Comparative efficacy of LDL-C-lowering therapies in first-time vs. recurrent myocardial infarction prevention: a meta-analysis of large-scale randomized controlled trials.

AIMS: Reducing elevated low-density lipoprotein cholesterol (LDL-C) is central to global efforts to prevent myocardial infarction (MI). While many studies have evaluated LDL-C-lowering therapies in first-time and recurrent MI prevention, direct comparisons of their relative efficacy are lacking. Therefore, we conducted a systematic review and meta-analysis to compare the efficacy of LDL-C-lowering therapies in first-time vs. recurrent MI prevention. METHODS AND RESULTS: We searched three databases until 30 November 2024, for randomized controlled trials (RCTs) with at least 1000 patient-years of follow-up. Efficacy was quantified as relative risk (RR) with 95% confidence intervals (CIs). Differences in benefit magnitude were assessed using Cochran's Q test. Data were pooled with a random-effects model, and heterogeneity was measured using the I2 statistic. Additionally, we applied the Cochrane Risk of Bias Tool to evaluate study quality and utilized the GRADE method to assess the certainty of the evidence. This study included 22 large-scale RCTs involving 180 304 participants. In first-time MI prevention, LDL-C-lowering therapies achieved a remarkable 38% reduction in MI risk [12 RCTs; 79 604 participants; RR, 0.62 (95% CI, 0.55-0.69); P < 0.001]. In recurrent MI prevention, these therapies were associated with a more modest but significant 16% risk reduction [11 RCTs; 100 700 participants; RR, 0.84 (95% CI, 0.80-0.88); P < 0.001]. Importantly, the benefit magnitude between the two groups was significantly different (Q = 22.63; P < 0.001), highlighting the greater relative benefit in first-time MI prevention. Furthermore, the robustness of our findings was consistently supported by leave-one-out analyses, the absence of publication bias, high-quality GRADE evidence, and subgroup and sensitivity analyses. CONCLUSION: Our findings suggest that LDL-C-lowering therapies may offer a greater benefit in preventing first-time MI compared with recurrent MI.

Histopathological evaluation of RPL5 expression in triple-negative breast cancer: an integrated immunohistochemical and transcriptomic study.

Triple-negative breast cancer (TNBC) is an aggressive subtype of breast cancer characterized by high invasiveness, limited therapeutic options, and unfavorable clinical outcomes. Ribosomal protein L5 (RPL5), a component of the large ribosomal subunit, has been implicated in ribosome biogenesis, translational regulation, and p53-associated cellular processes. This study investigated the immunohistochemical expression pattern of RPL5 in TNBC tissues and explored its potential biological significance through integrated transcriptomic analyses. Tumor tissues from 37 patients with TNBC and 7 adjacent non-tumorous breast tissues were collected from the Affiliated Tumor Hospital of Xinjiang Medical University between December 2017 and December 2023. RPL5 protein expression was evaluated by immunohistochemistry, and its association with clinicopathological characteristics was analyzed. Public transcriptomic datasets from TCGA-BRCA and GEO were further used to validate RPL5 expression patterns in TNBC. Co-expression analysis and Gene Ontology (GO)/Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were performed to investigate potential biological functions and signaling pathways associated with RPL5. Immunohistochemical analysis demonstrated significantly lower RPL5 protein expression in TNBC tissues compared with adjacent normal breast tissues (p=0.001). In contrast, transcriptomic analyses revealed significantly higher RPL5 expression in TNBC compared with non-TNBC breast cancer subtypes (p<0.001). No significant associations were observed between RPL5 expression and clinicopathological parameters, including age, tumor size, menopausal status, TNM stage, histological grade, or lymph node metastasis (all p>0.05). Survival analysis showed no significant difference in overall survival between patients with high and low RPL5 expression. Functional enrichment analyses indicated that RPL5-related genes were predominantly involved in ribosome biogenesis, translational regulation, and p53-related signaling pathways. These findings suggest that abnormal RPL5 expression may be associated with TNBC biology through ribosome-related programs, although causal roles require functional validation. RPL5 may represent a potential histopathological and molecular indicator associated with TNBC biology, although its precise functional role requires further experimental validation.

Whole-genome evolutionary dynamics of human parainfluenza virus type 3 in Shanghai, China, 2016-2024.

&#x2022; Fifty whole-genome sequencing revealed co-circulating HPIV-3 C3 sub-lineages C3f and C3a in Shanghai, China. &#x2022; Whole-genome phylogeny dated the HPIV-3 tMRCA to &#x223c;1925.6 and revealed two post-1990 demographic expansions. &#x2022; Recombination signals detected in the HN gene and other regions may lead to discordance in partial-gene phylogenies. &#x2022; The L gene showed the highest variability and harbored the largest number of putative positively selected sites.

Proteome analysis of heat shock protein expression in Pseudomonas alcaligenes NCIMB 9867 in response to gentisate exposure and elevated growth temperature.

Pseudomonas alcaligenes NCIMB 9867 (strain P25X) degrades xylenols and cresols via the gentisate pathway. P25X expresses two isofunctional gentisate 1,2-dioxygenases (GDO I and GDO II). The expression of both GDOs was not detected when P25X cells were grown at 42 degrees C, even in the presence of gentisate. A total of 19 heat shock proteins (Hsps) belonging to the Hsp100, Hsp90, Hsp70, Hsp60, Hsp45, and small heat shock protein (sHsp) families were identified among the protein spots that were either newly detected or were expressed at levels of at least twofold higher when P25X cells were cultured at 32 or 42 degrees C in the presence and absence of gentisate. Among these, 16 Hsps were commonly expressed at 42 degrees C. Two additional Hsps (H5 and H13) from the Hsp90 and Hsp60 families, respectively, were expressed only when P25X cells were grown at 42 degrees C and in the presence of gentisate. A protein of the sHsp (H16) family was expressed only in the presence of gentisate at 32 degrees C but not at 42 degrees C. The GroEL chaperonins of the Hsp60 family comprised the largest group of Hsps identified and exhibited high level of expression at 42 degrees C following gentisate exposure.

Studies on the structure and miscibility of mixed Langmuir-Blodgett films of a double-armed dibenzo-18-crown-6 ether with stearic acid.

Reported here is the study on the structure of Langmuir-Blodgett (LB) films of double-armed dibenzo-18-crown-6 contain biphenyl (DACE) which are newly synthesized and mixed with stearic acid (SA). In addition, the miscibility of the two compounds was also tested by the measurement of the surface pressure-area (pi-A) isotherms of DACE and DACE/SA with various proportions. It is noted that there is no phase segregation in the mixed monolayer film of DACE/SA. Upon calculation of the excess surface area, it is concluded that the SA molecule can enter into the crown ether ring, while an 18C6 ring can host a maximum of one SA molecule. The difference of spectroscopic properties of DACE in LB films and bulk solution has been investigated by ultraviolet-visible (UV-vis) and Fourier-transfer infrared (FTIR) measurements. Molecules of DACE exist in the mixed LB films as monomers in contrast to those in the concentrated solution as aggregates. The hydrocarbon chains in DACE and DACE/SA LB films are tilted to the normal of the substrate surface, but perpendicular to the dipole moment of CO. Both CO bonds in the phenyl ethers and carboxylic ester of DACE, and the long axes of phenyl rings are aligned nearly perpendicular to the substrate surface. Infrared spectra of mixed LB films of DACE/SA present further evidence that the SA molecules enter into the crown ether rings of DACE.

Surface-enhanced Raman spectroscopy and density functional theory study on 4,4'-bipyridine molecule.

The molecular geometry and vibrational frequencies of 4,4'-bipyridine (BPE) in the ground state were calculated using density functional theory (DFT) methods (B3LYP) with 6-31++G(d,p) basis set. The optimized geometric bond lengths and bond angles are obtained by DFT employing the hybrid of Beckes non-local three parameter exchange and correlation functional and Lee-Yang-Parr correlation functional. Fourier transform infrared (FT-IR), Fourier transform Raman (FT-Raman) and near-infrared surface-enhanced Raman scattering (NIR-SERS) spectra of BPE on the silver foil substrate have been recorded. All FT-IR, FT-Raman and NIR-SERS band were assigned on the basis of the B3LYP/6-31++G(d,p) method. The vibrational frequencies obtained by DFT(3LYP) are in good agreement with observed results. The NIR-SERS of BPE excited by 1064nm laser line is little difference with that excited by visible laser line. This phenomenon is result to the increase of the contribution of CHEM enhancement effect. Surface selection rules derived from the electromagnetic enhancement model were employed to infer the orientations of BPE on the silver foil substrate surface. Some vibrational frequency which are sensitive to the planar or non-planar structure of BPE, and to the dihedral angle were concluded.

Surface-enhanced Raman scattering on mercaptopyridine-capped CdS microclusters.

We obtained the high-quality Raman spectra of 4-mercaptopyridine (4-Mpy) adsorbed on CdS microclusters. The Raman signals were enhanced relative to the same molecules in solution. We compared the Raman spectra of 4-Mpy molecules adsorbed on CdS microclusters and Ag substrate. The difference of 4-Mpy molecules adsorbed on semiconductor and metal substrate was revealed. The results demonstrated that adsorbed species on semiconductor CdS can be detected by SERS spectroscopy.

Joint anti-estrogenic effects of PCP and TCDD in primary cultures of juvenile goldfish hepatocytes using vitellogenin as a biomarker.

This work evaluated the joint anti-estrogenic effects of pentachlorophenol (PCP) and 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) against 17beta-estradiol (E2) in juvenile goldfish (Carassius auratus) hepatocyte cultures. The level of vitellogenin (VTG) as a biomarker was determined by exposing hepatocytes to individual E2, PCP and TCDD, as well as to E2 in the presence of PCP, TCDD or their mixtures of various concentrations. PCP and TCDD did not exhibit estrogenicity. Both chemicals reduced the estrogenicity of E2, indicating the anti-estrogenic effects of PCP and TCDD. Their anti-estrogenic EC(50) values were calculated. The joint anti-estrogenic effects against E2 increased with increasing the PCP-to-TCDD ratio of mixture. Marking's indices were <0, suggesting an antagonism in anti-estrogenic effects between PCP and TCDD. The anti-estrogenic effects of PCP appeared to result primarily from the competitive binding to estrogen receptor. While TCDD may undergo an indirect binding process for its anti-estrogenic effects, the accurate mechanisms remain to be understood. The observed antagonism in anti-estrogenic effects resulted apparently from the mutual inhibition by PCP and TCDD.

Surface enhanced Raman scattering based on silver dendrites substrate.

A simple method of the reduction of AgNO3 by copper foil in aqueous medium was used to prepare silver dendrites, which can be used as a novel good reproducible surface enhanced Raman scattering (SERS) active substrate. The SERS spectra of 4-pyridinethiol on this novel substrate reflected the different SERS activities on the minuteness and strong Ag dendrites. The electromagnetic coupling enhancement and chemical enhancement mechanisms are used to explain the SERS effect.

A simple method to synthesize triangular silver nanoparticles by light irradiation.

We describe a simple method to synthesize triangular silver nanoparticles by photoreducing the silver ions by citrate. A noteworthy difference of the present method as compared with the previous photo-induced methods is that good shape control over the nanoparticles can be realized in the absence of soft templates or polymer directing agents. The formation process of the silver nanoparticles was investigated by UV-vis spectroscopy and transmission electron microscopy (TEM). It was found that the concentration of reactant plays important role in the morphology control of produced silver nanoparticles. As one of the applications of these nanoparticles, they were used as surface-enhanced Raman scattering substrates and 1,4-bis[2-(4-pyridyl)ethenyl]-benzene (BVPP) was used as a Raman probe to evaluate the enhancement ability of the triangular silver nanoparticles.

Cell growth and shikonin production of Arnebia euchroma in a periodically submerged airlift bioreactor.

Arnebia euchroma was grown in a 2-l periodically submerged, airlift bioreactor (PSAB) in which the non-submerged (immobilization culture) and submerged (suspension culture) operations were controlled automatically. PSAB had advantages in improving cell growth, shikonin content, shikonin production and cell aggregation compared with suspension culture. Under the optimal submerged/non-submerged period of 10 min/15 h, the shikonin content (4.6%, w/w) and, cell dry mass (16.8 g/l) were 229 and 26% higher than those in suspension culture.

Construction of a microsatellite linkage map with two sequenced rice varieties.

Based on the successful development of new microsatellite markers from the data of two whole-sequenced rice varieties, japonica variety Nipponbare and indica variety 9311, an F2 population of 90 lines, which was derived from a single cross between Nipponbare and 9311, was applied to construct a genetic linkage framework map. The map covered 2 455.7 cM of total genomic length, and consisted of 152 simple sequence repeats (SSRs) loci including 46 pairs of new SSR primers developed by our research institute. The average genetic distance between two markers was 16.16 cM. In addition, markers RM345 and RM494, which have not been mapped on the Temnykh's map et al. (2001) were anchored on the sixth chromosome of this map. We compared this research with maps of Temnykh et al.(2001) and LAN et al. (2003) regarding the aspects of type and size of population, type and quantity of markers, and the marker arrangement order on chromosome, etc. Results indicated that the similarity of marker linear alignment was 93.81% between this map and T-map. Finally, the important significance of using sequenced rice varieties to construct linkage map was also discussed.

Analysis of segregation distortion of molecular markers in F2 population of rice.

A genetic linkage map comprising 148 SSR markers loci was constructed using an F2 population consisting of 90 lines derived from a sub-specific cross between a japonica variety Nipponbare and an indica variety Guangluai-4. The F2 population showed high significantly distorted segregations. Among these SSR markers, 49 markers(33.11%) showed the genetics distortion(P<0.05). Of them, 36 markers deviated toward male parent indica GuangLuAi-4 and 13 markers toward heterozygote, but none toward the female parent Nipponbare. It was found that the segregation distortion might be caused by gametophyte and zygote. Since most gametophyte loci and sterility loci were mapped in segregation distortion regions, it indicated that the segregation distortion may be caused by these gametophyte loci and sterility loci. Finally, this research also analyzed the skewed segregation of some markers, which had not been mapped on chromosome.

Nicotine exposure refines visual map topography through an NMDA receptor-mediated pathway.

The precise mapping of one surface onto another is fundamental to visual system organization and depends upon adequate stimulation of postsynaptic targets to stabilize correctly placed synapses. As exogenous nicotine alters neuronal activity, we investigated whether it would affect the visual map created by retinal ganglion cell terminals in the frog optic tectum. Chronic exposure of the tectum to nicotine decreased the retinal area from which cells project to a given tectal site. This map refinement was also produced by exposure to either the alpha-bungarotoxin sensitive nicotinic receptor agonist, anatoxin-a or the alpha-bungarotoxin-insensitive nicotinic receptor agonist epiboxidine. Immunocytochemical studies using mAb306 and mAb22 demonstrated that alpha-bungarotoxin-sensitive and -insensitive nicotinic receptors, respectively, occupied different tectal sites. Choline acetyltransferase immunoreactivity overlapped with mAb306, but not mAb22, staining. The developing optic tectum was more sensitive to nicotine than the adult tectum and nicotine induced both map refinements and map disruptions in a concentration-dependent manner. Blockade of the N-methyl-D-aspartate (NMDA) receptor with D(-)-2-amino-5-phosphonopentanoic acid (D-APV) prevented nicotine from refining the map in the adult tectum. Exposure to the use-dependent NMDA antagonist MK801 alone had no effect on retinotectal topography but in combination with either NMDA or nicotine it disrupted the map. Exposure to NMDA alone produced refinement. We conclude that the map refinement induced by chronic nicotine treatment has as its basis an increase in the level of NMDA receptor activity. The data are consistent with a model whereby map topography can be bidirectionally affected by either increasing or decreasing NMDA receptor activity.

[Therapeutic effects and mechanisms of CpG oligodeoxynucleotides in mice infected with Mycobacterium tuberculosis].

OBJECTIVE: To investigate the protective effects and mechanisms of CpG oligodeoxynucleotides (CpG ODN) in mice infected with Mycobacterium tuberculosis. METHODS: Ninety-six female BALB/c mice were randomized into 4 groups, namely CpG ODN treatment (A group), control ODN treatment (B group), infection control (C group) and healthy control (D group) (n = 24 each). A group and B group were treated once with CpG ODN or control ODN (30 microg/mouse) intraperitoneally 2 weeks before infection with Mycobacterium tuberculosis. 1 x 10(6) bacili in a volume of 300 microl saline was injected into the tail vein of mice from A group, B group and C group. Twelve mice from each group were sacrificed at 3 weeks postinfection. Pathologic changes in lung tissues were observed. The expression of Toll-like receptor 9 (TLR9) mRNA and cytokine mRNA was detected by RT-PCR. The numbers of viable bacteria in lung and spleen were counted. The rest 12 mice from each group were monitored for 60 days to observe the mortality. RESULTS: CpG ODN was shown to increase the survival time of mice infected with Mycobacterium tuberculosis. The body weights of mice from A group [(20.37 +/- 1.12) g] were higher than those of B group [(17.50 +/- 0.62) g] and C group [(17.15 +/- 0.97) g, P < 0.01]. The lung weights of mice from A group [(0.25 +/- 0.02) g] were similar to those of B group [(0.27 +/- 0.34) g, P > 0.05], but less than those of C group [(0.28 +/- 0.26) g, P < 0.01]. The spleens of mice from A group [(0.63 +/- 0.37) g] were larger than those of B group [(0.39 +/- 0.05) g] and C group [(0.38 +/- 0.02) g, P < 0.01]. The inflammation in lung tissue of mice from A group was less than that of B group and C group. There was no mycobacterial outgrowth in lungs and spleens of mice from A group. The expression of TLR9 mRNA in lungs and spleens of mice from A group (0.61 +/- 0.29 and 0.72 +/- 0.48) was similar to that in B group (0.58 +/- 0.35 and 0.64 +/- 0.28) and C group (0.60 +/- 0.32 and 0.65 +/- 0.31, P > 0.05), but higher than that in D group (0.11 +/- 0.08 and 0.26 +/- 0.22, P < 0.01). CpG ODN did not affect the expression of TLR9 mRNA. The expression of IFN-gamma mRNA in lungs and spleens of mice from A group (0.44 +/- 0.07 and 0.76 +/- 0.09) was higher than that in B group (0.19 +/- 0.05 and 0.22 +/- 0.05) and C group (0.16 +/- 0.04 and 0.18 +/- 0.08, P < 0.01). The expression of IL-6 mRNA in lungs and spleens of mice from A group (1.56 +/- 0.29 and 8.21 +/- 0.82) was higher than that in B group (0.86 +/- 0.55 and 0.16 +/- 0.09) and C group (0.78 +/- 0.21 and 0.06 +/- 0.04, P < 0.01). The expression of IL-4 mRNA in lungs and spleens of mice from A group (0.18 +/- 0.05 and 0.06 +/- 0.02) was lower than that in B group (0.31 +/- 0.06 and 1.22 +/- 0.01) and C group (0.35 +/- 0.04 and 1.31 +/- 0.31, P < 0.01). The expression of IL-10 mRNA in spleens of mice from A group (0.05 +/- 0.02) was lower than that in B group (0.57 +/- 0.09) and C group (0.65 +/- 0.15, P < 0.01). CONCLUSION: CpG ODN could increase the immunity of mice against tuberculosis through up-regulation of Th1 immunity and down-regulation of Th2 immunity.

[Identification of Epimedium Koreanum Nakai by Fourier transfer infrared spectroscopy].

The herbal plant Epimedium Koreanum Nakai is a native herb growing in the Changbai Mountain region, Jilin province of China, and is used as a tonic, anti-rheumatic and aphrodisiac in traditional Chinese medicine. The analytical method existing for the identification of Epimedium Koreanum Nakai is time-consuming, and can not be used for fast, in situ and nondestructive analysis. Fourier transform infrared spectroscopy was used to identify wilding and planting Epimedium Koreanum Nakai. The spectra of 18 samples were analyzed. They show some differences in the intensity and the shape of absorption peaks in the range of 1,800-1,200 cm(-1). The absorption intensity of the spectra changed regularly with the producing area. So FTIR spectra could be used for the identification of Epimedium koreanum Nakai.

[Study on vibrational spectra and structure of 4-mercaptopyridine monomer and dimer using density functional theory].

The optimized molecular structure and vibrational frequencies of 4-mercaptopyridine monomer and dimer were studied by density functional theory using B3LYP method with the 6-311++G(d, p) basis set. On the basis of the calculations, the assignments of vibrational spectra were performed on monomer and dimer, and the change in structure and vibrational spectrum of dimer as well as the intermolecular force of forming dimer were investigated. It was found that the two pyridine ring planes are vertical to each other, and the dimer was formed through H-bonding, which is between the nitrogen on one ring and the hydrogen of SH moieties on another. Furthermore, the structure and vibrational spectrum of the dimer have some changes with respect to those of the monomer.

[Supernumerary teeth: report of a rare case and review of the literature].

Supernumerary teeth is a well-recognized clinical phenomenon. Multiple supernumerary teeth are commonly associated with variable syndromes. However the multiple supernumerary teeth reported in mandibular and maxilla premolar region of an 14-year-old female patient without any associated systemic condition/syndrome is a rare case. A review of the literature relating to supernumerary teeth is presented including possible presentations, diagnostic features and treatment options.
