[Risk of recurrence following partial thyroidectomy for benign lesions. Report of 58 patients 15-25 years after surgery].

BACKGROUND: Since the 1980's, thanks to the introduction of new techniques and methods for the study of thyroid function, new light has been shed on certain aspects of thyroid disease that constitute the basis for surgical therapy. Multinodular goiter is a disorder affecting the entire gland, even when alterations mostly or exclusively involve a single lobe. The functional deficit resulting from partial or subtotal resection has a hypertophic/hyperplastic effect on the remaining parenchyma. Partial resections often lead to postoperative hypothyroidism, usually of a subclinical nature, which implies difficult management by means of replacement therapy. METHODS: The authors report a series of 58 patients who underwent partial thyroidectomy to treat benign thyroid disease at the 2nd Surgical Department of the University of Florence during the period 1975-1985. RESULTS: Of these patients, operated on more than 15 years ago, 36.2% - the majority of whom (87.5%) have constantly been on substitutive therapy using L-thyroxin - currently shows no alteration of the remaining parenchyma. Conversely, in 60.3% of the patients there was evidence of nodular/pseudonodular alterations; in this group, only 40% of the patients were on substitutive therapy. There seems to be no doubt that, with few exceptions, disease involving an apparently circumscribed area of the thyroid in reality affects the entire parenchyma. CONCLUSIONS: Total thyroidectomy, therefore, represents the most appropriate treatment for multinodular goiter, even when the disease is limited to one lobe, thereby providing a cure for the disease and at the same time a state of euthyroidism easily achieved with the proper therapy.

Parathyroid glands: combination of (99m)Tc MIBI scintigraphy and US for demonstration of parathyroid glands and nodules.

PURPOSE: To determine the appropriate choice of imaging techniques for localization of nodular lesions of parathyroid glands. MATERIALS AND METHODS: First, computed tomographic (CT), magnetic resonance (MR), ultrasonographic (US), and technetium 99m methoxyisobutyl-isonitrile (MIBI) scintigraphic images in 49 patients with primary hyperparathyroidism were retrospectively evaluated. A single-blind, prospective study that included 16 patients with primary hyperparathyroidism was then conducted. MR, US, scintigraphic, and color Doppler US images of the neck were obtained and analyzed. RESULTS: In the retrospective study, CT, MR imaging, and US had low sensitivity (13%, 17%, and 27%, respectively) and specificity (39%, 65%, and 65%, respectively). Scintigraphy had 57% sensitivity and 85% specificity. In the prospective study, the use of latest-generation MR and US equipment and the participation of experienced operators led to improved sensitivity and specificity for these techniques. The combination of US and scintigraphy resulted in improved sensitivity (96%), specificity (83%), and positive and negative predictive values (88% and 94%, respectively), relative to the results obtained with either method alone. Doppler US was of little help in the setting of small glands. CONCLUSIONS: The combination of (99m)Tc MIBI scintigraphy and US performed by well-trained operators with up-to-date instruments appeared to be the best diagnostic tool for the preoperative diagnosis of parathyroid disease.

Fractionated total body irradiation in allogeneic bone marrow transplantation in leukemia patients: analysis of prognostic factors and results in 136 patients.

BACKGROUND AND PURPOSE: The results of a single-institution series of patients with chronic and acute leukemias are analyzed with regard to literature-reported predictor variables. MATERIALS AND METHODS: Between 1985 and 1994, 136 patients, 82 patients with chronic myeloid leukemia (CML) and 54 with acute leukemia (AL), received a uniform preparatory regimen of fractionated total body irradiation (TBI; 12 Gy in 3 days) plus different chemotherapy regimens before bone marrow transplantation. Eighty-six patients were considered to be in early phase of disease (CML in chronic phase or AL in first complete remission) and 50 in advanced phase (all those beyond first remission or first chronic phase). Ninety-five patients received unmanipulated allogeneic BM, and 41 T-lymphocyte-depleted BM. RESULTS: The 5-year overall survival (OS) and disease-free survival (DFS) of the whole series were 43% and 31%, and median survival was 43 and 10 months, respectively. A Cox proportional hazard model identified variables related to overall and disease-free survival. For OS, graft versus host disease (GVHD) was the first independent variable (P < 0.0001), followed by age (P < 0.001), T-depletion (P < 0.01), disease status (P < 0.05) and type of leukemia (P < 0.05). With regard to DFS, only T-depletion (P < 0.0001), disease status (P < 0.01) and GVHD (P < 0.01) resulted predictor factors. Early complications after BMT were reported in 59 patients, TBI-induced delayed toxicity in 9 patients, and 16 patients suffered late complications. CONCLUSIONS: Our results confirm the curability of early phase leukemias with standard fractionated TBI-induced Allogeneic bone marrow transplantation (ABMT). With an homogeneous fractionated TBI schedule as employed in our series, T-cell depletion negatively affected the outcome.

Nutritional status modulates rat liver cytochrome P450 arachidonic acid metabolism.

Alterations in nutritional status affect hepatic cytochrome P450 levels. Since cytochromes P450 participate in the metabolism of arachidonic acid, we hypothesized that changes in liver P450 arachidonic acid metabolism occur during fasting and refeeding. Male Fisher 344 rats were either fed, fasted 48 hr (F48), fasted 48 hr and then refed 6 hr (F48/R6), or fasted 48 hr and then refed 24 hr (F48/R24). F48 rats had reduced body weight, increased plasma beta-hydroxybutyrate, and reduced plasma insulin compared with the other groups. Although there was no significant change in total liver P450 content, there was a significant 20%, 48%, and 24% reduction in total hepatic microsomal arachidonic acid metabolism in F48, F48/R6, and F48/R24 rats, respectively, compared with fed rats. Epoxygenase activity decreased by 28%, 51%, and 26% in F48, F48/R6, and F48/R24 rats, respectively. In contrast, omega-1 hydroxylase activity increased by 126% in F48 rats compared with fed rats. Immunoblotting revealed that levels of CYP2C11 protein were markedly reduced, whereas levels of CYP2E1 protein were markedly increased in the F48 and F48/R6 groups. In contrast, levels of CYP1A1, CYP1A2, CYP2B1, CYP2J3, CYP4A1, and CYP4A3 were unchanged with fasting/refeeding. Northern blots revealed that levels of CYP2C11 mRNAs were decreased, whereas CYP2E1 mRNAs were increased in F48 and F48/R6 rats. Recombinant CYP2C11 metabolized arachidonic acid primarily to epoxides with preference for the 14(S),15(R)-, 11(R), 12(S)-, and 8(S),9(R)- epoxyeicosatrienoic acid enantiomers. We conclude that (1) nutritional status affects hepatic microsomal arachidonic acid metabolism, (2) reduced epoxygenase activity in F48 and F48/R6 rats is accompanied by decreased levels of CYP2C11, (3) increased omega-1 hydroxylase activity is accompanied by augmented levels of CYP2E1, and (4) the effects of fasting on CYP2C11 and CYP2E1 expression occur at the pretranslational level.

In the middle of the nineteenth century, the crisis of the traditional model of town planning also determined a strong change in supply and demand for public health. In Italy, a health reformation and reorganization of free hospital treatment and health service were considered essential for civil and social development by public opinion. Meanwhile hygiene was becoming an autonomous discipline incorporating medical, statistical and technical contributions. In particular new rules for hospital design were being formulated which did not only concern buildings, but also involved management and administration. Nevertheless, the presence of monumental hospitals in many Italian towns delays the spread of new typological models, especially because of the high costs in building new hospitals. Old or new hospitals was the theme of a very interesting debate, involving technical and scientific environments and public opinion. The Santa Maria Nuova Hospital in Florence is an outstanding example of the above.

The risk of non-Hodgkin's lymphoma after Hodgkin's disease, with special reference to splenic treatment.

BACKGROUND AND OBJECTIVE: One of the consequences of the enormous improvement in survival rates of patients treated for Hodgkin's disease (HD) is the emergence in the long term of treatment-related complications, particularly secondary cancers. This study was undertaken to observe the occurrence of non-Hodgkin's lymphoma (NHL) in patients treated for HD and to identify the etiological role of various risk factors, especially spleen irradiation, in the pathogenesis of this illness. DESIGN AND METHODS: From 1972 to 1996, the Department of Radiation Oncology and the Hematology Section of "La Sapienza" University of Rome observed and analyzed the occurrence of NHL in 1,391 patients treated for HD. The average follow-up period was 84 months. For a more accurate calculation of the risk of the occurrence of NHL, the patients were first divided into 3 groups according to their initial treatment and also according to the total treatment they had received. Then, in order to establish the possible connection between NHL and splenic treatment the patients were also divided into 3 subgroups according to whether they had undergone splenectomy, splenic irradiation or neither of these. Two different methods of statistical analysis were used: (a) the cumulative risk (confidence interval) was evaluated in relation to treatment (initial and at the time of salvage) and (b) the Cox model was applied to identify the variables which play a role in the appearance of NHL. The cumulative risk of developing NHL was assessed using the Kaplan and Meier method. A multivariate analysis was performed using the Cox Proportional Hazard Model. RESULTS: A total of 20 cases of NHL were observed, appearing between 17 and 206 months after initial treatment. The cumulative risk was 0.8%, 1.8%, 2.6% and 3.5% at 5, 10, 15 and 20 years respectively. According to the multivariate analysis, significant risk factors were splenic irradiation and age (> 40 years). Splenic irradiation (vs no splenectomy/no splenic irradiation) showed a relative risk of 5.69, p = 0.0280, while age over 40 showed a relative risk of 3.05, p = 0.0152. INTERPRETATION AND CONCLUSIONS: From the results of this study, if appears that there is a possibility that splenic irradiation and age over 40 increase the risk of NHL in HD patients. Further studies are needed to investigate in greater depth the role of spleen irradiation in the occurrence of this illness.

Combined modality therapy in advanced Hodgkin's disease: a report on 218 patients with a median follow-up of eight years.

BACKGROUND AND OBJECTIVE: This study was designed to evaluate the efficacy and toxicity of monthly alternating ABVD/MOPP compared to ABVD/OPP regimens in patients with advanced stage Hodgkin's disease (HD), as well as in early stage patients with systemic symptoms and/or bulky disease. DESIGN AND METHODS: 218 patients with previously untreated HD entered this study: 106 patients in arm A (ABVD/MOPP) and 112 in arm B (ABVD/OPP). Patients received eight courses of one of the two regimens after stratification according to the stage. Patients in complete remission (CR) received 20 Gy to the involved field and 40 Gy to the spleen. The actuarial survival curves were performed according to Kaplan and Meier. RESULTS: No statistically significant differences were observed between the two arms in terms of CR rate and toxicity. However, analysis of total relapses revealed that patients treated with ABVD/OPP had a significantly higher likelihood of achieving a second CR compared to patients who entered the ABVD/MOPP arm. INTERPRETATION AND CONCLUSIONS: Both schemes of chemotherapy followed by radiotherapy produce high percentages of CR, low risk of relapse and an acceptable toxicity.

Acute nonlymphocytic leukemia: onset after treatment for Hodgkin's disease.

This study was undertaken to examine the influence of various factors on the occurrence of acute nonlymphocytic leukemia (ANLL) in a group of longterm survivors of Hodgkin's disease (HD). From 1972 to 1992, 1045 patients with HD were assessed and treated at the Department of Radiation Oncology, the Institute of Radiology, and the Department of Human Biopathology, Hematology Section, University of Rome "La Sapienza". The average follow-up was 72 months. For a more accurate calculation of the risk of ANLL occurrence, the patients were first divided into three subgroups according to initial treatment and then according to the total treatment they had received. Moreover, to establish the probable connection between leukemia and splenic treatment the patients were also divided into three subgroups (splenectomy, splenic irradiation, and no splenectomy/no splenic irradiation). Sixteen cases of ANLL were recorded, giving an overall risk of 0.2% at 5 years and 3.4% at 20 years. In 12 patients overt leukemia was preceded by a myelodys-plastic syndrome. Five cases had evaluable chromosome analysis. Clonal chromosome abnormalities were demonstrated in two patients, whereas three patients showed an apparently normal karyotype. Bone marrow agar cultures were analyzed in two patients and were abnormal in both cases. In the radiotherapy (RT) group, the cumulative risk was 0.4% at 10 years and 3.2% at 15 and 20 years: in the chemotherapy (CT) group it was 1.2% at 10, 15, and 20 years; in the combined group it was 3.7% at 10 years and 4.9% at 15 and 20 years. In the multivariate analysis, MOPP treatment with or without RT is a statistically significant variable for ANLL occurrence (p = 0.009). This study demonstrates that splenic treatment does not lead to ANLL. Treatment with MOPP alone and with MOPP plus RT can increase the risk of ANLL.

Multicentre experience with combined hyperthermia and radiation therapy in the treatment of superficially located non-Hodgkin's lymphomas.

The combination of Radiation Therapy (RT) and Hyperthermia (HT) has proved to be an effective treatment for a wide variety of superficially located recurrences of different tumors, particularly those arising in previously irradiated areas. Few studies on the use of HT in the management of lymphomatous diseases have so far obtained interesting results. Eight patients with Non Hodgkin Lymphomas (LNH) - 4 with cutaneous lymphomas and 4 with nodal recurrences after RT-Chemotherapy (CHT) treatment treated in three different Italian institutions with combined RT and HT are here reported. Rt dose ranged from 15 to 40 Gy with different fractionations, on the basis of previously received treatment. Hyperthermia was delivered using 432 or 915 MHz external microwave applicators, according to extension and depth of the lesions and available equipment. All patients tolerated well the HT treatment, and in all cases average intratumoral temperatures were >42 degrees, with 3 out of 10 treated sites achieving the goal of average temperatures >42.5%. One patient, with recurrent NHL, is disease-free after 24 months from completion of combined therapy. Our results seem to confirm previous experiences, suggesting a role of HT/RT not only for palliative purposes in cutaneous lymphomas, but also as an adjunct to radiotherapy alone in selected patients with superficially located recurrences.

cDNA-directed expression of two allelic variants of cytochrome P450 2C11 using COS1 and SF21 insect cells.

Cytochromes P450 (CYP) are ubiquitous enzymes which metabolize multiple endogenous and exogenous substrates. CYP2C11 is the most abundant among the P450s present in untreated rat liver (approximately 50% of total P450) and is known for metabolizing testosterone mainly in positions 2 alpha and 16 alpha. In Gunn rats, the specific enzymatic activities of CYP2C11 are decreased by 90%, although the protein is present at levels similar to that of the Wistar reference strain. In order to explain this difference, we cloned and expressed CYP2C11 Wistar and Gunn cDNAs in two heterologous systems. Three mutations were identified in Gunn CYP2C11 form: Val4 --> Ala in the transmembrane region, Asn116 --> Ser in the substrate recognition site SRS-1, and Phe187 --> Leu in the (E --> F) interhelical region when compared to the spatial structure of CYP101. Sf21 insect cells displayed high yields of expressed CYP2C11 proteins (up to 2 nmol CYP2C11/mg microsomal proteins) which are more than 60-fold those observed in COS1 cells and 4-fold those present in hepatic microsomes. Testosterone-hydroxylating activities from CYP2C11 Gunn expressed in insect cells were decreased by 40% when compared to CYP2C11 Wistar, reflecting with a lesser extent the decrease we have previously reported in liver microsomes. The high level of active protein obtained with the baculovirus/insect cell expression system will be useful to analyze activities of site-directed mutants of CYP2C11 in order to elucidate the respective influence of the identified mutations on the enzymatic activities.

Acoustic rhinometry: practical aspects of measurement.

Rhinological diagnosis using acoustic rhinometry (AR) is described. The reliability of the test is improved by using a variable geometry craniostat allowing identical repositioning of the subject's head in various tests.

[Presumed physiopathology of the nasal wall surgery and contemporary techniques of the inferior turbinates surgery].

The lateral wall of the nasal cavity is anatomically complex and covers several nasal functions: ventilation, conditioning of the air, protection. This region also plays a prime role in maintaining nasal-sinus homeostasis through dynamic regulation of the two-way exchange between the nasal cavity and paranasal sinuses. In light of the most recently acquired information, physio-pathological aspect of the lateral wall are discussed in terms of ventilation mechanisms and mucociliary clearance of the paranasal sinuses. In this light, the key to nasal-sinus physiology is clearly the perviousness of the side wall ostium and, above all, the status of the structures making up the "ostiomeatal complex". The anatomo-functional integrity of the lateral wall is fundamental for sinus ventilation 90% of which depends on passive diffusion of air through ostium. It is also a key to mucociliary clearance, always directed toward the main ostium. This rationale provides the basis for the so-called functional surgery of the lateral wall aimed at restoring ostium permeability. One structure in the lateral wall is a key to nasal physiology: the lower turbinate. Anatomical or functional alterations of the lower turbinate is one of the major causes of nasal stenosis. Pharmacological treatment of this pathologies is not always satisfactory, hence one leans toward surgical treatments. Nevertheless, the wide range of techniques available attests to the fact that there is still great deal of uncertainty in this regard. After having analysed the data available in the literature, the authors propose a comparison of some of the most frequently used forms of surgery: electrocauterization, cryotherapy, laser therapy, submucosal decongestion with or without luxation, turbinectomy. Follow-up based on rhinomanometric evaluation, rhinometry, TTMC, IgA assay and symptom scoring has been performed for 4 years. The results show that the submucosal decongestion technique, particularly the variation with lateral luxation, is considered most effective in resolving obstruction while respecting nasal physiology.

[Hodgkin's disease in stages I and IIA. Analysis of results in a group of patients with a follow-up of at least 10 years treated at the onset of the disease with radiotherapy alone].

January, 1972, through December, 1982, ninety-four patients with Hodgkin's disease in stages I and IIA were treated in the Dept. of Radiation Oncology of the University "La Sapienza", Rome. All patients had a minimum follow-up of 10 years. After careful clinical and surgical examinations comprehensive of laparosplenectomy, all patients were submitted to extended field radiation treatment. Ninety-one of 94 patients (97%) obtained complete remission. Overall survival at 10 years was 91% and independent of stage (94% in stage I and 87% in stage II), while relapse-free survival at 10 years did depend on stage (83% in stage I and 49% in stage II); the difference was statistically significant (p < 0.01). Thirty-one of 94 patients relapsed, but fortunately 94% of them obtained a second complete remission after salvage chemotherapy. Our experience confirms the low tumorigenic potential of exclusive radiation therapy; only one of 4 secondary neoplasms (acute non-lymphatic leukemia) observed in this series had been treated with radiotherapy. The results of this study confirm the efficacy of exclusive irradiation in stage I patients, while treatment approach remains debated in stage II patients. The combination of short-time chemotherapy with curative irradiation is probably the best option to reduce both the incidence of recurrences and treatment-induced complications.

Multiple interferences on catecholamine metabolism by tetrahydroisoquinolines (TIQs).

TIQs are thought to be formed by condensation between dopamine and certain metabolites of ethanol, organic solvents and anesthetic gases. Described here are experiments aimed at evaluating TIQs interference with catecholamine synthesis. Rat adrenal pheochromocytoma (PC12) cell lysates were exposed to benzyl-TIQ and phenyl-TIQ. The activities of tyrosine hydroxylase (TH) and dopamine beta-hydroxylase (DBH) were measured by HPLC-based methods following exposure to variable concentrations of TIQs. The effects of TIQs on DBH activity were also assessed in human serum. Dixon plot analyses revealed that TIQs act on TH as competitive inhibitors with different affinity. Ki for benzyl- and phenyl-TIQ were 5 and 3 microM respectively. DBH activity in serum exposed to benzyl- and phenyl-TIQ ranging from 0.2 to 20 microM rose respectively by 12.5% to 58% for benzyl- and by 7.8% to 26% for phenyl-TIQ. Such TIQs interferences with catecholamine metabolism seem to account for dopamine (DA) depletion observed in parallel in vitro experiments on PC12 cells. The dose-dependent inhibition of TH and the increased activity of DBH together with the relatively low effective doses of TIQs suggest this mechanism as a possible explanation of the selective toxicity of styrene and other solvents to dopaminergic systems observed in rabbits following experimental exposure and suspected to occur in occupationally-exposed workers.

Reversible tubular proteinuria precedes microalbuminuria and correlates with the metabolic status in diabetic children.

Early tubular alterations were studied in 53 children with insulin-dependent diabetes mellitus (IDDM), 32 of whom were followed at regular 6-monthly intervals for 3 years. The urinary levels of retinol-binding protein (RBP), beta 2-microglobulin and brush border antigens (BBA) (determined by monoclonal enzyme immunoassay) were taken as indices of functional and cellular tubular alterations; urinary albumin was considered an early marker of glomerular alterations. All indices of tubular alterations were higher in IDDM children than in 368 normal children, while albuminuria was unchanged. Urinary levels of BBA, however, varied widely during follow-up, with 25 of the 32 IDDM patients who were followed at regular intervals having pathological values for BBA on at least one occasion, followed by normalization. Metabolic alteration was found to be the main cause of this variability, since a high statistical correlation was found between urinary BBA and fructosamine (P < 0.001) and between RBP and the stable fraction of glycosylated haemoglobin (P < 0.001). The data confirm that transient tubular proteinuria occurs in diabetic children before any other marker of renal involvement such as microalbuminuria. The maintenance of good metabolic control is essential to normalize this early abnormality that can be considered a reversible sign of functional renal involvement.
