Impact of PCB and p,p'-DDE contaminants on human sperm Y:X chromosome ratio: studies in three European populations and the Inuit population in Greenland.

OBJECTIVE: Recent studies indicate that persistent organohalogen pollutants (POPs) may contribute to sex ratio changes in offspring of exposed populations. Our aim in the present study was to investigate whether exposure to 2,2 ,4,4 ,5,5 -hexachlorobiphenyl (PCB-153) and dichlorodiphenyldichloroethene (p,p -DDE) affects sperm Y:X chromosome distribution. SUBJECTS AND METHODS: We obtained semen and blood for analysis of PCB-153 and p,p -DDE levels from 547 men from Sweden, Greenland, Poland (Warsaw), and Ukraine (Kharkiv), with regionally different levels of POP exposure. The proportion of Y- and X-chromosome-bearing sperm in the semen samples was determined by two-color fluorescence in situ hybridization analysis. RESULTS: Swedish and Greenlandic men had on average significantly higher proportions of Y sperm (in both cohorts, 51.2%) and correspondingly higher lipid-adjusted concentrations of PCB-153 (260 ng/g and 350 ng/g, respectively) compared with men from Warsaw (50.3% and 22 ng/g) and Kharkiv (50.7% and 54 ng/g). In the Swedish cohort, log-transformed PCB-153 and log-transformed p,p -DDE variables were significantly positively associated with Y-chromosome fractions (p-values 0.04 and <0.001, respectively). On the contrary, in the Polish cohort PCB-153 correlated negatively with the proportion of Y-bearing fraction of spermatozoa (p=0.008). CONCLUSIONS: The present study indicates that POP exposure might be involved in changing the proportion of ejaculated Y-bearing spermatozoa in human populations. Intercountry differences, with different exposure situations and doses, may contribute to varying Y:X chromosome ratios.

Gender-specific mortality associated with alcohol consumption in Poland in transition.

AIMS: To show the impact of rapid political and economic transitions on alcohol consumption and associated mortality in different socio-demographic segments of the Polish society, with particular focus on gender differences. Two causes of death associated with drinking (liver diseases and alcohol poisoning) are investigated. DESIGN: Mortality time series 1986-2002 are analysed against consumption estimates and population drinking survey data. SETTING: Poland 1986-2002. PARTICIPANTS: General adult population. MEASUREMENT: Age-standardized annual and 3-year death rates were calculated for age, gender and place of residence subgroups. For education, age-standardized relative frequency of deaths or proportional mortality in four basic educational levels was calculated. FINDINGS: Alcohol-related mortality shows great variability in response to substantial changes in alcohol consumption. Patterns of mortality and their magnitude of change correspond with average alcohol consumption and are therefore different for men and women, for different age and educational groups and in urban and rural areas. Level of education, however, may affect alcohol-related mortality more strongly than overall alcohol intake, particularly with regard to acute consequences of drinking. CONCLUSION: The Polish experience illustrates that the impact of rapid political and economic transitions on alcohol consumption and associated mortality is highly differentiated and specific to gender and social class.

[The application of DPS/HIS system in demographic study of the Polish-German cross border region].

Demographic situation in Polish and German cross border regions was compared in this study. The data from 17 Polish powiats, including 2 cities with powiat status: Gorzów Wielkopolski i Zielona Góra, and from 7 German powiats (Kreis), including 2 cities with powiat status (Kreisfreie Stadt): Cottbus and Frankfurt on Odra were analysed. The data concerning demographic structure of population, live births, perinatal and infant mortality were stored and analysed using Data Presentation System (DPS 2.0). In Polish powiats higher percentage of children and adolescents, and lower percentage of population in age 65 years and more was observed than in German Kreis. Both in Poland and in Germany the cities with powiat status differ from other powiats of their country by low percentage of the young (up to 14 years old in Germany and less than 24 in Poland). In Polish powiats more children are born in relation to both the total population and number of women in reproductive age (15-44 years). The decrease in number of live births observed recently in both countries is stronger in Poland. The perinatal mortality in Polish zone systematically decreased, reaching the level close to that of German zone. Despite the infant mortality in Polish zone is decreasing, it's still almost twice higher than in German zone.

[Methods of time series analysis effect of air pollution on mortality].

The paper describes main aspects of modeling of short-term effect of air pollution on mortality. Basic statistical tool--generalized additive models are presented as well as principles of inclusion of confounding variables in the model and measures of air pollution effect. In the second part of work methods of meta analysis allowing for combining results from several individual studies are presented.

[Short-term effect of air pollution with sulphur dioxide, black smoke and nitrogen dioxide on mortality of urban Polish population].

The paper presents results of analysis of short-term effect of atmospheric air pollution with sulphur dioxide (SO2), suspended particles (BS) and nitrogen dioxide (NO2) for daily number of death from all causes (without external), from diseases of cardiovascular and respiratory diseases and from malignant neoplasms in four Polish cities Kraków, Lódź, Poznań and Wrocław. Analysis was carried out using modern methods of statistical modeling--generalized additive models. Potential confounding factors (long-term changes of mortality, day of week, meteorological conditions, national and summer holidays and periods of influenza epidemics) were taken into account during model building process. Results for every city and from meta-analysis are presented. Statistically significant increase of mortality from all causes was associated with increase in all investigated air pollutants.

[Occurrence of intestinal parasites among first grade students in Poland in years 2002/2003].

UNLABELLED: In the school year 2002/2003 there were examined in Poland some of the population of 7-year-old children for the presence of intestinal parasites, continuing countrywide operation launched in 1988. METHODS: The studies were based on the examination of a single faecal specimen and a cellophane swab using the following methods: wet mount preparations in 0.9% NaCl and diluted Lugol's ioine, zinc sulphate centrifugal flotation and decantation. RESULTS AND CONCLUSIONS: The studies included 31 504 children from 15 provinces of Poland. The parasites most frequently encountered in the examinations included: Enterobius vermicularis (in 3 827 of the examined, i. e. 12, 15%), Ascaris lumbricoides (in 263; 0.83%), Giardia intestinalis (in 217: 0.69%), Entamoeba coli (n 189; 0.60%) and Trichuris trichiura (in 39; 0.12%) The overall percentage of the infected children was 15.4%. The highest number of infected children was found in the province warmińsko-mazurskie (29.6%0), the smallest in slaskie (8.8%). The number of infected among children inhabiting country (19%) was significantly higher than among those from the towns (10.4%). The obtained results confirmed hitherto existing views that in Poland the epidemiological situation of infections with intestinal parasites is steadily improving.

[Trends of hospitalization rates from asthma in children and adolescents in Poland].

Hospitalization rates from asthma in children and adolescents below 19 years of age showed a consistent rising trend over the last two decades and in the period 1984-1996 the rates were doubled in both genders. Among boys the rates were 1.6 times higher than in girls. On average the rates increased annually by 12.2% in boys and by 10.6% in girls. The fastest increase has been noted in the youngest children in the age 0-4 years (20.3% in boys and 18.9% in girls). In the older age groups (5-9 and 10-14 years) the annual increase was in the range of 10.5-14.1%. The slowest increase was observed in children in the oldest age group (3.7%) and in the latter age group there was no differences in hospitalization rates between both genders. We found the highest rates in the southeastern and northern part of the country and this was not compatible with the hygienic hypothesis of asthma and allergic diseases.

Highlights on the health status of the Polish population in 1999/2000.

In the paper we describe the main problems of the Polish population health status and their trends in the past years. A meaningful improvement in several health parameters can be noticed in Poland since 1991. In the year 2000 life expectancy was 69.7 and 78.0 for males and females respectively--an increase by 3.6 and 2.7 years in comparison with 1991 values. There have been declining trends in cardiovascular mortality, cancer mortality (more often in younger age groups), infant mortality, incidence of infectious diseases. Mental health problems are the growing issue. It has been stressed that the pace of progress is far too slow to achieve the level of health indicators in EU countries in the not too distant future.

[Spinal pain in school children].

The investigation was carried out to determine the prevalence of back pain among the population of the school-children aged from 6 to 17 yrs. 3386 children from primary Warsaw schools randomly chosen, were investigated according to the questionnaire based on the diagnostic criteria of juvenile spondyloarthropathies (JSA). Total prevalence of spinal pain was 10.14%. Back pain occurred more frequently than the low - back and cervical pain (5.1% vs 3.44% and 3.04%). The incidence of painful episodes was highest between the age 12 - 15 yrs. Although no definite spondyloarthropathy was found among the investigated children - spinal pain occurred most frequently in those who showed one of the signs included into JSA criteria. This may imply the presence of a risk factor for developing spondyloarthropathy in the future.
