Congenital adenoma of the iris and ciliary body: light and electron microscopic observations.

A 23-year-old man had a lesion in the right inferior iris which appeared to have enlarged since it was first seen when the patient was aged 5 years. The lesion was excised by a partial iridocyclectomy. Histopathologically the neoplasm was composed of both pigmented and non-pigmented cells. Pseudoacini, containing acid mucopolysaccharides, were present throughout the tumour matrix. Electron microscopically the non-pigmented cells were found to possess a convoluted plasmalemma, abundant rough endoplasmic reticulum, and numerous desmosomes and gap junctions. The pigmented cells contained large, round, mature melanosomes, occasional premelanosomes, and desmosomes, which resembled the posterior pigment epithelium of the iris. The intercellular matrix contained fine collagen fibrils resembling vitreous. We believe that this neoplasm represents a congenital adenoma of the ciliary body and iris.

Expert opinions on nutrition issues in clinical dentistry.

Diet assessment and counseling are essential components of comprehensive preventive dental care programs. However, the lack of consensus on what constitutes appropriate nutrition services has resulted in diversity in the level and quality of such services to dental patients. Seventy nine nationally recognized experts in dental nutrition were surveyed using the Delphi method to seek consensus on the appropriate scope of nutrition in clinical dentistry.

Nutrition and the oral health of the elderly.

The dental problems of the elderly, though they may seem slight, can have far-reaching effects on both health and quality of life. The complexity and variety of potential physical and psychosocial problems of older citizens require a multidisciplinary management approach. Dental team members need to target appropriate resources outside of the dental office, and nondental care providers need to attend to dental needs as well as other more evident problems. Only in this way can we help to optimize the health and longevity of this ever-increasing segment of the population.

Major abdominal surgery in the over-eighties.

We report a prospective controlled trial looking at the outcome of major abdominal surgery judged on the ability of patients to return to their preoperative activity level. Eighty patients were included in the study. Forty of these were over 80 years old and the remainder in the age group 40-65 years. Fifty-four per cent of the over 80s demonstrated an increased level of dependance six months after surgery compared to 18% of the 40-65 year olds. The mortality rate at six months was 42% for the over 80 group. The poor prognosis of elderly surgical patients not only in their increased mortality rate but also in their dependency should be considered before they are subjected to major abdominal surgery.

Androgen receptors in skeletal muscle cytosol from sheep treated with trenbolone acetate.

Female sheep were implanted with the anabolic agent trenbolone acetate and the effects of the treatment on the skeletal muscle androgen receptor studied. In two separate trials the binding capacity/mg skeletal muscle protein was reduced. In trial 1 the animals were implanted (80 mg trenbolone acetate) at the start and after 5 weeks of the trial and samples taken one week later while in the second trial samples were taken 15 days after a single implant (80 mg trenbolone acetate). The ability of a variety of steroids and steroid-like compounds to displace [3H] testosterone and [3H] methyltrienolone was also studied. The androgen receptor was also partially purified using heparin-sepharose chromatography The involvement of the anddrogen receptor in the mode of action of trenbolone is discussed.

A modified immunofluorescence test for Epstein-Barr virus-specific IgM antibody.

The fluorescent antibody (FA) test for Epstein-Barr virus (EBV)-specific IgM antibody was improved by the use of sodium butyrate to induce a higher level of EBV antigen expression in P3HR-1 slide preparations and by removal of rheumatoid factor (RF) and IgG antibodies from test sera by means of adsorption with suspensions of Sepharose-IgG and Streptococcus pyogenes strain AR1. This method was compared with the Paul-Bunnell test (PB) on 1106 sera submitted to a routine virus diagnostic laboratory for infectious mononucleosis serology and 96.4% of sera showed concordant results. Thus the EBV-IgM-FA method was suitable for routine diagnostic use. However, it proved helpful to test EBV-IgM positive sera by PB to assist in the detection of cross-reacting IgM antibodies sometimes present.

Single-radial-hemolysis test for the assay of rubella antibody in antenatal, vaccinated, and rubella virus-infected patients.

The single-radial-hemolysis test (SRHT) is a simple, convenient, and sensitive method that provides accurate measurement of IgG antibody to rubella virus. Lipoprotein inhibitors of rubella hemagglutination do not interfere with the SRHT; thus the assay of low levels of antibody is more reliable by SRHT than by the rubella hemagglutination-inhibition test (RHIT). The technique for the SRHT in a form suitable for large numbers of tests is detailed and compared with that of the RHIT, and the reproducibility of the SRHT in routine operation is assessed. Results obtained by testing various populations in Western Australia are used to show the application of the SRHT in the diagnosis of rubella and in the monitoring of rubella vaccination programs.
