UK collaborative randomised trial of neonatal extracorporeal membrane oxygenation: follow-up to age 4 years.

Extracorporeal membrane oxygenation (ECMO) is a supportive intensive-care technique used for babies with acute respiratory failure. We examined morbidity at age 4 years in surviving children recruited to the UK Collaborative ECMO Trial, and provide long-term data on ECMO support compared with contemporary conventional care. The neonatal ECMO policy resulted in improved survival and a favourable outcome. We therefore advocate the safety and efficacy of this intervention.

Patient acceptance of endovaginal ultrasound.

OBJECTIVE: To assess women's experiences and attitudes regarding endovaginal ultrasound. METHODS: Women attending the obstetrics and gynecology clinics at the University of Florida were asked to complete an anonymous questionnaire. RESULTS: Of respondents who had had a prior endovaginal ultrasound examination, 26% reported that 'it hurt a lot' and 50% reported that 'it hurt a little'. In contrast, only 23% of patients had experienced any pain with a prior abdominal ultrasound (P < 0.001). Multivariate analysis showed that discomfort with a prior vaginal ultrasound was related to patient age category, with less discomfort being experienced with increasing ages (P = 0.001). A history of unwanted sex, sexual or physical abuse, or dyspareunia was not associated with more painful examinations. Most patients were willing to undergo a future vaginal ultrasound investigation if it were recommended by their doctor. Multivariate analysis showed that more willingness was associated with older age category (P = 0.004), a history of dyspareunia (P = 0.03) and bleeding in a current pregnancy (P = 0.005). Of the patients (less than half) who had a preference, most preferred a female sonographer for endovaginal sonography (P < 0.001) and most preferred that a doctor, rather than a nurse or technician, perform the examination (P < 0.001). CONCLUSIONS: The majority of women who had experienced a vaginal ultrasound examination found it somewhat uncomfortable, but almost all women who returned the questionnaire expressed a willingness to undergo endovaginal ultrasound if it were recommended.

Preventing nursing turnover in home care.

Turnover is costly to any organization. Frequently, turnover creates a ripple effect that is felt by the agency, its patients, and staff at all levels. The very nature of home care creates an environment in which nurses are not visible in the office every day. It is difficult to track the activities of new employees on a day-to-day basis to know how they are progressing through the orientation process. What causes turnover among registered nurses within the first six months of employment? One agency shares its story and related findings.

Restraint-free care. Part 1: Legal and regulatory mandates. Part 2: Creating a restraint-free environment.

In response to the revised 1996 Joint Commission of Accreditation of Healthcare Organization's restraint standards, many facilities are assessing and revising their current practice. Part 1 discusses the regulatory, legal and ethical dimensions of patient restraints. Part 2 describes one hospital's approach to promoting a restraint-free environment. As a result of this hospital's assessment, restraint use decreased 25% in intubated patients; 50% in nonintubated patients.

Efflux of alanine by Giardia intestinalis.

Giardia intestinalis trophozoites synthesise and then secrete large amounts of alanine into the external medium during growth. This efflux of alanine was studied by preloading cells with L-[2,3-3H]alanine, and determining efflux of radiolabel from intact trophozoites. The efflux of alanine was also determined by measurement of alanine concentration in trophozoites and external medium using high pressure liquid chromatography amino acid analysis. Over the temperature range 4 degrees C to 37 degrees C there was a slow efflux of alanine, but this efflux was greatly stimulated by a number of amino acids structurally similar to alanine, notably glycine, L-serine, L-threonine, L-asparagine and L-glutamine. In contrast, 2-aminoisobutyrate, D-amino acids, and other naturally occurring amino acids had no effect. Those amino acids which stimulated the efflux of intracellular alanine are the same amino acids which inhibited uptake of extracellular alanine. This concordance suggests that an alanine antiport functions for both the influx and efflux of alanine, and acts to maintain a balance between intracellular and extracellular alanine concentrations.
