Nursing management of soft tissue sarcomas of the extremities.

Soft tissue sarcomas of the extremities are uncommon malignancies that require combined modality treatment. Treatments include extensive surgical procedures, usually radiation therapy, and, in some cases, chemotherapy. Nursing care requires an understanding of surgical and reconstructive procedures, brachytherapy implants, radiation safety regulations, and chemotherapeutic agents. The combination of extensive surgery and radiation has greatly reduced the need for amputation, but the complexity of the treatments requires skilled and knowledgeable nursing care.

Radiation protection for nurses. Regulations and guidelines.

Rules and regulations of federal agencies and state radiation protection programs provide the bases for hospital policy regarding radiation safety for nurses. Nursing administrators should work with the radiation safety officer at their institutions to ensure that radiation exposures to staff nurses will be as low as reasonably achievable and that special consideration will be given to pregnant nurses. Nurses' fears about their exposure to radiation can be greatly reduced through education.

Radiation exposure of nurses in a coronary care unit.

In response to increasing awareness of radiation as a possible occupational hazard, nursing personnel staffing a hospital CCU were monitored over a 3-year period to determine occupational exposure. Portable x-ray machines, fluoroscopic units, and patients injected with radiopharmaceuticals were all potential radiation sources on such a unit. Whole-body TLD badges, exchanged monthly, indicated no cumulative exposures over 80 mR during the entire study period. The minimal exposures reported do not justify regular use of dosimeters. Adherence to standard protective measures precludes most exposure to machine-produced radiation. Close, prolonged contact with a patient after an RVG study that utilizes 99mTc may account for some exposure. The data indicate that radiation is not a significant occupational hazard for CCU nurses at this hospital; similar minimal exposures would be expected of other nonoccupationally exposed nursing personnel in like environments.

Elevation of serum uric acid as a clue to alcohol abuse.

Hyperuricemia in unselected admissions to a general hospital was found to be a significant correlate of alcohol abuse in male subjects. Unexplained elevation of the serum uric acid level should always stimulate inquiry for the behavioral features of alcoholism.

A sex difference in the serum vitamin B-12 levels of hospitalized alcoholics.

The relationship of elevated serum vitamin B-12 concentration to alcoholism in general hospital patients was studied both in a retrospective and in a prospective group of patients. Alcoholism was the most common disease correlate of vitamin B-12 elevations in females. Of the unselected female alcoholics, 61% had a serum vitamin B-12 concentration larger than or equal to 1000 ng/liter at the time of admission; only 17% of the nonalcoholics, whatever their other disease, exhibited such high values. The sensitivity of high B-12 levels for alcoholism in females paralleled that of high SGOT, GGT and MCV values. In contrast, males (approximately 70% of our alcoholics) far less often exhibited B-12 elevations even when SGOT, GGT and MCV were abnormally high. Elevation of serum B-12 concentration should always raise the suspicion of alcohol abuse, especially in females.

Diagnostic correlates of discharge against medical advice.

The clinical histories of patients discharged against medical advice from a general hospital were studied to discern important diagnostic correlates of this event. Drug addiction occurred in 22% of the 73 test subjects and in none of 175 random controls. Chronic alcoholism was diagnosed in 42% of patients discharged prematurely vs 15% of controls (P less than .01). Discharge against advice should be considered a clue to the presence of one of these conditions if not previously appreciated. Since both are identifiable chronic disorders, early counseling is recommended to prevent such noncompliant behavior and its consequences for effective hospital care.
