, using HL-7702 cells, in terms of cell attachment rate within 3 h, and cell viabilities at 3 and 6 days determined by the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) method. The zein degradation test was carried out using collagenase and trypsin, and the degradation product was added to the culture medium at different concentrations in order to examine the concentration-dependent cytotoxic effect. RESULT: The adhesion rate of the HL-7702 cells on both Pzein and Fzein was higher than that on collagen film. Cell viabilities were higher on both Pzein and Fzein than on films of PLA, PHB, PHBV, and collagen from fish skin. Zein can be degraded by both trypsin and collagenase, and the degradation product can enhance cell viability within a certain range of concentrations.
