Benefits of a school-based health center in a preschool.

Although school-based health centers (SBHCs) deliver health care to vulnerable children, their effectiveness has not been well documented. This study compared the benefits of an SBHC with a School Health Survey and selected HEDIS measures in preschool children with and without access to an SBHC. Preschoolers with access to an SBHC (N = 130) and preschoolers without access (N = 131) were compared on (a) HEDIS measures including well-child care, immunizations, dental care, and smoke exposure; (b) measures of access and use of physical and mental health services; (c) satisfaction with health care; (d) barriers and facilitators to care; and (e) health insurance. Significant differences were found in parents' perceptions of children's physical and emotional health, self-esteem, incidence of behavioral problems, difficulty in obtaining care, number of hospitalizations, and satisfaction with care received. Findings suggest that holistic services provided by an SBHC positively impact the health of vulnerable preschool children.

Arkansas school nurses' role in statewide assessment of body mass index to screen for overweight children and adolescents.

National surveys that have tracked weight and physical activity in the United States for more than 40 years have shown a continuing increase in the number of overweight children and adolescents. Overweight children and adolescents are showing an increase in diseases related to overweight: Type II diabetes, cardiovascular disease, hypertension, and orthopedic and respiratory conditions. Overweight children are also likely to become obese adults. This article describes Arkansas's efforts to connect with families through schools to prevent or reduce overweight in children. Arkansas school nurses measured the heights and weights of more than 400,000 public school students during the 2003-2004 school year. A detailed protocol for accurate measurement of students is provided.

Adaptation to Addison's disease in a child: a case study.

INTRODUCTION: Addison's disease is a rare chronic illness caused by adrenocortical insufficiency. Understanding the process of family adaptation to this chronic illness in a child will help practitioners enhance the ability of the child and family to cope with this stressful diagnosis. METHOD: A qualitative descriptive case study methodology was used to explore the process of family adaptation to this chronic illness. Eight interviews were conducted with a newly diagnosed 11-year-old White girl with Addison's disease, along with her parents, grandparents, and siblings. Participant observations were made in the family's home. Drawings by the girl with Addison's Disease were included in the data analysis, along with an interview with the child. RESULTS: The overall theme of adaptation to this chronic illness is living a normal life. The family noted that after reaching some acceptance, they began learning how to manage the disease. This process was enhanced by seeking information about the disease and receiving the support of others; in addition, the child was affected by the acceptance of her peers. DISCUSSION: Recognizing factors that may influence the family's adjustment to the diagnosis of Addison's disease in a child will help to direct educational and support efforts by practitioners.

School-based health centers. Statewide quality improvement program.

School-based health centers have been proposed as an avenue to provide health care for vulnerable youth. Colorado has 45 school-based health centers that provided 50,000 visits to 18,600 vulnerable youth in the 1998-1999 school year. Sustainability of the centers, however, depends on the ability to obtain an ongoing stream of revenue including managed care contracts, which in turn depend on the center's ability to meet the standard of care in the community. School-based health centers should meet standards of care similar to those of community health centers including certification, credentialing of providers, and a systematic evaluation of the outcomes of services. This article reports on a statewide quality improvement program that established certification standards for school-based health centers, verified the credentials of providers at the centers, and measured quality of care in centers across the state.

Qualitative evaluation of a school-based support group for adolescents with an addicted parent.

BACKGROUND: Adolescents with an addicted parent are at risk for physical, emotional, and social problems. They are particularly at risk for developing substance abuse. School-based support groups have been suggested as a beneficial treatment and prevention strategy. OBJECTIVE: To examine the features, critical attributes, processes, and benefits of school-based support groups for adolescents with an addicted parent. METHODS: A qualitative evaluation using the ethnographic method was conducted at two Midwestern suburban high schools. RESULTS: The study resulted in a comprehensive description of school-based support groups and an outline presenting benefits of participation for adolescents with an addicted parent. The benefits of group participation included increased knowledge, enhanced coping, increased resilience, improved relationships, and improved school performance. DISCUSSION: Findings from this study suggest that school-based support groups are beneficial to adolescents with addicted parents. Experiential knowledge is the foundation of these self-help groups. School-based support group participation enhanced self-knowledge and led to self-care and self-healing. The school-based support groups expanded the adolescents' awareness, resulting in their ability to make critical choices that facilitated changes in the dysfunctional pattern. Support group participation empowered youth to make these changes.
