[Quality controls for the gamma camera with single photon emission tomography].

Some tests and quality controls to assess the initial conditions and the working performance of a SPET system (Single Photon Emission Tomography), are described. Suitable tomographic phantoms were designed and built. We outline the methods to check MTF, stability of the photoelectric peak, centre of rotation, spatial uniformity, spatial resolution, sensitivity. NEMA tests should first be carried out to check the planar scintigraphic performance of the gamma-camera. This study allows to comprehensively evaluate the qualitative and quantitative performance of the SPET system and the correct check frequency of investigated parameters.

[A new sterile generator of Tc 99m. Quality control].

Production has started on a new Tc 99m sterile generator in Italy. We give the results of a series of quality controls in order to have a first evaluation of the product. The elution profile and the volume, activity, pH, aluminium concentration, 99 Mo content and radiochemical purity of the eluate were examined. Radiochemical purity tests were also carried out with some of the more widely used "in vivo" radiopharmaceuticals. The results are in line with the Official Pharmacopeia and good technical standards.

Human platelets 5'-nucleotidase: a cell membrane ectoenzyme with a possible regulatory role in the aggregation reaction.

The existence of a 5'-nucleotidase has been demonstrated in human blood platelets. The enzyme has a low Km (20 microM) for adenosine monophosphate. It is prevalently located on the external surface of the plasma membrane as demonstrated by the similar degradation of exogenous AMP by intact and lysed platelets. Also results of inactivation studies by means of non penetrating chemical reagents point to this conclusion. Activity is inhibited by glucosyl moieties specific lectins (e.g. Concanavalin A) with varying sensitivity in intact platelets, isolated membranes and in the solubilized form. Also micromolar concentrations of ADP inhibit the activity, possibly with a competitive pattern since this effect is much more evident at low substrate concentrations. On the basis of these results, it is suggested that this enzyme is involved in in loco adenosine production in the platelets, a process which can assume a physiological important role.

[Analysis of the use of disinfectants in Italy particularly in Southern Italy].

In an investigation of the use of disinfectants in hospitals, the replies received from 338 institutions were analysed and subsequently compared with those obtained from southern Italy. It was found that benzalkonium chloride was the substance most widely used in Italy for disinfection of the surgical field, wounds, excoriations, sores, the skin prior to injections, surgical instruments, medical apparatus, catheters, thermometers and sanitary appliances, whereas chlorhexidine was preferred for disinfection of the hands, and for ambient surfaces in combination with cetrimide. Formic aldehyde was preferred for ambient surfaces, the air of confined spaces, and, in conjunction with chlorine and its derivatives, for sanitary services and their vessels. Glutaraldehyde was the substance most commonly employed for the disinfection of medical apparatus fitted with optic fibre lenses. In southern Italy, by contrast with the position in Italy as a whole, there was a greater use of chlorine and its derivatives for the disinfection of ambient surfaces, benzalkonium chloride for the disinfection and cleaning of sanitary services, vessels, and equipment with optic fibres, and alcohols for the disinfection of surgical instruments and sanitary appliances.

[The use of disinfectants in Northern Italy with special attention to the Emilia-Romagna region].

As part of an extensive investigation into the use of disinfectants in hospitals, data from 189 hospitals in the north of Italy, including 38 in the Emilia-Romagna region, were examined. Eleven fields of employment were assessed. It was found that benzalkonium chloride was primarily used for disinfection of the operative field, wounds, excoriations, sores, the skin prior to injection, surgical instruments, catheters, thermometers, and utensils. Chlorhexidine was preferred to disinfection of the hands and medical equipment. Formaldehyde was preferred for ambient surfaces, the air of confined spaces, and (in association with chlorine and its derivatives) for sanitary services and vessels. Glutaric aldehyde was preferred for apparatus fitted with lenses or optical fibres. By comparison with other parts of N. Italy. Emilia-Romagna made greater use of hydrogen peroxide for skin lesions, and alcohol for disinfection prior to transcutaneous injections.

Effect of pH on the kinetic properties of rat skeletal muscle AMP deaminase.

1. The optimal pH for activity of rat skeletal muscle AMP deaminase depends on substrate and salt concentrations. 2. At pH 7.12, differently from what is observed at acidic pH, the sigmoid kinetics shown by the enzyme in the absence of salt are not reversed to a hyperbolic one by increasing KCl concentration. 3. At alkaline pH the enzyme is also more sensitive to inhibition by nucleoside triphosphates, which enhance the sigmoidicity of the substrate saturation plot. At acidic pH, ATP elicits negative cooperativity for substrate and the same phenomenon is induced by high salt concentration. 4. The different properties of the enzyme at acidic and alkaline pH suggest that AMP deaminase can exist in either of two different conformations; at physiological pH the less active form of the enzyme predominates.

[Primary antiphospholipid antibody syndrome: clinical features and analysis of personal cases].

We studied 11 patients with primary antiphospholipid syndrome (PAPS), basing our diagnosis on Harris's clinical and serological criteria. Analysis of these cases brought to light a number of symptoms that have a statistically significant association with PAPS. Although these symptoms have not yet been recognized internationally as diagnostic criteria, we anticipate that they will be included on a revised and lengthened list of clinical symptoms used for the diagnosis of PAPS. There was a high incidence of neurological symptoms in our case group: disorders were often observed before the onset of full-blown disease and could be confirmed by means of cerebral NMR and SPECT analysis. We believe that further investigation will enable researchers to determine the predictive value of these PAPS-related neurological disorders.
