[Heart failure patient management: evolution, organization, application on a local scale].

Heart failure is a severe disease which prevalence and incidence are steadily growing, with high morbidity and mortality rates, and responsible for significant financial cost. This evolution requires our therapeutic approach to be modified, by favouring multidisciplinary management and including patient education. The proof of the effectiveness of these measures is brought by many studies, and therapeutic education is now recommended by the European Society of Cardiology. This new approach of management is set out according to various procedures. In Le Creusot, the therapeutic unit of heart failure management was created in September 2004 and operate on a day hospital pattern. The results are satisfactory knowing that the average duration of spells fell by 33% after one year. Further local initiatives are spreading and need shall arise to pooling equipment and personnel as well as reinforcing collaboration between public and private professionals in order to improve heart failure patients' management. In the meantime a more sustained involvement of the authorities is to be hoped.

Clearance of human-pathogenic viruses from sludge: study of four stabilization processes by real-time reverse transcription-PCR and cell culture.

Sludges derived from wastewater treatment are foul-smelling, biologically unstable substances. As well as containing numerous pathogenic microorganisms, they also consist of organic matter that can be used as agricultural fertilizer. Legislation nevertheless requires sludges to be virologically tested prior to spreading by the counting of infectious enterovirus particles. This method, based on culture of enterovirus on BGM cells, is lengthy and not very sensitive. The aim of this study was to propose an alternative method of genome quantification for all enteroviruses that is applicable to verifying the elimination of viruses in complex samples such as sludges. Our complete protocol was compared to the official method, consisting of enterovirus enumeration with the most probable number of cythopathic unit (MPNCU) assay through the study of four stabilization procedures: liming, composting, heat treatment, and mesophile anaerobic digestion. Enterovirus quantities at the start of the stabilization procedures were between 37 and 288 MPNCU/g on the one scale and between 4 and 5 log genome copies/g on the other. It was shown that all procedures except mesophile anaerobic digestion were highly effective in the elimination of enterovirus particles and genomes in wastewater sludges. Reduction of viruses by mesophile anaerobic digestion was by only 1 log (infectious particles and genomes). In conclusion, stabilization processes can indeed be checked by virological quality control of sludges with gene amplification. However, the infectivity of genomes needs to be confirmed with cell culture or a correlation model if the virological risk inherent in the agricultural use of such sludges is to be fully addressed.

Client communication behaviors with health care providers in Indonesia.

Patient participation in health care consultations can improve the quality of decision making and increase patients' commitment to the treatment plan. This study examines client participation, operationally defined as client active communication, during family planning consultations in Indonesia. Data were collected on 1203 consultations in the provinces of East Java and Lampung. Sessions were audiotaped and the conversation coded using an adaptation of the roter interaction analysis system (RIAS). Culturally acceptable ways for Indonesian clients to participate in consultations include asking questions, requesting clarification, stating opinions, and expressing concerns. Factors significantly associated with client active communication were, in order of importance, providers' information giving, providers' facilitative communication, providers' expressions of negative emotion, client educational level, and province. The latter suggests the influence of culture on client participation. The results suggest that a combination of provider training and client education on key communication skills could increase client participation in health care consultations.

The influence of biosolids treatment files on the mobility of metal trace elements.

The production of sludge in France is estimated to be about 900,000 metric tons dry matter per year and 60% of this is recycled onto agricultural land. At present, the long term future of this procedure is open to question and among the different arguments being put forward are the levels of metallic trace elements and the risk of accumulation in soils. This study presents the behaviour of metallic trace elements in sludges from three different treatment procedures: thickened liquid sludges, dewatered sludges and dried sludges. These biosolids are mixed with a clay soil and then placed in a temperature and humidity controlled glasshouse. Several containers are seeded with ryegrass and compared with controls. For the three harvests, covering all the amendments studied (including non-amended soil), the differences are not really representative. Absorption by the ryegrass is low in all cases. For the cadmium, the chromium, the nickel and the lead, the roots are 5 to 10 times more concentrated than the leaves. The majority of these elements stay absorbed in the roots, regardless of the amendment used. The addition of the sludges has considerably reduced the uptake of water in ryegrass throughout its growth cycle. Quite apart from their fertilizing qualities, wastewater treatment plant sludges could offer important implications for irrigation.

[Outcome of 111 thyroid papillary carcinomas. Retrospective study from 1953 to 1994. Experience of Bergonié Institute].

The evolution of techniques has increased the number of diagnosed thyroid papillary microcarcinoma challenging the previous therapeutic guidelines. We have studied all thyroid cancers in our data base (from 1953 to 1994, N = 698). One-hundred and eleven were papillary microcarcinomas (none before 1967): 83 M0N0, 24 M0N+ et 4 M+. One-hundred and eight pathologic slides could be obtained and were checked by one of us (pathologist). The mean follow-up was 8.6 yr [1-28]. Two clinical and pathological features were different between M0N0 and M0N+ (groups: i) the sex-ratio, 12 vs 50% men (M0N0 vs M0N+, p < or = 0.0001), ii) the invasiveness of the tumor, 54/83 vs 20/24 (M0N0 vs M0N+, p = 0.01). All patients received a suppressing treatment by thyroid hormones (except lobectomy). The other treatments were: for 64/83 M0N0 a total thyroidectomy followed by a I131 treatment, for the remaining no I131 (15 partial thyroidectomies and 4 total thyroidectomies). None of the M0N0 patients displayed metastasis or cancer relapse. All patients M0N+ had a total thyroidectomy followed by a I131 treatment. Among the latter, I had a major node evolution, the death was attributed to the cancer at the age of 83 after a 6 yr long evolution. Among the 4 patients considered as metastatic. 1 was poorly differentiated. For the remaining 3, the thyroidal origin of the metastasis was not clear. In summary, the papillary microcarcinomas display a good prognosis if devoid of metastasis. We propose the following guidelines: a total thyroidectomy followed by a I131 treatment and suppressing treatment if clinical nodes, or non-encapsulated tumor or multifocal tumor. For solitary tumors, well encapsulated a lobectomy appears sufficient.

Gentamicin, norfloxacin and lysozyme concentration in human tears: in vivo and in vitro study.

Hen's egg lysozyme (HEL) activity was measured in vitro with gentamicin and norfloxacin by a turbidimetric technique. Gentamicin at the concentration of 10(-3) M inhibited HEL activity by 39%, while 10(-3) M norfloxacin did not affect HEL activity. However, an in vivo study in healthy persons did not show any significant statistical difference in tear lysozyme activity when 0.3% gentamicin or 0.3% norfloxacin were topically applied.
