Safety and efficacy of intracameral injections of unpreserved lidocaine to reduce intraocular sensation.

PURPOSE: To determine the safety and efficacy of intracameral injections of unpreserved lidocaine to reduce intraocular sensation. SETTING: Carolina Eye Associates, Southern Pines, North Carolina, USA. METHODS: In this prospective study, 100 patients having phacoemulsification under topical anesthesia were randomly assigned to receive an intracameral injection of unpreserved lidocaine (0.5 cc of a 1% unpreserved solution) or balanced salt solution. RESULTS: Ninety-three patients completed the study. There were no significant between-group differences in cell density parameters or Kowa laser flare-cell meter measurements. Seventy-eight percent of lidocaine patients and 56% of controls reported no sensation (P = .048, Fisher's exact test). CONCLUSION: Intracameral injections of unpreserved lidocaine safely and effectively reduced intraocular sensation during phacoemulsification.

Superintendents' perceptions of schoolsite health promotion in Missouri.

This study assessed school district superintendents' perceptions of schoolsite health promotion in Missouri using a telephone survey method. Superintendents of 150 school districts were surveyed using The Worksite Health Promotion for Faculty and Staff section of the Eight-Component Assessment. Staff identification of blood pressure, healthful cafeteria menus, and smoke-free school policies were perceived as the most frequently offered health-promoting activities. Quality of programs as perceived by district superintendents ranged from "average" to "exemplary." For school districts not offering programs, healthful cafeteria menus, staff identification of blood pressure, and stress management were perceived by superintendents as highly necessary. The number of programs offered in each area differed significantly by demographic characteristics: accreditation classification, assessed valuation, size, and residence. Supportive attitudes by superintendents are critical to attain schoolsite health promotion programs for faculty and staff as part of a comprehensive school health program in Missouri.

Effect of cataract wound incision size on acute changes in corneal topography.

A randomized prospective trial compared keratometric and visual outcomes for 196 patients receiving 3.2 mm, 5.0 mm, or 6.0 mm incisions for cataract removal and intraocular lens implantation. There were no statistically significant differences among the groups preoperatively or at one to two days or three to six months postoperatively in mean keratometric cylinder, no differences postoperatively in surgically induced cylinder, and no differences in best-corrected visual acuity. Significantly more (P < .01) 3.2 mm cases (75%) had uncorrected visual acuity of 20/40 or better at one to two days postoperatively than 5.0 mm (50%) or 6.0 mm cases (47%). Computer-assisted corneal topographic images were available for the last 56 patients enrolled. Patients receiving 3.2 mm incisions had significantly less wound-related flattening than other patients (P < .05) and tended to have fewer surgically induced changes in corneal shape overall. Changes identified by corneal topography were inconsistent with keratometrically assessed cylinder in 14 patients.

Differential stimulation of the respiratory burst and lysosomal enzyme secretion in human polymorphonuclear leukocytes by synthetic diacylglycerols.

Binding of chemoattractants to receptors on human polymorphonuclear leukocytes (PMN) stimulates the phosphodiesteric cleavage of phosphatidylinositol 4,5-bisphosphate to produce inositol 1,4,5-trisphosphate and 1,2-diacylglycerols. To investigate the possible second messenger function of diacylglycerols in PMN activation, we tested the ability of a series of synthetic sn 1,2-diacylglycerols, known to stimulate protein kinase C in other systems, to promote superoxide anion release, oxygen consumption, lysosomal enzyme secretion, and chemotaxis. None of the diacylglycerols initiated the chemotactic migration of PMN. Several of the diacylglycerols however, were, active in stimulating superoxide anion release and lysozyme secretion, with dioctanoylglycerol (diC8) being the most potent. Unexpectedly, didecanoylglycerol (diC10) induced lysosomal enzyme secretion, but failed to stimulate superoxide production or oxygen consumption. All other biologically active diacylglycerols tested displayed similar EC50 for stimulating lysozyme secretion and superoxide production. The ability of the diacylglycerols to compete for phorbol dibutyrate (PDBu) binding in intact PMN suggested a mechanism for the divergent biological activity of diC10. Although the compounds that stimulated both superoxide production and lysosomal enzyme secretion competed for essentially all [3H]PDBu binding from its receptor, diC10, which only stimulated secretion, competed for 45% of the bound [3H]PDBu. Thus diacylglycerols can selectively activate certain functions of leukocyte chemoattractant receptor. The data suggest that a discrete pool of protein kinase C may mediate activation of the respiratory burst in PMN.

Receptor-coupled activation of phosphoinositide-specific phospholipase C by an N protein.

Cleavage of phosphatidylinositol 4,5-bisphosphate by phospholipase C results in the production of two important second messengers: inositol-1,4,5-trisphosphate and 1,2-diacylglycerol. Although several receptors promote this cleavage, the molecular details of phospholipase C activation have remained unresolved. In this study, occupancy of a Ca2+-mobilizing receptor, the oligopeptide chemoattractant receptor on human polymorphonuclear leukocyte plasma membranes, was found to lead to the activation of a guanine nucleotide regulatory (N) protein by guanosine 5'-triphosphate. The activated N protein then stimulated a polyphosphoinositide-specific phospholipase C by reducing the Ca2+ requirement for expression of this activity from superphysiological to normal intracellular concentrations. Therefore, the N protein-mediated activation of phospholipase C may be a key step in the pathway of cellular activation by chemoattractants and certain other hormones.

Pseudomonas alcaligenes endocarditis.

Pseudomonas alcaligenes is a common soil and water inhabitant that has rarely been proven a human pathogen. We describe a fatal case of Pseudomonas alcaligenes endocarditis. The need for accurate identification of unusual organisms isolated in a clinical setting are discussed.

Toxicity of platinum (IV) salts for cells of pulmonary origin.

The acute toxicity of tetravalent platinum was studied in vitro by use of rabbit alveolar macrophages and human lung fibroblasts (strain WI-38). Alveolar macrophages were exposed in tissue culture for 20 hr to platinum dioxide (PtO2) or platinum tetrachloride (PtCl4). There was no evidence of dissolution of PtO2 and no decrease in viable cells at concentrations as high as 500 mug/ml. PtCl4 was soluble in the macrophage system and after a 20-hr exposure, resulted in loss of viability in 50% of the cells originally present at a concentration of 0.30mM (59 mug Pt/ml). After a 20-hr exposure, rapidly growing human lung fibroblasts were rendered nonviable by PtCl4 at comparable concentrations. A decrease in total cellular ATP was observed at lower concentrations in macrophages and fibroblasts along with a reduction in phagocytic activity of macrophages as compared to controls. With the fibroblasts, a 50% decrease in incorporation of 14C-thymidine was observed after a 22-hr exposure to PtCl4 at a concentration of 0.007mM; higher concentrations were required to inhibit the incorporation of 14C-uridine and 14C-leucine. Time-course studies indicated that the inhibition of 14C-thymidine incorporation was nearly complete (90%) after 7 hr in the presence of 0.06mM PtCl4. Under the same conditions, there was little inhibition (15%) of 14C-leucine incorporation and moderate inhibition (50%) of 14C-uridine incorporation. Higher concentrations of PtCl4 were required to inhibit 14C-thymidine incorporation into the acid-soluble fraction than were required to inhibit incorporation into the acid-precipitable fraction. Hence, the preferential inhibition of DNA synthesis by PtCl4 may result from an impairment of the incorporation process.

The Nuer Nutrition Education Program: breaking down cultural barriers.

Very little is known about the Nuer culture, partly because of its widely misunderstood language and because of the mixture of its people with the other tribes in Africa, according to Evans-Pritchard). However, it is known that the Nuer women's roles in the family seem to be centered around cooking duties. In the Nuer culture, no work is considered degrading, and the women know and accept their domestic duties. During the summer of 1996, a pilot project was conducted as an attempt to help Nuer refugee women of Des Moines, Iowa, incorporate nutritional concepts and American food preparation techniques into their existing methods of food preparation. The barriers faced involved the Nuer women's unfamiliarity with American foods and household items and their inability to read and understand English. Cultural issues and barriers were overcome when the health educator was willing to take the time to gain the trust and respect of the Nuer people. Structured interviews indicated an increase in knowledge of American foods and cooking skills. This education program in no way meant to replace traditional Nuer cooking methods; rather, it acted as a way to adjust to life in the United States.

Comparison of the scope of allopathic and osteopathic medical school health promotion programs for students.

PURPOSE: To compare the number and scope of health promotion programs for students in allopathic and osteopathic medical schools in the U.S. and Canada. DESIGN: A one-time cross-sectional survey design was applied in this study. SETTING: This study was conducted in 141 accredited allopathic and 17 accredited osteopathic medical schools. SUBJECTS: A total of 158 representatives from the allopathic and osteopathic medical schools participated in this study. The response rate for the survey was 100%. MEASURES: A structured telephone interview was conducted to survey representatives from the medical schools. The survey contained 85 multiple-choice questions organized into four sections: administrative characteristics, types of institutional and health promotion program policies, participation incentives and facilities, and type/scope of health promotion program activities. Chi-square analysis was used to analyze survey variables by type of medical education and level of intervention. RESULTS: Of the 158 medical schools, only 20% (n = 32) provided a health promotion program for students. Although osteopathic institutions (29.4%) had a greater percentage of programs than allopathic schools (19.2%), there was no significant difference in scope of program offerings by type of medical education. Allopathic programs offered exercise and nutrition/weight management significantly more often and at a higher level of intervention. Lastly, allopathic programs had significantly more monetary resources available for programming. Following prudent research protocol, investigators should be mindful of the limitations of this study. In this study, some school representatives chose not to answer personnel- and finance-related questions. Additionally, because of the self-report nature of the survey, the responses given to the questions may not have been accurate. CONCLUSION: Allopathic and osteopathic medical school health promotion programs for students were very similar in scope.

Private sector worksite health promotion programs in Missouri: a comparison with national survey findings and Healthy People recommendations.

Questionnaries with 22 multiple-choice questions were mailed to a random sample of 450 Missouri employers with 250 or more employees to determine the prevalence and types of workplace health promotion programs; 262 (58%) responded. Only 34% of the responding companies offered any kind of program and 65% of these offered only awareness programs, but most offered programs on all 18 topics listed in the questionnaire; 48% had a full-time program manager, 79% had some form of evaluation, and 36% had some form of financial incentives.

Temporal patterns of parturition in mammals in captivity.

Previous research indicates that many primate species are more likely to give birth at night than during the day. This study attempted to determine whether temporal patterns of birth are discernible in a number of non-primate mammals. In addition, it sought to relate time of birth to the sleep/wake cycle and to the level of predation on the species. Data were collected and analyzed on birth hours in 26 mammalian species from two Chicago area zoos . Although temporal patterns of parturition were discernible for many species, the hypothesis that most birth would occur during the restive phase was not supported. However, a significant positive correlation between level of predation and the likelihood that birth would occur during the restive phase was found (n = 0.40). These findings are discussed in terms of predator/prey interaction and the protection (or lack of protection) that restive birthing might afford the mother and offspring at this vulnerable time.
