[Phoniatric aspects of reconstructive laryngectomy].

Reconstructive laryngectomy has been performed at the E.N.T. Clinic in Ferrara for some time. After surgery the patients undergo speech therapy and phoniatric treatment for a period of time varying from 2 to 6 months. In order to better evaluate the vocal quality obtained after such reeducation the vocal emissions of 25 patients were examined and recorded. The recorded material, made up of prolonged vowels and 6 phonetically balanced sentences, was then evaluated by a panel of 7 "trained" listeners. The evaluation score-card proposed by Woiers in 1977 was used in taking the data. This not only provides a scale for evaluating the main voice quality features, but also includes a final judgement on parameters including intelligibility, acceptability and pleasantness. Statistical processing of the data inherent to voice quality indicated a decrease in intensity and pitch when compared to normal values. Nonetheless, the listening test showed a high degree of intelligibility, acceptability and pleasantness. These values confirm the fact that, although the new voice achieved through reconstructive laryngectomy surgery is less sonorous, it allows for perfectly understandable, socially acceptable speech.

Effect of low doses of interleukin-2 injected perilymphatically and peritumorally in patients with advanced primary head and neck squamous cell carcinoma.

For 10 days, four patients with advanced squamous cell carcinoma of the head and neck received two daily injections of 100 units of natural IL-2, one around the tumor, the second near the tumor-draining lymph nodes. No side effects were observed. Histologic and ultrastructural examination of tumor and lymph nodes recovered at surgery showed that in many instances neoplastic cells were intermingled with numerous lymphocytes and eosinophils. Lymph nodes displayed hyperplasia of both cortical and paracortical areas and epithelioid venules infiltrated by lymphocytes and granulocytes. In a few cases, a decrease or disappearance of neoplastic lesions was also documented both clinically and histologically.

A personal experience with subtotal and conservation surgery as treatment for laryngeal cancer.

A personal technique for laryngeal cancer reconstructive surgery is presented and discussed. The functional and therapeutic purpose of this surgery is to broaden its indications and to improve functional results. In particular, our surgical technique involves removal of the soft internal part of the cricoid cartilage (mucosa, submucosa and perichondrium), which is otherwise conserved. Satisfactory functional results can be achieved by: modeling of two symmetrical pseudoarytenoids; an anterior epiglottiplasty or the use of a Hiranandani base-of-the-tongue flap to close the anterior gap (if present); muscular flap lateral-plasty avoiding a cricoidhyoidpexy.

Occult metastases in cancer of the larynx and their relationship to clinical and histological aspects of the primary tumor: a four-year multicentric research.

The authors have tried to verify whether some clinical or pathological features of laryngeal cancer may favor the occurrence of occult metastases in the lymph nodes of the neck in N0 cases. The purpose of the investigation was to define the possible existence of tumors, where elective neck dissection, in the absence of palpable nodes, could be done without, thus contributing to settlement of a long debated problem. Different parameters, both clinical and pathological, have been considered. The result of a cross comparison of such parameters in 237 cases of cancer of the larynx without palpable nodes, submitted to surgery on the primary and elective neck dissection, indicates that, although the frequency of occult metastases is significantly related to the site and size of the tumor, its grading, and the degree and type of stromal reaction, the favorable concurrence of all said parameters occurs in 2% of cases only. However, if only clinical parameters, viz. site and size of tumor, are taken into account, a favorable concurrence occurs in 18% of cases with an incidence of occult metastases lower than 2%. These cases are almost exclusively represented by T1N0 supraglottic and T2N0 glottic tumors. We can conclude by saying that clinical and pathological preoperative findings may offer a useful clue in evaluating the risk of occult metastases, and hence advising, or not, an elective neck dissection in N0 cases. Nevertheless, in the vast majority of cases, an elective functional neck dissection still offers the best guarantee of oncologic safety while avoiding unnecessary mutilation.

Supraglottic surgery in vestibular cancer.

This paper deals with the experience acquired in the E.N.T. Clinic of the University of Ferrara regarding horizontal supraglottic laryngectomy in vestibular cancer. The anatomical background of this functional surgery and the most important principles of the technique are specified. The results regard 121 cases (13 T1 cases, 93 T2 cases and 15 T4 cases) divided into three groups: 1. patients who had only surgery (68 cases); 2. patients who for various reasons also had complementary postoperative radiotherapy (39 cases); 3. patients with extralaryngeal expansion who had preoperative radiotherapy (14 cases). The high overall survival rate for the 121 cases is particularly significant, as is the shortening of the high-risk period to less than the usual 5 years. The 4-year cure rate is 78% and remains unchanged right through the 7-year follow-up. The authors feel that these data demonstrate further the validity of supraglottic laryngectomy when its precise indications are rigorously observed.

Verbal tests of central audition.

The use of sensitized verbal tests in the diagnosis of central deafness is critically reviewed. The concept of intrinsic (neural) redundancy is the main point in the operational model; different modalities of coupling between intrinsic and extrinsic redundancy bring forth different patterns of intelligibility. The age of the subject thus becomes critical; on the contrary, the influence of the IQ can be minimized by carefully selecting the tests. These can be divided into monaural and dichotic; among the latter, only the tests involving competing information are of value, whilst the so-called summation tests should now be disregarded. The clinical results are only briefly quoted, since they are well-known. The contribution to the advances in neurophysiology seems to be more interesting. The influence of reticular formation on auditory stimulation, the crossing of the auditory pathways, the existence of a secondary auditory area have been psychoacoustically confirmed by the use of the monaural tests. In their turn, the dichotic competing message tests have enabled a beautiful model of language perception to be drawn, and have definitely demonstrated the so-called hemispheric specialization (or dominance) for verbal and nonverbal messages.
