Severe ovarian hyperstimulation syndrome is a rare complication of assisted reproductive technologies. It presents with massive ovarian enlargement, fluid accumulation in peritoneal, pleural cavities and thromboembolic diseases. The authors report an exceptional case of internal jugular vein thrombosis in an ovarian hyperstimulation syndrome. Pathogenesis of this event is discussed.

We report the concomitant occurrence of erythema elevatum diutinum and specific skin lesions in a patient with a myelodysplastic syndrome (MDS). This patient's course, and review of other reported cases, support the opinion that neutrophilic dermatoses are associated with a poor prognosis of MDS. The simultaneous appearance of these manifestations could be the consequence of a particular chemotactism of myeloid cells, expressed after acute transformation.

Deep cutaneous lesions are seldom encountered in lupus erythematosus. The disease described in the literature as lupus erythematosus profundus or lupus erythematosus panniculitis usually occurs in middle-aged women. The authors report a case of deep lupus erythematosus which was exceptional in that it occurred in an 11-year old girl. The lesions were situated on the face and consisted of solid subcutaneous nodules and clear-cut ulcerations leaving atrophic pigmented scars. The histological image of a nodule was one of lobular lymphocytic panniculitis with homogeneous hyalinization of adipose nodules, to which must be added periadnexal and perivascular dermo-epidermal lymphocytic infiltrates. Granular IgM deposits arranged along the dermo-epidermal junction were observed at direct cutaneous immunofluorescence. Laboratory examinations showed leucopenia (3,300/mm3) with neutropenia (1,100/mm3) and the presence of antinuclear antibodies at 1/100 speckled fluorescence, as well as antibodies directed against native DNA. Studies of renal function and for complement gave normal results. The other causes of lobular panniculitis were excluded. The lesions regressed within 3 weeks under hydroxychloroquine; this drug was also successful in arresting a relapse consecutive to withdrawal of treatment. The authors have analysed the 17 paediatric cases of deep lupus erythematosus and were able to determine their main characteristics: 1. The lesions occur mostly in girls (70 p. 100). They are located electively on the face and the lateral aspect of the shoulders. They consist of well-limited, solid or hard subcutaneous nodule which may congregate to form plaques. The epidermis may be normal or pathological, poikilodermic, looking like a discoid or ulcerated lupus erythematosus. The lesions regress, leaving a characteristic atrophic scar.(ABSTRACT TRUNCATED AT 250 WORDS)
The case of a sixty-two year old man with Campylobacter fetus spondylodiscitis is reported. Despite of adequate antibiotherapy, the outcome was bad and death occurred. Three other cases of Campylobacter fetus spondylodiscitis are reported in the literature. Tomodensitometry disclosed a paravertebral abscess with gas, that make our observation original.

INTRODUCTION: Cutaneous squamous cell carcinoma is an invasive and destructive tumor, and may cause death by local extension or because of metastasis. We report the case of a patient with a giant squamous cell carcinoma of the nose and extension to the brain and discuss the main risk factors from this extension. OBSERVATION: Resection of a giant squamous cell carcinoma was performed of the nose in a 45 year-old man after debulking radiotherapy. Histological examination disclosed a well-differentiated tumor and perineural involvement, with at least a 6 mm margin. A first relapse occurred on the orbital edge of the initial resection, the lesion was removed revealing an involvement of the infra-orbital nerve. Whilst the patient was receiving chemotherapy, a second relapse occurred responsible for ophthalmoplegia and loss of vision, with involvement of the left orbital apex, cavernous sinus and temporal lobe. The patient died from grand mal seizures. DISCUSSION: Brain extension of cutaneous squamous cell carcinoma of the head is rare. It develops along the anatomic pathways, especially perineural spread. Main risk factors for such a poor course are discussed, including the size of tumor, the anatomic site, the depth and perineural invasion.

A 36-year-old man with testicular cancer had an acute myocardial infarction during the first course of chemotherapy with bleomycin, etoposide and cisplatin. Since the patient had no significant risk factors for coronary heart disease, the infarction was likely to be attributable to the chemotherapy regimen. The physiopathological mechanisms of this causal relationship are discussed here.

BACKGROUND: The diagnostic and prognostic value of specific cutaneous lesions in acute leukemia is well-known. Paradoxically, these lesions may initially develop without peripheral blood or bone marrow involvement. We report the case of a patient with cutaneous lesions of acute monoblastic leukemia whereas peripheral blood was normal and massive infiltration of dermis was demonstrated. OBSERVATION: A 49 year-old man had papules and nodules of the back and upper arms evolving for several months. Histological examination with appropriate immunostaining led to the diagnosis of specific cutaneous lesions of acute monoblastic leukemia. Several hemograms with peripheral blood smears were normal, bone marrow smear demonstrated an important blastic infiltration on one site and a discrete infiltration on another. Cutaneous lesions disappeared with chemotherapy. DISCUSSION: Specific cutaneous lesions may be isolated during acute leukemia, and called aleukemic leukemia cutis. These are a rare form, the underlying mechanism of which relies on the accumulation of small quantities of myeloblasts in bone narrow and with high tropism for the dermis.
99mTc-HMPAO leukocyte scintigraphy was prospectively studied in 20 patients with Crohn's Disease and was compared with radio-endoscopy and with disease activity index (Crohn's Disease Activity Index, ESR, C-reactive protein). Disease localization as defined by the scan was well correlated with those visualized at endoscopy or radiography (rs = 0.90 - p = 0.0001). A scan score was calculated by evaluating uptake of the tracer in 6 bowel segments with lumbar bone marrow activity, correlation with endoscopic score was strong (rs = 0.96 - p < 0.0001). Scan score was well correlated with the Crohn's Disease Activity Index (rs = 0.63 - p = 0.0026), but no correlation was found between ESR, C-reactive protein and scan score. In four patients with ileal localization, scintigraphy was helpful to separate inflammatory from scared stenosis. Scintigraphy with 99mTc-HMPAO appeared to be useful in evaluation of Crohn's Disease activity, and for the diagnosis of complications as stenosis.
A patient with chronic tophaceous gouty arthritis developed paraparesis due to medullar compression from a urate sodium tophus. We report the MRI features of this tophaceous gout, which showed an extradural hyperintense signal with an isointense nodule signal on T1 weighted sequences. Surgical removal of the epidural lesion resulted in complete clinical recovery. At the upper level of the spine, an extension of the epidural lesion was not removed, but after one year of medical treatment of the gout, MRI showed regression of the epidural compression.
