Resection and anastomosis of a cervical vertebral artery aneurysm.

A patient with a fusiform aneurysm of the cervical vertebral artery at C-1 presenting with posterior circulation infarctions is reported. Exposure via the posterolateral approach, resection of the aneurysm, and end-to-end anastomosis are described. The authors emphasize the importance of the surgical approach and ability to expose and mobilize a generous portion of the cervical vertebral artery.

Visual loss from cavernous hemangiomas of the middle cranial fossa.

We studied a case of cavernous hemangioma of the middle cranial fossa. This rare tumor, of which only nine prior cases have been recorded, is characterized by insidious visual loss, multiple cranial nerve pareses, proptosis, papilledema, and bitemporal hemianopsia. Pituitary dysfunction has been noted in the more advanced cases. The female-male ratio has been 8:2; ages ranged from 30 to 62 years. The hemangiomas may occur on either side. Contrast-enhanced computed tomography can clearly demonstrate the presence of these lesions, but is not diagnostically specific. High-quality angiograms using magnification and subtraction techniques may show areas of dye retention in the cavernous spaces of the lesion. This tumor is amenable to successful excision, and early removal may prevent progressive visual loss.

A comparative study of performance characteristics of cerebral aneurysm clips.

The authors have developed a procedure to evaluate the performance of cerebral aneurysm clips as a function of intravascular pressure and aneurysm neck diameter. Numerous models of Mayfield, Scoville, Variangle, Heifetz and Yasargil clips were tested. The various clips demonstrated decreasing capabilities for secure occlusion of a vessel as vascular diameters and intravascular pressure increased. These findings are in record with predictions based upon La Place's Principle and indicate that the size of an aneurysm must be considered in the choice of an appropriate aneurysm clip.

Glossopharyngeal schwannomas.

Glossopharyngeal schwannomas are rare entities. Our recent experience with three such cases treated successfully, suggests that recognitton of this tumor as a discrete entity is both desirable and feasible. Although this tumor shares with the far more common acoustic schwannoma the presenting symptom of hearing loss, it may be distinguished by an elicitable history of hoarseness, demonstration of abnormal palatal function and absence of expected findings upons standard radiographic examination of the petrous bones and internal auditory canal. We have found that identification of this tumor may be accomplished with currently available diagnostic technics, including computerized tomography. The authors review the literature and report their experiences regarding preoperative assessment, operative technic and the postoperative course.

Some fundamental applications of the Law of La Place in Neurosurgery.

The Law of La Place is a simple principle of physics, applicable to any fluid-filled vessel, relating stress on the wall of the vessel to the radius and transmural pressure. This principle has major implications for neurosurgery and examples are provided relating La Place's Principle to the genesis and treatment of hydrocephalus, the surgical clipping of cerebral aneurysms and the technic of surgical sponge hemostatis.
