Grief elaboration in families with handicapped member.

Families with handicapped member seem to follow the same five stages (rejection and isolation, anger, dealing with the problem, depression, acceptance) of Kubler-Ross grief elaboration theory while dealing with the narcissistic wound of a handicapped child. Some of these families show a block in one of the stages. The effort of psychotherapy is to remove the block and let them reach the last stage. In this paper families under systemic psychotherapeutic treatment are analyzed, who had in common the birth of a child with low or modest invalidating signs and psychotic or autistic features. The families structure did not show the characteristics of a psychotic family. Nevertheless either one or both parents ignored the evidence of their child disease and they built a "disease-incongrous" wait around the child, trying to push away the painful reality. The authors explain the importance of this approach for the improvement of the autistic traits.

Spiritual dimension in the dynamic relationship with the "helping professions".

The attention given to the spiritual dimension in the "helping professions" practice represents a major step for a coherent approach toward the ethical aspects in the resolving individual and familial conflicts; furthermore they can be used as a key in understanding dysfunctional relationship. Psychiatry interest for religion and mysticism must be considered within a wider framework of cultural changes of our society, from which neither religion, understood as a system of faith, nor medicine, as an answer to human suffering could remain indifferent. The evaluation of this aspect prevents the occurrence of "burn-out" syndrome in the "helping professions" workers, because it would give them the opportunity to a greater spiritual growth with expression of empathy, respect and better understanding of help request from their patients.

Carcinoma of the cervix, endometrium, and ovary: an 11-year review.

Over an 11-year period, 164 patients with genital tract carcinoma were managed at the Austin Hospital, Melbourne. For patients receiving primary treatment at the hospital, the five-year survival rates of patients with carcinoma of the cervix, endometrium, and ovary were 47.8%, 41.6% and 4.3% respectively. Improved results rest upon earlier diagnosis and alternative therapeutic measures, especially for carcinoma of the endometrium and ovary. Routine surveillance after initial therapy may detect asymptomatic metastatic disease when curative treatment is still possible.

[24 hours plasma tryptophan levels in patients with endogenous depression (author's transl)].

The variations of total and free tryptophan during 24 hours were measured in patients with endogenous depression. In depressive patients, plasma tryptophan levels are significantly lower than in control subjects. Plasma tryptophan levels in depressive patients are low during the night (when high at the same hours in normal subjects). The physiopathological mechanisms of such alterations and their possible influence on symptomathology are described.

Circadian rhythm of plasma tryptophan in clinically healthy subjects and patients with endogenous depression.

Variations of plasma tryptophan along the 24-h scale are studied in clinically healthy subjects and in patients with endogenous depression. In both groups a circadian rhythm in free and total tryptophan has been found. In healthy subjects plasma tryptophan levels were higher during the night in comparison with daytime hours; the opposite was demonstrated in depressed patients. The possible physiopathological mechanisms underlying the different behavior of plasma tryptophan in the 24-h span, in these two groups of subjects, are discussed.
