Uveal melanoma in young patients.

INTRODUCTION: This study aimed to investigate the clinical profile of young patients with uveal melanoma treated in 1995-2002 in the Department of Ophthalmology, Jagiellonian University, Krakow, Poland. METHODS: Retrospective case-control series. RESULTS: Out of 800 patients with uveal melanoma treated between 1995 and 2002, there were 11 patients (1.4%) aged 20 or younger. The mean age of those patients was 17.9 (12-20, median 19) years. There were five men (45%) and six women (55%). In all cases, the tumor was unilateral. In five (45%) cases the tumor affected the right eye and in six (55%) cases the left eye. Eight (72%) patients were treated with local resection. Among these, eight patients, in two (18%) with iris or ciliary body melanoma we used adjuvant Ru-106 episcleral plaque brachytherapy (EPB). In two (18%) cases of choroidal melanoma, we performed I-125 EPB: one case ended with regression of the tumor, while the second did not respond to EPB and we had to enucleate the eye. In one (9%) case we performed enucleation as the primary treatment. The mean follow-up was 61.4 months, ranging 30-125 months. All those patients are still alive and no metastases were detected. CONCLUSIONS: Uveal melanoma is a rare malignancy in children and teenagers. In the present series, it occurred in the iris in more than 50% of cases.

[Diode laser photocoagulation for retinopathy of prematurity--outcomes in one-year observation].

PURPOSE: To evaluate structural and functional outcomes in one-year old patients, treated for retinopathy of prematurity (ROP) with transpupillary diode laser photocoagulation. MATERIALS AND METHODS: 58 children (116 eyes) 12 months old, after treatment for ROP with diode laser photocoagulation, were examined. Both, functional outcome (visual acuity--basing on the examination with Teller Acuity Card Procedure), and structural outcome were evaluated. The cycloplegic refraction was examined in all cases. RESULTS: We have found good functional outcome in 81.9%, and good structural outcomes in 96.5% of examined eyes. 64.6% of eyes had hyperopic refractive error, and 31% were myopic. CONCLUSIONS: Diode laser photocoagulation is an effective method of treatment for ROP.

[Results of 10 years observation of organ of sight in prematurity].

PURPOSE: (1) To compare the incidence of refractive errors and the orthoptic condition in ten-year-old children, born prematurely and treated with cryotherapy versus children, whom did not apply such a procedure. (2) To estimate condition of the extraocular muscles. MATERIAL AND METHODS: A group of 60 prematurely born children was divided into 2 subgroups: I--30 children treated with cryotherapy, II--30 no treatment was applied. All the children had anterior segment, fundus and visual acuity examined. Refraction following cycloplegia was evaluated. Visual acuity and orthoptic status was assessed. RESULTS: In the group of prematurely born children who had retinopathy in the infantile age and were treated with cryotherapy, refractive errors were frequently observed. In over half of those children squint or impaired eye movement were found. In the group of children who required no cryotherapy the percentage of those with refractive errors was similar. CONCLUSIONS: In prematurely born children with retinopathy refractive errors and squint occur quite often. Prematurely born children should be subject to ophthalmologic follow-up throughout their childhood.

[Results of combined chemoreduction and local treatment for intraocular retinoblastoma].

PURPOSE: Of this study was to evaluate the results of combined treatment for intraocular retinoblastoma. MATERIALS AND METHODS: We examined 23 children (32 eyes), 14 with unilateral and 9 with bilateral disease. 53% of eyes had the stage V of the disease, according to Reese-Ellsworth. RESULTS: In I-III group according to R-E, it was possible to save more than 90% of eyes. In group IV-V we saved 20% of eyes. CONCLUSIONS: Chemoreduction combined with local treatment is an optimal method of treatment for retinoblastoma, especially in I-III group.

[Bronchial carcinoid--a rare neoplasm metastasis to choroid].

PURPOSE: To present a clinical picture and results of treatment in two patients with bronchial carcinoid metastases to choroid. MATERIAL AND METHODS: Two females aged 47 and 56 with bilateral and multiple metastases of bronchial carcinoid were enrolled in this study. Choroidal metastases occurred 6-7 years after primary tumors had been diagnosed. The follow-up period ranged from 2.5 to 5 years. In one patient teleradiotherapy of choroidal metastases was performed, while in a second case plaque therapy with 106 Ru and 125 I and a diode laser therapy were used. RESULTS: After therapy, one patient demonstrated partial regression of two choroidal metastases and total regression of one tumor, while the second one, showed total regression of all intraocular tumors. One patient developed irradiation neuropathy and retinopathy as a result of teleradiotherapy, which were the main cause of decreased visual acuity in her one eye. CONCLUSIONS: Regarding the good prognosis and potentially long survival of patients with bronchial carcinoid, prompt treatment of choroidal metastases is recommended. Applied method of therapy depends on tumor size, its localization and patient's general condition.

[Photodynamic therapy of circumscribed choroidal hemangioma].

PURPOSE: To evaluate efficacy of photodynamic therapy (PDT) with verteporfin in treatment of posterior pole symptomatic circumscribed choroidal hemangiomas. MATERIAL AND METHODS: Four patients (3 men and 1 woman), 15-55 years old (mean age: 29 years) with circumscribed choroidal hemangiomas of the posterior pole, were examined. The hemangiomas localization were subfoveal in one patient, extrafoveal in 2 cases, and juxtapapillar in one case. The diagnosis of choroidal hemangioma was established on the base of clinical examination, A and B--scan ultrasound imaging, Doppler ultrasonography, fluorescein and indocyanine angiography results. Tumour thickness before treatment ranged from 2.3-3.6 mm (mean thickness: 2.8 mm). One course of PDT with verteporfin was performed in 3 cases, while in one patient PDT was performed four times. RESULTS: In a follow-up period which range from 3 to 14 months (mean 5.7 months) all cases demonstrated clinical features of lesion regression with a reduction of thickness on ultrasonography and choroidal vasculature on angiograms. Visual acuity was improved in 3 patients (from 1 to 4 lines on Snellen charts), or remained stable in 1 case. CONCLUSIONS: Our preliminary results indicate that PDT with verteporfin is a safe and effective method of treatment for circumscribed choroidal hemangiomas, especially in patients with posterior pole location of the tumour.

[The analysis of I125 brachytherapy complications in cases of uveal melanoma].

PURPOSE: Analysis of type and frequency of complications after brachytherapy 1125 between 1-12 months after treatment and during the following controls. MATERIALS AND METHODS: 147 persons (76 men and 71 women) aged 23-86 years (mean 57 years), were treated in our clinic in 1998-2001. In 43% of the cases the thickness of the tumor was more than 8 mm and in 94% the maximum diameter was bigger than 8.1 mm. The irradiation dose was 80-120 Gy, mean 100 Gy. RESULTS: Regression and stabilization of the tumor was achieved in 84.3% of all cases. Complications of different type and intensity associated with brachytherapy I125 were observed in majority of the cases. The complications observed were: radiation retinopathy (52%), radiation maculopathy (33.6%), radiation optic neuropathy (25.3%), vitreous hemorrhage (8.8%), glaucoma (20.5%) and cataract (18.5%). CONCLUSIONS: 125I brachytherapy yields good results in cases of uveal melanoma, however it's associated with post radiation complications.

[Surgical management of iris melanoma].

PURPOSE: To describe the own experiences of surgical treatment of iris melanomas. MATERIAL AND METHODS: 56 patients with iris melanoma were treated between January 1995 and June 2001. Two surgical modalities were used depend on tumor location: partial iridectomy (in 45 cases) or transscleral iridotrabeculectomy (in 11 cases with the iridocorneal angle involvement). Additional ruthenium brachytherapy was performed in 3 cases. RESULTS: There were no any complications during surgery. Filtration bleb occurred in 1 patient during post-operative follow-up. Local recurrence of melanoma successfuly treated with ruthenium plaque appeared in one person. Histologically, the majority were spindle B melanomas (70%), mixed in 27% and epithelioid in 3%. The visual acuity was better or not changed in 90% of cases. CONCLUSIONS: Surgical management of iris melanoma seems to be save and effective treatment.

[Surgical management of ciliary body melanoma].

The aim was to report my own experiences of the surgical treatment of ciliary body melanoma. 42 patients were treated by trasnscleral resection of melanoma between January 1995 and June 2001. Additional rutenium brachytherapy was performed in 29 cases. Intraoperative complications included bleeding (3 cases), vitreous loss (4) and retinal detachment (2). Residual tumor in one case was the reason of enucleation. Cataract surgery and intraocular lens implantation was necessary in 7 cases. Postoperative filtration bleb appeared in 6 eyes. The evaluation of functional results has shown no impairment of visual acuity in 56% of cases and did not change during follow up. These observations show that transscleral resection with additional brachytherapy is a method of choice in the management of ciliary body melanoma.

[Visual acuity after surgical management of ciliary body melanoma].

The aim was to evaluate the visual acuity after transcleral resection of ciliary body melanoma. 42 patients were treated by trans-scleral resection of melanoma between January 1995 and June 2001. Additional rutenium brachytherapy was performed in 29 cases. Cataract surgery and intraocular lens implantation was necessary in 7 cases. Postoperative filtration bleb appeared in 6 eyes. The evaluation of functional results has shown no impairment of visual acuity in 56% of cases and did not change during follow up. These observations show that functional results after trans-scleral resection with additional brachytherapy are satisfactory and stable during follow-up.

[Surgical management of choroidal melanoma].

The aim of this paper, was to report my own experiences of the surgical treatment of choroidal melanoma. 10 patients treated by transscleral resection with additional rutenium brachytherapy between January 1995 and June 2001, were evaluated in the study. Intraoperative complications included bleeding (3 cases), vitreous loss (3) and residual tumor successfully treated with iodine brachytherapy in one case. Postoperative retinal detachment with painful hypotony in one case was the reason of enucleation. The evaluation of functional results has shown no impairment of visual acuity in 20% of cases and did not change during follow up. These observations encouraged us in our belief that transscleral resection is an alternative method of treatment of choroidal melanoma.

[Epidemiology of choroidal melanoma in the material of the Department of Ophthalmology CM UJ in Cracow].

PURPOSE: Epidemiological analysis of the choroidal melanoma cases-sent from all over the Poland and treated in our clinic between January 1998 and December 2002. MATERIAL AND METHODS: The study included 687 patients with choroidal melanoma 348 women and 339 men. RESULTS: The age of patients was 14-97 years (mean 56 years). The most numerous group consisted of patients between 60-70 years of age (22.7%). Most of the cases came from the regions of Malopolska, Silesia, Lower-Silesia and Mazovia. The time between first symptoms and the first visit in our clinic was between 1 week to 2 years. According to the COMS classification we found small melanoma in 13% of cases, medium in 51% and large in 36% of cases. We found the presence of other malignancies besides the choroidal melanoma in 4.3% patients. We analyzed the demography and positive family history of ocular and general malignancies in the patients.

The histology of pigmented Bomirski melanoma growing in the hamster eye--preliminary results.

We demonstrated that a quickly growing hamster skin melanoma developed a tumor after autologous implantation into the anterior chamber of the eye. Tumor cells were seen invading all the surrounding tissues, including the iris, ciliary body, choroid and cornea. Histological examination confirmed the presence of numerous blood vessels of large diameter. Their walls were very thin, thus only the endothelium could be identified using light microscopy. Macrophages, microemboli and extravasations were present within the tumor mass.

[Use of indocyanine green for photodynamic therapy of choroidal melanoma--preliminary report].

PURPOSE: To evaluate the usefulness of indocyanine green (ICG) in the photodynamic therapy (iPDT) of choroidal melanoma. MATERIAL AND METHODS: Thirty eight patients with choroidal melanoma were treated with iPDT. Tumor thickness ranged from 2.6 to 4.1 mm in ultrasonography. Before iPDT Ruthenium-106 plaques were used in 18 cases, in 6 cases with additional transpupillary thermotherapy ("sandwich method"), and Iodine-125 plaques in 3 cases. In 11 eyes the iPDT was the only performed treatment. Therapy was performed with 810 nm diode laser after intravenous injection of 25 mg solution of indocyanine green. Six months after iPDT ophthalmological examination was performed with ultrasonography and ICGA images evaluation. RESULTS: The baseline ICG study showed pathological intrinsic vasculature in all examined cases. Six months after iPDT changes in microcirculation, as well as significant decrease of tumors thickness in ultrasonography (mean 38%), were detected in all cases. Complete regression of intrinsic vessels was demonstrated by ICGA in 26 cases, and partial regression of pathological vascularization was found in 12 patients. CONCLUSIONS: Our preliminary study suggests, that ICG mediated iPDT can be a new and promising way, to treat choroidal melanoma.
