Cleaning of blood-contaminated reprocessed angiographic catheters and spinal needles.

OBJECTIVES: To evaluate the efficacy of a multistep cleaning method using a cleaner and a chemical disinfectant on blood-contaminated angiographic catheters and spinal needles intended to be sterilized by hydrogen peroxide gas plasma. METHOD: A mixture of radiopaque iodine contrast, bovine blood (plus ethylenediaminetetraacetic acid), and a suspension of Bacillus subtilis spores was used to simulate catheterization and needle use. The mixture was a 1:1 proportion of contrast and blood, inoculated so that there was a final concentration of B subtilis spores of 1.0x10(6) colony-forming units (CFU)/mL. The inoculated devices were cleaned using a hydrogen peroxide solution at a concentration of 1.5+/-0.5 percent by weight, followed by distilled water with enzymatic detergent. After drying, the devices were sterilized with hydrogen peroxide gas plasma. RESULTS: The initial B subtilis spore concentration inoculated into catheters and needles varied from 2.12x10(4) to 2.74x10(7) CFU/mL. The residual load of B. subtilis spores after cleaning varied from zero (no count) to a maximum of 200 CFU/device. The multistep cleaning procedure was responsible for an average 5log10 reduction of B. subtilis spores in the catheter and needle lumens. CONCLUSIONS: The hydrogen peroxide and enzymatic detergent aqueous solutions were shown to be efficacious when used as part of a multistep cleaning method. The low level of microbial contamination prior to sterilization with hydrogen peroxide gas plasma assured that the intended sterility assurance level was reached.

[Contribution of teaching organizations to the process of partnership between faculty and clinical nurses].

The rapid changes that are occurring in the societies have affected daily life of organizations that also need to equip and adapt themselves to correspond to this need. The first initiatives were related to the search for transformations in the organization structure and in the applied management model in order to form work teams, exchange and share information. The main purpose of this study is to point out five disciplines on the learning organization proposed by SENGE (1990) as a reference that can contribute to the process of faculty and clinical nursing partnership. Authors believe that continuous development and learning of those disciplines by nurses can enable the consolidation of this partnership between academic and clinical nursing institutions.

[Health care reform and changes in nursing practice in philanthropic hospitals in Ribeirão Preto (SP), Brazil].

This paper describes part of a multicenter study sponsored by the Pan American Health Organization to assess health care reforms and their implications for nursing in several countries. The objective of this research was to learn the views of nurses working in philanthropic hospitals in Ribeirão Preto, in the state of São Paulo, Brazil, regarding the changes in nursing practice coming from Brazil's health care reform and implementation of the Unified Health System (UHS). Data were obtained through structured interviews with seven nurses who met the selection criteria, from the three philanthropic hospitals in Ribeirão Preto. The nurses reported a decline in the quality of care and in the number of beds for UHS patients. The nurses reported that UHS implementation initially led to infrastructure improvements in the philanthropic hospitals. However, the reforms eventually shifted toward improving the care of private and privately insured patients. In addition, the nurses emphasized their heavy work loads and low pay. The nurses' reports indicated that Brazil's UHS is going through a crisis. In general, the nurses linked this crisis to problems in funding and allocation of resources.

The presterilization microbial load on used medical devices and the effectiveness of hydrogen peroxide gas plasma against Bacillus subtilis spores.

OBJECTIVES: To determine the microbial load found on used critical medical devices (5 spinal anesthesia needles, 21 catheters, and 28 sheaths) prior to sterilization and to evaluate the effectiveness of hydrogen peroxide gas plasma against inoculated Bacillus subtilis var globigii (American Type Culture Collection 9372) spores. METHODS: Membrane filter and pour-plate methods were applied to estimate total microbial loads (aerobic and anaerobic, mesophilic and thermophilic, vegetative and spore forms). Spinal anesthesia needles (102 units) and sheath components (61 units) were inoculated with a suspension of B. subtilis spores. After drying, the devices were sterilized with hydrogen peroxide gas plasma. RESULTS: Higher counts of aerobic, mesophilic, and fungal organisms were recovered when the drying period was insufficient. Anaerobic spores were not found in any analyzed presterilization items. The hydrogen peroxide gas plasma effected a 5 to 7 log10-fold reduction in B. subtilis spore counts in well-dried needles and sheath components. CONCLUSIONS: The success of hydrogen peroxide gas plasma sterilization depends mostly on educating the staff to assure well-cleaned and dried reusable medical devices, allowing penetration of the hydrogen peroxide gas plasma into the critical points of the items and providing a reduction in organisms.

[Phenomenology as a methodological alternative for research: some considerations].

The authors present the fundamental ideas of the phenomenology as an epistemology aiming human sciences which is concerned with the past experiences of the subjects and necessarily uses reflection as a way to consider the facts as they show up. As a methodological alternative for research, the phenomenology seeks the essence of the posed phenomenon and, to analyse its structure the investigator gets descriptions of the subjects who undergo the situation, aiming the formation of significant units. Hermeneutics is a kind of interpretation which must rely on a philosophical body of experience.

[Nurses' leadership in the perspective of postmodern ethics].

The administrative models arising from modernity are becoming increasingly obsolete in view of the needs of organizations, especially those which provide health care such as health institutions and their human capital. Since rationality is an essential element for the accomplishment of organizational objectives, modern ethics requires only that workers adjust to their prescribed functions and perform their tasks according to the rules. Post-modernity is characterized by saturation of information and consumption, pleasure of using goods and services, lack of values and meaning of life, reflecting a post-modern apathy. Without projects man's interior life has no substance. Health organizations are being strongly influenced by this context. Hence, ethics needs to be treated in a new way. Thus, the purpose of this paper is to approach nurse's leadership in light of post-modern ethics.

[Theoretical study of the demarcation of the intestinal stoma].

The present research focuses on the study of intestinal stoma demarcation. It reviews the historical development of the knowledge produced in this field, considering the advancement implemented in the area of abdominal surgeries which proposes the externalization of intestinal segments. In order to make the demarcation of intestinal stoma, it is necessary to limit the ideal region and to perform it with special pen which will propitiate the making of an anatomically appropriate opening. The stoma should favor the adaptation of devices which do the collection of effluents from the patient's intestines with a minimum of discomfort to the patient. The choice for the localization of the stoma, which is based on scientific principles, is made taking into consideration the structure of the recto-abdominal muscle and the distance from critical areas. At last, the most performed techniques of demarcation of the intestinal stoma are presented, as well as the procedures to be followed by the nurse or by the stoma therapist nurse.

[Evaluation of the quality of the process of stoma demarcation and late complications in ostomized patients].

Following the characterization of adults with an intestinal stoma who were registered in the Program of Education and Support to Stomized Patients, the authors developed a descriptive study in order to evaluate the results of the process of intestinal stoma demarcation as well as to analyze late complications. The sample was formed by 50 subjects, 76% of whom had colostomies and 34% had ileostomies. Regarding the stoma location, 12% did not demonstrate the scientific specifications indicated for the distance from the waist line, 2% for the iliac crest and 2% for the surgery scar. Late complications appeared in 44% of the cases, such as: 28% of peristomal hernias, 16% of stoma prolapses and 4% of skin hyperemias.

Bacillus stearothermophilus sporulation response to different composition media.

To evaluate the effectiveness of 11 commonly used ingredients to improve Bacillus stearothermophilus ATCC 7953 sporulation, with high spore yields in a short period of incubation, 32 composition media were set up by a fractional factorial 2IV11-6 design at two levels: D-glucose (0.018-0.25%), L-glutamic acid (0.040-0.10%), yeast extract (0.050-0.40%), peptone (0.30-0.50%), sodium chloride (0.001-1.0%), magnesium sulfate (0.001-0.20%), ammonium phosphate (0.010-0.035%), potassium phosphate monobasic (0.050-0.25%), calcium chloride (0.001-0.05%), ferrous sulfate (0.0003-0.002%), manganese sulfate (0.001-0.50%). The largest variation on Log10 CFU response took place due to sodium chloride main effect, by changing it from low to high levels. Magnesium sulfate, calcium chloride, and ferrous sulfate were split and exerted no detectable main effect influence on sporulation. Setting up two 16 runs for sodium chloride effect, in each of which the remainder levels were kept constant, other components contribution was studied. At low sodium chloride, best average 7.25 Log10 CFU yielded by fastening yeast extract and peptone at high level, and remainders at low level. Considering high level of sodium chloride, peptone, yeast extract and ammonium phosphate kept at high level and remainders at low level confirmed the best sporulation yield. Adjusted models evidenced a strong influence of joint yeast/peptone effect, associated to ammonium phosphate contributing positively. The reduced incubation period from 15 days to 3-6 days at 62 degrees C was attained for all 32 experimental runs.
