Scintigraphic evaluation of childhood malignancies by 67Ga-citrate.

The utility and limitations of 67Ga scintigraphy in children with solid tumors were evaluated. Thirty-five patients with malignancies (13 lymphoreticular neoplasms, 11 soft-tissue sarcomas, 8 neuroblastomas, and 3 primary bone tumors) had a 67Ga-citrate scan as part of their clinical evaluation. The sensitivity and specificity of the test were analyzed for the different tumor types. The overall sensitivity of the 67Ga-citrate scan for the lymphoma group was 87%. Higher values were obtained for the mediastinal and abdominal regions. Ninety-three per cent of the involved sites were correctly identified by 67Ga scintigraphy in the soft-tissue sarcoma group. Small lung metastases, however, were missed on scan. Thus, 67Ga scans should be complemented with chest radiographs and whole chest tomograms for both initial evaluation and follow-up in those patients. 67Ga had low sensitivity for neuroblastoma.

Diagnostic significance of persistent colonic gallium activity: scintigraphic patterns.

One hundred and forty-eight gallium scans were reviewed to determine the diagnostic significance of colonic gallium activity and its relationship to colonic disease. Location or intensity of the activity was unchanged during the course of the radionuclide study in 25 patients imaged more than once. Of those patients who had colonic gallium activity that did not change in location or intensity during the study, 9 had proved colonic disease. The patterns of colonic activity were either focal or diffuse. These distinct patterns, however, cannot distinguish benign from malignant disease.

Scintigraphic evaluation of lymphoma: a comparative study of 67Ga-citrate and 111In-Bleomycin.

Sequential whole-body scans with both 67Ga-citrate and 111In-bleomycin were performed for 14 patients undergoing staging for Hodgkin's disease or other lymphomas. A comparison or these scans gave a sensitivity in detecting the pathologically-proved tumor sites of 84% for 67Ga-citrate and only 56% for 111In-bleomycin. In 12 patients, the gallium scan demonstrated the lesions more clearly; in the others, the uptake of the two radionuclides was equal. In no case was a clinically diagnosed or histologically proved lesion identified by 111In-bleomycin that was not also demonstrated by 67Ga-citrate. 111In-bleomycin does not complement the 67Ga-citrate scan in the scintigraphic evaluation of lymphomas.

The reflex sympathetic dystrophy syndrome. II. Roentgenographic and scintigraphic evidence of bilaterality and of periarticular accentuation.

Patchy osteoporosis is the primary roentgenologic manifestation of the reflex sympathetic dystrophy syndrome (RSDS). As recent clinical and histologic data suggested articular changes in RSDS, fine-detail roentgenograms were obtained in eight consecutive patients. Juxta-articular and soft-tissue swelling, osteoporosis and erosions of the subchondral bone were found. 99mTcO4 and 99mTc-EHDP scintigraphy showed localization of nuclide predominantly in the juxta-articular tissues. Serial roentgenographic, scintigraphic and quantitative bone densitometric measurements showed changes that reflected the clinical course of the disease.

A unified radiological approach to the detection of skeletal metastases.

Radiographic bone surveys and radionuclide images, the principal methods of detection of skeletal metastases, have traditionally been treated as separate entities. Since their interpretation is reltaed, we attempted to determine the practicability of a unified radiological approach which employs both methods. All consultation requests for "skeletal surveys" or bone scans were reviewed, and an appropriate examination was "tailored" for each patient. During the inital four-month period, 141 patients were examined. Comparison of the final "tailored" examination with the original consultation request indicated that, without radiological guidance, excessive (18%), and inappropriate or inadequate (27%) examinations were ordered in many cases. The unified approach has proven to be feasible and desirable, resulting in a significant reduction of inappropriate examinations.

Radionuclide imaging of the bones and joints of the hand. A definition of normal and a comparison of sensitivity using 99mTc-pertechnetate and 99mTc-diphosphonate.

Normal radionuclide patterns of the hand obtained with 99mTcO4 and 99mTc-Sn-EHDP are described and their validity supported by clinical, radiographic, and laboratory examinations performed to exclude articular disease in the control group. Comparison of the 99mTcO4 and 99mTc-Sn-EHDP images in patients with various articular diseases demonstrated higher sensitivity of 99mTc-Sn-EHDP in detection of involved joints; this was also true when the radionuclide images were compared with fine-detail radiographs in patients with rheumatoid arthritis.

Salivary gland uptake of 67Ga-citrate following radiation therapy.

Radiogallium uptake in the salivary glands is strongly correlated with prior therapeutic radiation which includes the cervical region. Fifteen of 17 patients with increased salivary gland uptake had such a history. An easily identified characteristic pattern of symmetric localization of 67Ga occurs in the parotid and submandibular regions in these patients. This pattern must not be mistaken for recurrent tumor in the cervical region.

Gallium-67 scanning for carcinoma of the lung.

Gallium-67 citrate scanning was prospectively evaluated in 55 patients who had lung lesions suspected to be primary carcinoma on chest x-ray films and in whom subsequent histologic diagnosis was obtained. Of 47 patients with histologically proved carcinoma of the lung, 44 (94 per cent) had a positive 67Ga scan. No patient with a positive scan had a benign lesion, so that the positive scan accuracy rate was 100 per cent. All 8 patients with a benign lesion and 3 patients with a malignant lesion had negative scans, for a negative scan accurary rate of 72 per cent for benign lesions. These results give statistical validity for the usefulness of the 67Ga scan in diagnosing carcinoma of the lung (p less than 0.001). Tumor cell type had little effect on the sensitivity of 67Ga scan. The 67Ga scan was equally useful in the evaluation of peripheral and central lesions. There was little difference amount T1, T2, and T3 classified lesions in their ability to take up 67Ga. The 67Ga scan was competitive with mediastinoscopy in assessing mediastinal lymph node metastases and provides a noninvasive method of assessing hilar lymph node metastases. There was a good correlation between the clinical staging of patients with lung cancer based on a chest x-ray film and 67Ga scanning and the staging after surgical treatment based on the histology of the resected specimens.

Renal localization of 67Ga-citrate in renal amyloidosis: case reports.

In scans taken 72 hr after intravenous administration of 5 mCi of 67Ga-citrate, both kidneys were clearly visible in two cases of histologically proven renal amyloidosis. Neither patient had clinical manifestations or laboratory evidence of concurrent inflammatory disease or tumor involving the kidneys. Increased renal concentration of lysosomal organelles and increased affinity of 67Ga for amyloid material contained in the organelles could explain the renal uptake of 67Ga in amyloidosis.

The reflex sympathetic dystrophy syndrome. A comprehensive analysis using fine-detail radiography, photon absorptiometry, and bone and joint scintigraphy.

Nine patients with reflex sympathetic dystrophy were examined. Clinical manifestations suggesting arthropathy were supported by radiographic demonstration of juxta-articular and subchondral bone erosions and by radionuclide demonstration of increased activity localized in the joint regions. Aggressive demineralization was demonstrated by fine-detail radiography and consisted of endosteal and intracortical excavation and subperiosteal and trabecular bone resorption. A one-third reduction in bone mineral was confirmed by quantitative analyses. Newer modalities of study have aided in the documentation of arthropathy in reflex sympathetic dystrophy and have helped in defining the patterns of aggressive bone resorption.
