Successful surgical removal of occult metastases of medullary thyroid carcinoma recurrences with the help of immunoscintigraphy and radioimmunoguided surgery.

Patients with recurrent or metastatic medullary thyroid carcinoma (MTC) were referred for pretargeted immunoscintigraphy (Affinity Enhancement System; AES) and radioimmunoguided surgery (RIGS). Data collected from 13 patients establish that whole-body AES immunoscintigraphy revealed metastases < 360 mg and RIGS detected micrometastases (5-15 mg). All tissue samples removed by the surgeon were diagnosed by histology and immunohistochemistry of calcitonin to check the accuracy of IS and RIGS results. AES immunoscintigraphy is very sensitive. Of 34 metastases or recurrences detected, 22 had escaped physical examination or conventional imaging. The accuracy of RIGS was 86%, its sensitivity 75%, and its specificity was 90% (n = 208). IS and RIGS detected occult tumors that would have escaped surgery, clearly demonstrating clinical benefit. Serum calcitonin (normal, 10 pg/ml) and carcinoembryonic antigen (normal, 5 ng/ml) of two patients were restored to normal. In patients whose tumors were discovered, progression of their disease was slowed, as evidenced by the large decrease in serum calcitonin and carcinoembryonic antigen, an important prognostic factor. Surgery was canceled in one case where IS detected distant metastases out of surgical reach. Thus, AES immunoscintigraphy and RIGS might be of valuable help for the surgical management of medullary thyroid carcinoma.

[Buccal lupus lesions (author's transl)].

Lupus is a particularly polymorphic disease involving all medical disciplines and inciting the publication of an increasing number of papers. The two clinical types of lupus (acute disseminated lupus erythematosus and chronic lupus) have, until recently, led to doubts as to the unified nature of the disease. Grouping the symptoms and the results of their investigation in a computer has shown that there are 14 elements which occur most frequently, though there is no absolute pathognomonic sign of the disease. Oropharyngeal ulcerations, without being typical, are on of the fundamental signs and were found in 10% of cases of chronic lupus. The Gougerot-Sjögren syndrome is sometimes found, but it is not one of the fundamental signs : its existence, however, has enabled progress to be made during research into the auto-immune physiopathology of lupus disease.

[Buccal metastases from soft-tissue cancers. A report on 3 cases (author's transl)].

Histological examination of the gingivovestibular lesion in the first case with known kidney cancer, confirmed the metastatic nature of the buccal tumor and its origin. The second case raised the problem of the diagnosis of a labial swelling occurring during widespread metastases from breast cancer. The third case was more complex as the palatine lesion was seen initially, and metastatic seeding extremely rapid. This led to the hypothesis only, that bronchial cancer was the primary lesion.

[Aneurysmal cysts located in the maxilla or mandible (author's transl)].

Aneurysmal cysts of the mandible and maxilla are rare conditions, though not exceptional, which are seen in particular in the child and adolescent. A part from facial asymmetry and tumefaction, it is rare to find other clinical manifestations. No neurological signs were seen in the present study. An aneurysmal cyst gives a radiological appearance of non-characteristic lysis compatible with a large range of diagnoses. Histological study permits early diagnosis but must always involve the whole specimen with careful examination to detect any other associated lesion. The pathogenesis of the condition remains obscure and, at the extreme, it may be questioned whether the aneurysmal cyst is a separate entity or a particular form of chronic osteopathy.

[Oral aphthoid toxic dermatoses].

In a description of 4 cases (phenindione, 1 case; niflumic acid, 1 case; gold salts, 2 cases), the authors describe a new variety of oral toxicodermatitis characterized by an eruption of painful, infiltrated aphthoid ulcerations, not precededbybull ae, small (less than 1 cm), roundish, with a greyish yellow necrotic centre surrounded by an erythematous halo. Histological investigation reveals a compact, polymorphous inflammatory infiltrate with numerous polynuclear neutrophiles or, less commonly, eosinophiles some of which are in a state of pyknosis of leukocytoclasia, associated with phenomena of spongiosis or necrosis of the epithelium.
