[Diagnosis by imaging in renal and urinary tract malformations. Comparison of echography and traditional radiological studies].

The diagnostic sensitivity of Ultrasound (US) was studied in 142 children with suspected kidney and urinary tract malformations. According to the clinical tests performed the patients underwent excretory urography (EU) and/or voiding cystouretrography (VCU); the results were compared to US findings. In the 75 patients with malformations, US proved to be extremely sensitive in abdominal renal ectopies, in "horseshoe" kidney, and in congenital obstructions of the ureteropelvic and vesico-ureteral junctions. US showed a higher sensitivity than EU in identifying multicystic kidney and in most cases of hypodysplasia. On the other hand, VCU was more accurate in vesico-ureteral reflux studies; US should thus be used in the follow-up of the patients undergoing medical therapy. EU must however be considered as the most important tool in the evaluation of early renal injuries and their possible development.

Arteriovenous fistula associated with renal cell carcinoma: demonstration by magnetic resonance imaging.

Two cases of arteriovenous fistula associated with renal cell carcinoma were studied using multiple techniques, including magnetic resonance imaging (MR). These techniques clearly delineated the vascular abnormalities without the use of a contrast agent; they also demonstrated the extent of the tumors. Data derived from MR were confirmed upon surgical exploration and were useful in planning the approach to the kidney.

[Methods for studying the heart with magnetic resonance].

Magnetic resonance imaging is a new completely non-invasive diagnostic technique now available even for heart studies. Due to the long time of this examination the main goal for the application to the cardiovascular diagnosis is to obtain the major number of information in a reasonable time for the patient. On the basis of our initial experience with this technique we propose a method suitable for the morphological, functional and tissular characterization in cardiovascular diagnosis.

[Cardiac imaging with nuclear magnetic resonance. Preliminary experience].

Magnetic Resonance Imaging (MRI) is a new completely noninvasive diagnostic tool available for studying the cardiovascular system. In order to assess its utility in cardiovascular protonic imaging, we studied normal individuals and patients with different heart diseases by means of a 0.5 Tesla superconductive magnet triggered by the ECG. Images were obtained in 38 subjects. By electronically angulating the gradients and by acquiring the images at different phases of the cardiac cycle we obtained several different diagnostic possibilities which can allow both anatomic and functional studies of the heart. On the 21 patients, MRI showed the main features of each disease. Our initial experience with MRI confirms the need for oblique images of the heart in order to obtain planes useful in cardiovascular diagnosis. Among many diagnostic possibilities we tested the ability of MRI in the detection of acute and old myocardial infarction. Although MRI requires extensive application in order to delineate its role in cardiovascular diagnosis it is to be considered an important diagnostic tool and a very interesting investigative method.

[Magnetic resonance and computerized tomography in the study of hypophyseal adenomas. Comparative results].

Magnetic resonance evaluation of 28 cases of pituitary adenomas has shown remarkable accuracy. Compared with HR-CT, MR gives comparable results in tumour identification. MR better demonstrates the suprasellar extension of macroadenomas and their relationship to the visual pathway and is more effective in showing direct and indirect signs of microadenomas. HR-CT however better recognized bone abnormalities of the sella turcica, due to adenomas. A typical increased signal intensity has been demonstrated in most of the adenomas studied.

[Cystic dilatation of the bile ducts : value of various methods of diagnosis. Apropos of 3 cases].

Three cases of cystic dilatation of the biliary tract of 1st, 4th and 5th type are reported here according to the classification established by Longmire and Hadad amended. In two patients the diagnosis was carried out through ultrasound examination and confirmed, in one case, by intraveinous cholangiography associated to computed tomography (and by peroperatory cholangiogram in the other one). In the third patient suffering from Caroli's disease the diagnosis was carried out during peroperating cholangiography. The possibilities of the different methods in diagnosing cystic dilatations of the biliary tract are discussed and therapeutic problems are taken into consideration.

The role of ultrasonography and ultrasound guided biopsy in the management of patients with cold nodules of the thyroid.

The histological diagnoses of surgical specimens from 86 patients with cold nodules of the thyroid (55 goitres, 16 adenomas and 15 malignancies) were compared with the clinical data, ultrasonographic appearances and the results of the fine needle biopsy of each patient to evaluate the role of ultrasonography alone and combined with guided biopsy in distinguishing patients with malignancies from patients without malignancies. The Authors suggest that ultrasonography alone cannot be used for distinguishing malignancies from non-malignancies in cold nodules of the thyroid; the combined use of the ultrasonographic appearances with guided biopsy offers a remarkable diagnostic gain in this field. In the Authors' case material the detection of a simple cyst by ultrasound always excluded the possibility of malignancy.

Ultrasound and computer tomography in the evaluation of focal liver disease.

The results of ultrasound and computer tomography in 150 patients with possible focal liver disease were compared. Ultrasound was more sensitive than CT in detecting lesions but similar information on the nature of the lesions was obtained with both methods. CT provided more complete information than US on the extent of the lesion. Thus, CT and US are supplementary in the evaluation of focal liver disease. Because of the higher sensitivity US should be the first examination, followed, if clinically indicated, by CT confirmation and possible further information regarding the extent of the lesion.

After an anatomical and anatomo-radiological introduction, the authors describe the morphological aspect of the thoracic duct examined by means of lymphangiography performed during computed tomographic examinations.

[Delayed post-traumatic stenosis of the small bowel: report of a case (author's transl)].

The authors present a rare case of delayed post-traumatic stenosis of the small intestine localized in the mid jejunum, due to blunt abdominal trauma. Several physiopathological and radiodiagnostic problems are reviewed. The lesion belongs, in our opinion, to the wide group o ischaemic stenoses of the digestive tract and is interpreted as the effect of vascular damage to the mesentery with the formation of an intramural haematoma.

[Multi-institution, multidisciplinary radiology and endoscopy trial of the diagnostic accuracy of the double contrast examination of the stomach surface].

In 12 X-ray and gastroenterology departments 1570 patients, clinically suspected to have a gastric pathology, have been examined by double contrast (surface) examination of the stomach followed by gastroscopy. Diagnostic accuracy was 93% in respect of endoscopic data, assumed correct by hypothesis. On the basis of a cost and benefit evaluation, made according to the well known criteria, a wider routine use of the double contrast technique is proposed.

Transcatheter arterial embolization followed by percutaneous ethanol injection in the treatment of hepatocellular carcinoma.

PURPOSE: The aim of this study was to evaluate the effectiveness of transcatheter arterial embolization (TAE) followed by percutaneous ethanol injection (PEI) in the treatment of large hepatocellular carcinoma (HCC) lesions. METHODS: Fifteen patients with HCC were treated by means of TAE followed by 6-16 ethanol injections. In 10 patients, the HCC was solitary (3-8 cm); 3 patients had 1, and 2 patients had 2 daughter nodules (3 cm or smaller) in addition. In 12 of 15 main tumors and in 4 of 7 daughter nodules, a tumor capsule was observed by computed tomography or magnetic resonance imaging. RESULTS: Combined treatment with TAE and PEI resulted in complete necrosis of 12 of 15 main tumors and 7 of 7 daughter nodules on biopsy. Treatment failure (incomplete necrosis) occurred in 3 unencapsulated main tumors. The 1-year survival rate in 10 patients was 100%. CONCLUSION: The combination of TAE and PEI proved to be an effective treatment for large HCC, including those with 1-2 small daughter nodules. The presence of a tumor capsule significantly correlates (p < 0.05) with a favorable outcome of treatment.

Percutaneous biopsy of liver tumors with color Doppler US guidance.

BACKGROUND: Percutaneous liver biopsy is a well-established procedure. Aim of this study was to investigate the usefulness of color Doppler ultrasound (US) for guiding percutaneous biopsy of liver tumors in patients at high risk of bleeding. METHODS: Forty-seven patients with severe, uncorrectable coagulopathy (platelet count < 50 x 10(9)/L and/or prothrombin time ratio [normal/patient] < 50%) were submitted to percutaneous biopsy of focal liver lesions using a 21 G cutting needle. The procedure was guided by color Doppler US, avoiding the needle cross into any liver or tumor vessel. RESULTS: Biopsy material was adequate for pathological reading in 46/47 cases (97.9%). No major post-biopsy complication was observed. In particular, no case of hemoperitoneum was detected on US examinations performed 2 hours after biopsy. Minor complications (including one subcapsular hematoma) occurred in 4 patients (8.5%). CONCLUSIONS: Color Doppler US can safely guide percutaneous liver biopsy in patients at high-risk of bleeding.
